As the ntsb issues its final report on the smoke crisis that killed one woman and sent 91 people to the hospital. One ntsb member says metro has a fa to keep it from learning from past mistakes. The ntsb chair says the safety board has been pointing out the same problems for 33 years. Wusa 9 bruce leshan has been covering this. And joins us from the ntsb. I doubt seriously we have heard from this. Reporter and its really extraordinary just how many problems the ntsb discovered. One of the most disturbing, metro would send trains, loaded with passengers into tunnels to investigate reports of smoke. The new metro ceo says that practice has stopped. Reporter the ntsb released what it says is the most likely cause of the smoke crisis. A prolonged electrical short, like this one. A short caused by ineffective inspection and maintenance. Bad management and inadequate fires lacked preparedness, one ntsb member called it a comedy of errors. It is absolutely essential that these lessons be learned. Reporter this has been causing so much of the arcing issues. On this side, it is sealed. This side, open. You can see how contaminants would get in there. Can you ensure people that those sleeves are being installed from now on . Yes, definitely. Reporter the ntsb also found the smoke detector, closest to the fire, was inoperable. That the operator had to wait long minutes for approval to shut off vents in the train. And then only shut them off in the lead car. And that the Operations Center triggered fans that actually concentrated smoke around the hundreds of trap passengers. The smoke we frustrating to the ntsb is after 33 years and 101 safety recommendations, the same problems keep recurring. The ntsb chair hopes this time is different. But if the new leadership has already shown willness willingness and ability to take unprecedented steps to make it safer. Which is what were all about. Reporter the ntsb says that the threestate safety oversight of metro is unique. And uniquely dysfunctional. It wants the federal Railroad Administration to take over. But the department of transportation has designated the federal Transit Administration to babysit safety at metro for now. The ntsb says safety is not a job for babysitters.
