A lot of fliz yoridians are wai to see what the next step is going to be. Ron desantis says there will be baby steps so theyll move into opening slowly. He does rely heavily on the sugges suggestions of the president. They were siding those projections for potential death tolls, thats when desantis apriled t issued the stayathome order on april 1st. We do know desantis is facing questions of why hes waiti wai long to issue the stayathome orders. We see governors playing against the white house. All of this comes one day after yesterday in the rose garden the president was acknowledging those complaints and concerns that we heard from governors for weeks about the ability to do enough testing to reopening. It is a big question of whether or not what the white house roll out is going to be enough, a lot of Health Experts say it is going to be higher than every state testing 2 of its population. We see how the administration and the president respond to those concerns from Health Experts. Very much looking forward to hear from the president and governors seeing if they are leaning forward being cautious. Lets start with the president in florida. Neighboring georgia has reopened and i know a lot of people are waiting. I know you are waiting to see ten days or two weeks from now will the infection rate go up. If you look at florida, the 14day trend is the Third Largest state in america. If it starts to reopen, it will be a huge experiment in whether this can work. My read is not yet. It is mostly flat but a couple of spikes that would make me think lets wait and get another week or ten days. How do you read it, professor . We need testing. We need Situational Awareness and make decisions based on this kind of Situational Awareness. Right now we have no idea how many cases there are and we dont have the Contact Tracing in place and testing does not come back fast enough. The tests are not validated. Results are not always clear. I think this really needs to be resolved before we move forward or as dr. Fauci have said to us, well be well risk all the gains we have made through all this collective sacrifice. It is remarkable. We had this same conversation about a week ago that we are still flying blind because we dont have enough testings to understand the deaths of the problem of these community. You mentioned dr. Fauci, the white house had another event yesterday. The president says i dont know what the governors are talking about, they already have everything they need. He questioned the intelligence of a couple of them. Dr. Fauci said this. I said it before so i am not creating any new dialogue here. Testing situation has improved rapidly. Whether or not we are exactly where we want to be . That remains to be seen. I think we are there, david, or getting close to being there. This is your wheel house, professor rimoin, are we very close . Dr. Fauci knows what it is like for people to say we are there and on the ground realizing you dont have what you need. He says maybe this is a week or so. We know on the ground that people still cant get a test. People unless who they have acutely ill or there is a lot of confusion on the ground. Where to get test or how to get test or how sensitive these tests are. The big issue is we know swabs and reagents, these chemicals you need to do the tests are in high demand and not always available. The reality on the ground is very different than theoretically, maybe possible or coming down the pipe. Well, let me try to get into the theoretically and on the ground in the sense of this. The white house often says especially the number is going down, look our social distance guidelines are going and projected american deaths are going down but now it is going up. We are also seeing signs of mobility data that people are getting more active and thats also feeding into our assessment. If more mobility and more people moving around right now, the more americans will die. Do you reopen the economy and more of your people will get infected or likely to get sick or some of them likely to die or you wait a couple more weeks . Again, it is all about having what we need in place to be able to reopen. Everybody wants to reopen. It is necessary that we get to a place where we can reopen. What we need to do to be able to do so is throw everything we have got behind getting the testings and tracing and all these places in place. Facts are our friends, people want to make decisions they want to know the facts without spinning or opinions. Thats what we have been giving them. Total hospitalization is down a tick which is good news. The change in hospitalization on a rolling total is down. Number of intubation is also down. The number of covid per day, these are new people who are diagnosed with covid. It is under 1,000 which is good news. It is still a significant number of people, 900 people. After all of this we still have 900 new infections yesterday. On a three day rolling average. Overall you see the numbers are coming down. Thats good news. This is the worst news. Everyday we think when the nightmare is over. 335 people passed away yesterday from this virus in the state. It is 335 families. You see this number is basically reducing but not at a tremendous rate and the only thing tremendous is the number of new yorkers who still pass away. Everyone is talking about reopening. I get it. Your kids are staying home and it individual families cant sustain it and our children cant sustain it. When we talk about reopening, this should not be a political discussion or a philosophical discussion, should