Butler, Blue Cross partner on program aimed at reducing readmissions

PROVIDENCE – Butler Hospital and Blue Cross & Blue Shield of Rhode Island are collaborating on an innovative new program designed to reduce hospital readmissions by improving behavioral-health outcomes of discharged patients.
“The Butler Hospital Transitional Outpatient Program intends to provide short-term care to patients immediately following a discharge from an inpatient stay,” Butler and the insurer said Thursday in announcing the program. “The short-term care efforts will serve as a bridge to longer-term care.”
The program’s ambition is a 5% reduction in the number of readmissions.
“We are excited to be able to provide this innovative new clinical service to patients to ensure they have the care they need following hospital discharge,” said Brandon Gaudiano, clinical director of the new program, which, he noted, “follows a team-based approach and offers an increased level of support at a critical time to prevent relapse and help patients get bac ....

RI Foundation gives record grant amount in 2020, much to help COVID need

The Rhode Island Foundation awarded a record $87 million in grants to more than 2,200 nonprofit organizations in 2020, the foundation announced Wednesday.
In a year of great need because of the coronavirus pandemic, the foundation received $68 million in donations, the third-highest total in its 105-year history. Gifts ranged in size from $1 to several million dollars.
We are grateful for the passionate and committed donors who have partnered with us for 105 years to take on the challenges and issues of the day, said Neil D. Steinberg, the foundation’s president and CEO.
“Working with nonprofit organizations to support their critical services in the community, inspiring donors, and providing leadership around crucial issues all played a role as we worked to meet the needs of all Rhode Islanders during a year unlike any we’ve experienced before, Steinberg said. ....
