systematic failures and egregiously poor decision-making resulted in more than an hour of confusion and chaos before police finally confronted and then killed the gunman who had murdered 19 children and 2 teachers. the victims family members got a copy of the report before it went public. vincent salazar, whose granddaughter, you probably remember her picture, was among those young children killed. he reacted with skepticism. we re not going to get the truth. because there is cover-up. everybody s throwing everybody under the bus. the only ones that ain t under the bus is because they re six feet in the ground now, and that s our children. and the two teachers. it is unimaginable heartbreak for these families. it goes on and on. also detailed in this report, major, major lapses in school security leading up to the shooting, including broken locks and a, quote, culture of noncompliance with safety policies. also even poor wi-fi that may have delayed the lockdown alert. we
investigative committee finally has been released and outlines a series of failures by law enforcement. the 77-page report criticizes a lackadaisical approach by the nearly 400 officers who ended up at the scene of the school shooting. for the first time, we are seeing hours of body cam video. it captures chaos, confusion, and excruciating delays, as critical minutes were ticking by and no law enforcement charged into that room. much more on that in a moment. but let s begin with athena jones in indiana. what are you learning? reporter: this is remarkable. this took place in a mall s food court at about 6:00 p.m. on sunday night. three people were killed by this shooter who came armed with a long gun, and authorities still don t know what led to it, whether these people were targeted. they re looking into that, looking at surveillance video, and further investigating it. but in this case, the police chief of greenwood, a suburb of indianapolis, pointed to what he called a

lives? reporter: ryan, in the report it does mention a few lines about why officer waited. it states they were waiting for the keys. they were waiting for more equipment. but let me take you through it because there are very telling details we did not know before reading this report. according to this report chief pete arredondo actually wrote the active shooter policy. he wrote in himself as the incident commander but, according to this report, he didn t take on that role on that ill-fated day. it says that he not only didn t take the role, he didn t transfer command of the scene to any other officer which, according to this report, could have been done in this case in this jurisdiction. now the report is much broader than this. it does not just point the finger at arredondo. it also says that there were hundreds of police officers who responded to the scene, and it points out those trained police officers then did not question that command. they didn t question that there