not be because people are protesting or some people want it or dont want it. It is a factual discussion on reopening, right . So lets dymistify it a little bit. We want to reopen but we want to do it without in fefecting more people or overwhelming the Hospital System. Our great fear was the numbers f of people infected would overwhelm the hospital capacity. Thats the balance. Reopen but dont increase the number of infected people and dont want to overwhelm the hospitals system. Well redesign that system in reopening. You can factually with data design a system that does just that. Thats what government is supposed to do. Government is not about spouting political or philosophical opinion. Government is about running services, designing programs that actually work for the people to address the problem. In this situation we can measure, we have data, we have facts, so measure what is happening in society and calibrate your reopening to those measurements, right . We are adopting a set of rules and guidelines of reopening and restudying plans all around the country. We spoken to every expert on the globe who have been through this before and we come up with factual data points to guide us on reopening. First point, dont overwhelm the Hospital System. If the Hospital System in an area exceeds 70 capacity which you only have 30 left or the rate of transmission of the virus hits 1. 1, those are danger signs. We know that. Remember hospital capacity, if you are a 70 at your hospitals is a twoweek lag on this virus. If you hit 70 , you can expect the number to go up for the next two weeks as people just got infected get ill and some of them coming to the hospital. 70 is a safe metric to use for your hospital capacity. If the transmission rate hits 1. 1, thats what they call outbreak. That means it is going to spread much, much faster. You would not start reopening unless you have a transmission rate below 1. 1. Really below 1. If it hits 1. 1, that means you are in trouble. So those are the two main data points. If you look at the state and this state is different than most states. This state has new york city one of the most denser area on the globe and then we have upstate new york. If you look at our infection rates, upstate new york is different than the state down new york. If you look at the rate upstate new york, it is comparable to many states in the midwest and the west. We hear the discussion everyday. Some states are opening because they dont have that bad of a problem. Some of the places upstate have a problem thats comparable to state in the midwest or the west. Much different than new york city. Okay. Lets come up with data points, factual points of what we have to do to reopen so everyone has the same opening template that we are dealing with. We have to be smart about this. Again, i know it is emotions and people are feeling emotional. Emotions cant drive a reopening process. We are talking about infection rates or hospital capacity. Separate the emotions from the logic, we have to act like our logical selves here. Thats what smart means. Dont be political or dont get pushed politically into a situation, protesters are in front of the capitol, we better reopen. No, we are not going to open. No, thats not how we make decisions. The first point is cdc set guidelines as to reopening for states. We think those cdc guidelines made sense which you have to have a 14day deline cline in t number of hospitalizations before you go forward. Second, identify industries that you can start reopening that will bring people back to work and get the economy going but you know you can do the appropriate precautions and social distancing. So phase one we are talking about construction and manufacturing industry, right . Those are two industries that employ a lot of people. We believe you can put the right precautions in place and learn the lessons from where we had been. And say to those businesses, this is not just about government. Say to the businesses tell us how you are going to incorporate the lessons that you just learned. How you incorporate social distancing or fewer people into space so reduce density. How do you have the right ppe and how are you going to monitor . Are you taking temperatures of everyone who walks in . Thats for businesses to decide also. Make sure you dont have attractive nuisance. It is a legal term. It is not the right use of a term. Attractive nuisance in this context, you open up a facility that can bring infections for you. Make sure you dont open up something that brings hundreds of people on the outside in. What business precautions will those individual businesses take . Watch the healthcare capacity. Your Healthcare System could not go over 70 capacity. Again, if you are at 70 , bells should go off. Dont go over 70 in your icu beds. Many of the people who come in with covid need an icu bed because respiratory illness. Almost at the heat of this, almost every hospital turned into an icu bed. Thats why we needed the ventilators because these people who get seriously ill with covid need that level of care. You have a flu season coming up in the fall. The number of hospitalizations normally goes up in the flu season. Anticipate that, stockpile the equipment. We learned a lot of painful lessons here. O one, you have to have the masks, gowns and there is an International Demand on it. Make sure we have a stockpile reserved of the ppe. We have to have testings. How many tests . Dr. Birx recommend 30 for 1,000 people. Different people and different record and different numerical percentages. I