of the strictest covid countermeasures many of them have ever seen as the world confronts a second olympic games amid a pandemic. cnn's selena wang is live inside tokyo with more now. how many spectators will there be, and how will the government be choosing them? >> reporter: well, ana, it's hard to believe we're only 17 days away from the start of the games and we still don't know how many spectators are allowed or how they will be chosen. all we know from organizers is that anyone allowed to attend is going to have to follow strict covid rules before, during and after the games. international spectators have already been banned and with the latest change reflects just the growing restrictions and what is already an extraordinarily restricted olympic games, even for than what i experienced in the tokyo summer games. i'll be traveling in a couple of weeks but already i've had to download an olympic health tracking app, put in my daily temperature and once i enter beijing i'll be part of this massive bubble which is being called a closed loop system. there's even special transportation to go between the venues and even who arrives at these games unvaccinated will have to quarantine for 21 days, and get this, ana. for the local staff and volunteers in china at the games, they have to quarantine for 21 days before exiting this closed loop system and going back to their families. beijing right now is on extremely high alert after detecting its first omicron case. this is a country where even one covid case is one too many, and we're seeing several cities across china using the snap lockdown, mass testing and extensive queen teens. this zero covid approach had works thus far but the concern is will it hole up? >> christine, the everyone -- there's still time to get vaccinated. what do you think he ought to do? >> he better get packsated or he's --. his legacy and a part of history and making a lot of money at the history and sponsors, lacoste is not happy or at least wants to take to him because theyify have him play. it's extraordinary that djokovic, we presume he's vaccinated for polio and other things like this. all these athletes, as you well know, take shots for painkillers, whatever, if they are hurt and that he would make this decision and take this stand, wow, what a thing it is. never thought i'd see it in in my career but if he wants to play in the french open and wimbledon and the u.s. open he needs to get vaccinated, and let's see where he decides to draw the line on his career, his livelihood, everything about his image. >> you talk about his leg circumstances his brand. he does have his supporters in all of this. the serbian government calling the deforgotation a scandalous decision and that's. >> he's going to -- the tennis community is on, does djokovic emerge from this a martyr or a villain? >> i'm going to pick villain. we know that australia has gone through so much so just the fact that he wanted to saunt err in and do melbourne, i mean, just the arrogance and the cluelessness of this athlete to think that he could just get his way. most of the top tennis players in the world are vaccinated and we know where they stand on the vaccine and everybody was sick and tired of the restrictions. but at end of the day if you're vaccinated you look at someone not vaccinated and it's creating chaos in the ethat you love and the serbians, of course, they are on his receipt and that's what pays the $30 million in endorsements last year and that's what he has to care about, more the rest of the world and not serbia. >> as you see djokovic being held out as maybe a litmus test or an example of sorts in terms of the bigger battle at our hands right now. what kind of pivotal moment is this in this time of sports and covid? >> reporter: well, ana, the fbi is coordinating with its counterparts in the uk and israel, and as part of that investigation two teenagers were detained in the uk on sunday night for questioning. authorities have said there isn't any wider threat right now, but as you mentioned the fbi did call this a terrorism-related incident where the jewish community was specifically targeted. unfortunately that threat is something that this seen that was possible but it has gone through cries training before. the rabbi says that was crucial before he was ultimately able to throw a chair towards the suspect to hem him essame. the fbi has identified 44-year-old british national malik faisal akram as the man who held four people hostage at a texas synagogue on saturday. the nearly 11--hour standoff ended after an fbi hostage team shot and killed him. all four hostages are safe and unarmed. the situation began during the synagogue's livestream of its saturday morning sabbath. you can here him speaking but he can be heard saying he plans to die. . in a statement to cnn, one of the hostages, rabbi charlie cytron-walker said in the last hour the gunman became increasingly belligerent and threatening and he credited the security training his congregation had taken part in in getting them through the traumatic event. >> this is the first time that anyone at abl can recall a hostage-taking it at a synagogue, but, unfortunately, jewish sites have been targeted again and again and again by extremists over the years. >> according to officials akram entered the united states legally in december. he was vetted and cleared prior to his arrival at jfk airport in new york five weeks ago. federal authorities do not believe that this was part of a greater plot, but they are questioning how akram was able to travel to texas. according to law enforcement british intelligence officials tell their u.s. counterparts a preliminary search showed no derogatory information on akram. once he arrived in dallas he spent several nights at a homeless