An independent review of a technology failure that led to the loss of 413,000 records of evidence from the UK’s Police National Computer (PNC) has found a lack of reviews and affective testing contributed to the debacle.
Although the lost data has all been recovered, according to Kit Malthouse, minister for crime and policing, an independent investigation into the incident, led by Lord Bernard Hogan-Howe, has found that established procedures, such as reviews, were only loosely followed or in some cases not followed at all, during the introduction of the faulty script. Hogan-Howe is a former head of London s Metropolitan Police. ....

Priti Patel 'considers senior QC or retired judge for probe into VIP paedophile inquiry'


A source said Patel is ‘very anxious’ to do something.
Earlier this week it was reported the police watchdog could consider reopening its probe into how officers conducted themselves during the failed Operation Midland if new and compelling evidence was presented, its boss has said.
Michael Lockwood, director general of the IOPC, told MPs the body is now allowed to reopen cases if fresh information indicates its original findings were flawed.

Speaking to the Commons Home Affairs Committee, he said: If somebody provides me with compelling, new information - significant new information - which would come to the view that the original decision was flawed, then I would scope that, talk to the individual parties and make a decision (as to) whether that is something we do.