think we start with that. Do we have enough testing sites. How long it takes to turn around the test and we advertise to people this is where you go and this is what you do to get a test if you think you may be infected. The whole thing with keeping that infection rate down is find a person whos infected, let them know it and trace and ice isola tracing is once the person says they are positive, you trace their contacts back, you notify people, you test people. Thats a whole different operation. The current recommendation is you need at least 30 tracers per 100,000 people. We have to have isolation facilities in place. When someone gets sick, they dont want to go home to quarantine because if they go home, they could infect their family which is whats happening now with a lot of these new cases. We have to have a facility where somebody is positive can quarantine for the two weeks without going home. We have to coordinate regionally, schools and network and testing tracing. This all has to be coordinated on a multicounty effort. We have to reimagine te telemedicine. We have to have regional control room that monday tos aitors all indicators. We have to have one Central Source thats monitoring all these dials and hit the danger button so you can actually slow down the reopening. We have to protect and respect essential poworkers which i wil talk about in a moment. On businesses, safety precautions. Social distancing. Today we are announcing an Advisory Board that is made up of statewide Business Leaders and academic leaders and Civic Leaders who are advising us on this and they have been for week and i want to thank them very much. Manufacture construction as the first phase, businesses thats 46,000 jobs in places like Central New York. It is a major employer. These businesses can adopt to the new normal in terms of their employees and in terms of the processes they put in place. On the healthcare capacity again we just lived this. We cant be in a situation where 70 capacity is exceeded. You need at least 30 buffer on hospital beds and you need 30 of icu beds available if that number starts to tick up. In terms of testing, we have to have the testing regiment in place and we have to prioritize the people who get tested. Simil simil symptomatic , individuals came into contact with a symptomatic person and front line essential workers. They do have a higher rate of infection because they are putting themselves in harms way and we want to make sure they have the testing so we have an earlier alert system. You have to have the right number of sites. Testing wont work if it is impossible to get. You have to have the right number of sites for the area you are dealing with. The advertisement is very important. It has to be available. People have to know it is available and they have to know what the symptoms are that would have them go get tested. This is about people understanding it and people buying into it. This is not government orders, this is people get it and they know the facts and they know what they are suppose todd d tod they do it because we have communicated successfully of the circumstances and facts. You need that testing and trace the contact otherwise you see that infection rate increase. On the tracing, the estimate is 30 tracers for every 100,000 people. So thats a data point. Isolation facilities is a proportionate number of people who test positive who say i cant go home or i dont want to go home or infecting my family. Isolation facilities available for those people. And then the regional control room where you are monitoring all of those metrics and monitoring hospital capacities and rate of infection and how businesses are complying and it has an emergency switch that we can throw if any one of those indicators are problematic. We have gone through hell and back over the past six or so days. What we have done is really tremendous. What people have done and the American People have done and new yorkers have done has been to save lives literally. We have to remain vigilant. This is not over. I know as much as we want it to be over. It is not over. We have to respect what we accomplished here. When they started this, the projection for this state were 120,000 new yorkers would be infected and hospitalized. Only 20,000 were infected and hospitalized. How could they be so wrong . They were not wrong. We changed reality. The differential variance is what we did. It is the closed down and Wearing Masks, we reduced the rate and flatten the curve. Well, that meant 100,000 fewer new yorkers didnt get seriously ill or did not go into the hospitals or overwhelm the system. We literally saved lives. We cant negate everything we have accomplished. We have to do the opposite. We have to take this experience and we have to learn and grow from the experience. We have to build back better than before. As a society and as a community. We need better systems. This expose a ld a lot of issue. We have to do a better job on teleeducation and remote learning. Sounds great. You have to have the perfect equipment and people have to be trained. We have to do a better job on te telemedicine. We have to do a better job in our basic health system. The virus was in china last november and december. Why didnt someone suspect well maybe the virus gets on a plane last november or december and lands in the United States the next day . Everybody talks about global interconnection and how fast. Everybody knows there is a virus in china last november december. China says dont worry, we are taking care of it. All you need is one person