For Failed MitraClip, Next Procedure Might Depend on MR Etiology

As MitraClip procedures continue to expand as an option for nonsurgical patients with functional or degenerative mitral regurgitation (MR), a new question will increasingly arise: what’s the best approach for treating patients whose clip procedures fail?
Now, a small, retrospective, single-center study is trying to shed some light. As Hannes Alessandrini, MD (Asklepios Klinik St. Georg, Hamburg, Germany), and colleagues write in
EuroIntervention this month, the best approach may depend on the source of mitral valve dysfunction in the first place.

Should Cardiologists Be Performing Stroke Thrombectomy?

It’s possible to start a successful stroke thrombectomy program in an experienced cardiac cath lab when cardiologists, neurologists, and radiologists work together closely, the experience at a single center in Prague, Czech Republic, suggests.
In the 7 years after the program launched, the team achieved outcomes similar to those seen in randomized trials conducted at experienced stroke thrombectomy centers, lead author Jakub Sulženko, MD, PhD (Charles University and University Hospital Kralovske Vinohrady), and colleagues report.
A good functional outcome defined as a modified Rankin Scale (mRS) score of 0 to 2 at 90 days was observed in 47.9% of patients, right around the 46% rate from a meta-analysis of five randomized stroke thrombectomy trials. There did not appear to be a learning curve. ....

A significant proportion of real-world patients with severe aortic stenosis and low surgical risk have an additional valvular or CV condition that was never accounted for in pivotal trials of TAVI versus SAVR and that requires a serious discussion by the heart team, Canadian research shows.
In the study of primarily low-risk patients who underwent SAVR between 2000 and 2019, 40.8% had at least one of the following conditions: non-tricuspid aortic valve (NTAV), severe coronary artery disease (SevCAD), concomitant mitral/tricuspid valve (CMTV), or concomitant ascending aorta replacement (CAAR). All four of those patient subgroups were routinely excluded from RCTs of TAVI compared with SAVR. ....
