Was caused by inadequate traffic signs. pam thats according to the National Transportation safety boards report. Which was released late today. Grant lodes is here to break down the specifics. grant this was on highway 101 in san jose in january of last year. It was dark and raining at the time. The ntsb says the bus was exiting the freeway and because markings were worn or missing. The driver thought he was in the exit lane. It turns out we was not and went up a barrier. Rolling the bus. It wound up on its side. The ntsb says the barrier should have been visible. Even in the bad weather. But it was not. The ntsb faults the state for not properly marking the roadway. Also, investigators say the bus had seatbelts but only two of the 21 passengers were wearing them. The two passengers who died in the crash were not belted, and were thrown from the bus