to get on a plane. As it happened, one person got a plane and it went from china to europe and europe to new york. The flights from china goes to the west coast and the flights from europe goes to the east coast. We got it through europe. Where was the whole International Health community. Where was the whole host os of experts, w. H. O. , the whole alphabet soup of agencies. Where does the Intelligence Community with the brieferings saying this is not china. You can get on the airplane and come to the United States. Governors dont do global pandemics, right . National Health Community will do that. Where are all the experts and where was the New York Times and wall street journal and where were all the blowers who should say be careful, there is a virus in china may be in the United States. We are sitting here january, february, still debating how serious this is. Agai again,it is not a states responsibility. But, in this system who was supposed to blue the bugal and didnt. I would bank that this would happen again and tis the same thing going to happen again . I hope not. We have to figure these things out. We have to remember that as a society and as a community, we are about government and syst systems. We are about values. What makes us who we are, are our values. Point number 12, protect and respect essential workers. I had two nightmares when this started. One, that i would put out directives on what we need to do and 19 million new yorkers would say i have not been convinced and i am not going to do it. We are going to close down every businesses and you have to stay in your home. The most disruptive Government Policies put in place. I cant even remember or seen the history books government was more disruptive to individual life. No businesses, everybody stays home. No schools. What else if new yorkers said we are not doing that. We are not doing that. Too much. It is an over reaction. It is political, everything is political nowadays, right . So easy, oh, thats just political. That was a fear. If new yorkers did that, governmentally had no ability to enforce 19 Million People staying in their homes. Thats why communications was so important. Give them the facts, give them the facts so they understand why. Second nightmare was what if these essential workers dont know up . You have to have food and transportation and lights have to be on and some have to pick up garbage. What if the essential workers said i am not showing up. You communicated so effectively, the fear of the virus that essential workers say yeah, if everybody is staying home, i am staying home, too. That could have happened. I went through the h. I. V. Virus when it started. People were petrified. People knew how it was or how it lived or how long it would transfer. People were petrified. What happens if essential workers he or she says i am not going to show up to run the bus. You dont pay me enough to put my life in danger. I am not doing it. They showed up. They showed up. I just finish communicating how dangerous this was and convince 19 Million People to stay home and close schools and businesses and the social workers still showed up. Thats a value. They didnt show up for a paycheck. They didnt show up because government asked them show up. They didnt show up because their employers said i need you to show up. They showed up out of their values out of their honor and dignity. Thats why they showed up. My grandfather, little italian immigrant, andrea cuomo, no education, came here the classic immigrant story, whines up having a Little Grocery store in south jamaica, queens, during the depression he almost lost the store. He loved to tell story. Why did he almost lose the store . Because finances and depression. No, because he gave away food during the depression and he would not let anyone be hungry. A family would come in and nobody would have money and he would give them food. And, he was giving away so much food that he had problems paying his bills. Gave him a lot of stress and he whined up having a heart attack as a young person. No one told him to do that. That was just his values. I would ask him about it afterwards. Grandpa . What am i going to do . Let them go hungry . I am going to let somebody go hungry . That was unimaginable to him. He was an essential worker nobody called him an essential worker but hes an essential worker. Thats what people are doing day in and day out here. The person delivers the groceries. The person driving the bus, the person thats driving the subway. The nurses and the doctors and the police. All these people who are showing up everyday. Not because of the check. They can stay home and file for unemployment. No, theyre doing it out of their sense of honor and sense of dignity and sense of pride. This is their mission, this is their roles. The firefighters. Everyone is petrified. Tha youre going to be a Police Officer and pulling people out of their car and go into the house for domestic disturbance and you wrestle in the white house and you dont know who it is. I want to thank healthcare workers. It is not Just Healthcare workers, it is all the people who have been out there all this time making sure everyone else can stay home. They have higher infection rates and theyre getting paid a minimum amount of money. They have familiy at home they are suffering but theyre doing their job everyday. As i talk about opening, protect our workers, they need testing and respects the social wo essential workers. Public transportation kept on running because they needed to get to work. We talked about closing