Challenger Disaster Whistleblower Allan McDonald Dies

On Saturday March 6, 2021, an 83-year-old man living in Ogden, Utah died from the effects of a fall. His name was Allan J. McDonald, and you may never have heard of him, but he did something remarkable.
Allan McDonald received a degree in chemical engineering in 1959 from Montana State University, then went to work for Morton-Thiokol, who manufactured the rockets for the Minuteman intercontinental ballistic missile and later, the solid rocket boosters (SRBs) used by NASA in its space shuttle.
After 26 years with the company, McDonald was on site at Cape Canaveral, Florida to oversee the solid rocket boosters that would power the shuttle Challenger on its tenth flight, which was also the twenty-fifth Space Shuttle flight. ....

Of the 15 astronauts to be killed after launch, 14 were American, and with the exception of one, Michael J. Adams, they were all killed in space shuttle missions: the Challenger launch disaster and the Columbia re-entry explosion. This week, America and the world looked back at the 35 years that have passed since the Challenger tragedy.
January 28, 1986, was the date when the space shuttle Challenger, one of the four such spacecraft to be assembled by NASA and its partners, was supposed to take off in the 25th overall space shuttle mission. The moment came after several days of delays caused by logistical issues, a malfunction of the exterior access hatch, and bad weather. ....

US space shuttle Challenger explodes after take-off – archive, 1986 | The space shuttle

Nasa said last night that it had suspended operations of the shuttle indefinitely, pending the findings of a board of inquiry. But in a televised address to the American people, President Reagan stressed that the space programme would go ahead.
A spellbound nation watched with high expectations as Challenger lifted cleanly off the launch pad at 11.38am, trailing the normal blazing geyser of fire behind it.
Some 60-seconds into the launch, as the elegant spacecraft soared nine miles into the deep blue sky above Cape Canaveral, it turned into a ferocious fireball as a bolt on the main fuel tank exploded, detonating an eruption with the power of what was described by Nasa scientists as a ‘small nuclear weapon.’ ....