Public Transportation. They said forget it. Thats how nurses are getting to work and nobody will be in the hospital, nobody will be there to deliver the food. Nobody will be in the power plant to keep the lights on. Nobody will be in the department. Public transportation permissive vit is vital for them. Theyre making sure it is safe and is infected. The New York Daily News ran a story today on the Public Transportation in new york city. And, the front page is a picture of a subway car filled with Homeless People and their belongings. Respect the essential workers. Thats disgusting whats happening on those subway cars. It is disrespectful to the essential workers who need to ride the subway system. They need to ride the bus to get to work. We need to have a public system thats clean and train our disinfected. We have Homeless People on trains. It is not even safe for the Homeless People to be on trains. No face masks. You have this whole outbreak. We are concerned about Homeless People so we let them stay on the trains without protection in this epidemic of the covid virus . We have to do better than that and we will. Well learn from this and well be better than this. We are new york tough, tough means not just tough. Tough is smart, discipline and unified and loving. Thats who we are. Thats what we are. Thats why we got through this as well as we have thus far together buecause of our values respect and dignity and mutuality and our love for one another are willing to sacrifice and because we are fortunate where we have many, many heroes in our midst and not because they have metals on their uniforms but because they have honor in their souls and they have strengths in their character and they have dignity and pride in themselves and because they show up everyday. They make sure everyone is protected. They have to be at the top of the list. Theyre going to be the top of the list in the next generation of whatever this is. Theyre going to be at the top of the list at the golden gate. They deserve our respect and protection here. Theyre going to get it. Questions . Govern. Governor cuomo is at syracuse today talking about his briefing. The governor making clear he believes new york is on the path to progress. May 15th is how long his pause is in effect. Talking about reopening process and saying he wants to make sure the state gets this right. We cant negate everything we have accomplished and also saying emotions cant drive the process. Upstate where he is today, Central New York has a lower infection rate than new york city. Constant talking about testing and the need to ramp up testing. New york also had we heard the numbers yesterday of an interesting series of Antibody Testing to try to determine who already has been exposed to the coronavirus. Right now there are 8 antibody tests thats given by the fda. Thats a handful. Berkeley and uc San Francisco are working to evaluate some of these tests and revealing whos still at risk or been infected. Joining me now two researchers dr. Alex marshan and dr. Patrick hsu. Dr. Marsha, let me talk to you first. Yesterday to hear Governor Cuomo says they have done testing. The question is and this is what you are trying to get in this research. Do you trust those numbers and these tests . These numbers are crucial for understanding this pandemic. There is now a wide variety of different tests coming in. Theyre showing up at peoples doorsteps and there are different types of tests and each one have their own characteristics. We team up to ask basic questions, do these different tests give us reliable information. We decided to take a standard size of sample even people we know are infected with coronavirus or blood samples that were drawn before the outbreak going back to 2018 and before. Just compare head to head and see how all the tests that are starting to become available before. Do they give reliable information of a connection interpret. Thats what we set out to do. If i am watching this at home and is it safe for me to go back to work, have i been exposed and do i have hopefully some limited or immunity, do you have any sense or when will you know if some of these tests out there, i am going to call it bogus, maybe na theyre not designed to be. Do you know and what percentages of these tests are good and helpful and what may have been rushed on the scene too soon. There is a big market out there for sure. We find tests really good and other tests may not be as good as you may need. Our goal was to do this head to head count off, to have a tier thats reliable. There are some tests that are quite sensitive and have good specificity as well. Dr. Marson, the numbers are important. When you look at the 25 , do you trust that number, do you think it is a ballpark number because we are not sure of reliability test yet . Just to backup for one second. I think it is critical for viewers to understand. Before we talk about reliability and numbers. One thing no one knows for sure and we and no one else can tell you, even if these tests are reliable and we see the presence of antibodies, that does not tell anybody they are safe to go back to work. We do not know any of these tests will tell us whether someone can get infected in the future. Thats a critical point. Now each tests have its own r s characteristics and we have to go look at the actual test which we have not done. The general process that people will need to gather information of how many people are exposed and antibody tests in general are a powerful way to do that if we know the number interprets false positives.