shelter. cnn was told in a phone call that we were a weigh station for hip. he had a plan. he was very quiet and was in and out. now the fbi is conducting a global investigation to try to determine a motive. >> i'm not ready to add any more. they were specifically focused on one issue. >> according to two law enforcement sources, one boss motive was a desire to free aafia siddiqui who is serving an 86-year sentence in federal prison in texas for the attempted murder and armed attack of u.s. service members. you can see right behind us now that police investigators are still actively working the zone. they have been since this happened saturday. there's somebody on the roof right now. yesterday all day our colleagues saw them lowing items into a truck, likely processing evidence, so it is still quite the activity behind us as theywork on this case and figure out how and why this happened. in the meantime, the congregation tonight is holding a special service at 8:00 p.m. eastern time at a different location. it will also be livestreamed. the post about that special service said we are strong. we are resilient. the time to heal our community has begun. it's an attempt they say to put this terrible event behind them and be thankful for the safe resolution. ana. >> sure sounds like a strong community. thank goodness the hostages made it out. natasha chen in colleyville, texas. north korea launches more ballistic missiles today, their fourth test just this month. what does the reclusive kim jong-un want? >> well, first of all, north korean technicians always need a a test to value tate their designs, but as for why now, we never know for sure, but there are two things that come to mind. first of all, the biden administration's week sanctions imposed on wednesday continue lax enforcement from the trump administration so i think kim jong-un thought he wouldn't get pun pushed for this and march 9th is the south korean election for the president and one of those candidates is very pro-north korea so i'm sure that kim jong-un thought that these tests would actually help that candidate. >> the there was the suspected hypersonic missile test that week and faa issued a ground stop of planes and very briefly paused depart use at some west coast airports out of an abundance of caution. it didn't last long, but it makes mow wonder does north korea has increased capability to make it a bigger threat to the u.s. right now? >> well, they are testing hypersonic live vehicles and those pose a great threat to the united states because they can drop out of orbit and incinerate an american city with very little warning. the question that the biden administration needs to ask, which the trump administration should have asked, which is where did north korea get this technology, because the least likely explanation is they developed it on their own. the most likely explanation is they got it from china or russia, maybe both. >> you talk about how the biden administration and the trump administration have both failed in some ways in your eyes, but i wonder as well does north korea show that it -- that it sees the biden administration in a different light than the trump administration? is it treating this administration differently? >> probably not in terms of broad general policy. kim jong-un wants sanctions released, and so he's testing the biden administration, just like he tested the trump administration and other administrations before that in their early phases to see how far the north koreans could push the u.s., so in general i think that kim jong-un's policies and the policies of his father are relatively consistent. >> in terms of taking actions to deter north korea, the u.s. did just announce new sanctions against eight north korean and russian individuals last week. you say sanctions rnts enough, so what more can the u.s. do about this? >> well, those sanctions aren't enough because those six korean individuals, they were actually proliferating chinese technology and materials, so china runs a near total surveillance state. bay swing knows what's going on because north korea is a very sensitive issue sass nuclear proliferation. so what we need to do is to show china we're not afraid of it by actually starting to impose sanctions on china and since may 2018, trump did not impose sanctions on china, and, of course, the biden straight hasn't evening. i think beijing says to north korea, look, can you do what you want and by the way, ana, today is the first day that we see a train leaving china for north korea with all sorts of stuff on board. we don't know exactly what. >> so you think the u.s. has to do something to basically send a message to china in order to change the behavior of north korea? >> given how fast omicron has surged and how fast it seems to be abating in the places that it is affected, i don't know that either of these is going to be timely in order to able to take on omicron. think about it. when it comes to third doses, only 38% of americans have availed themselves of that and then you think about how fast it might -- how quickly we might be able to get an omicron-specific booster and march seems to have missed the boat entirely and when it comes to a fourth vaccine, perhaps for folks who rimno compromised or are seniors, that may be a helpful approach here, but even if it's not going to protect against breakthrough cases, we know that a third dose in and of itself protects against serious illness so given what we know about omicron i don't know that this data suggests we should be doing anything more than what we're doing right now and for fwhoeks have not gotten the third dose please do so. that's probably the most important intervention you can do to protect yourself and your loved ones. >> the third dose is what they are folk yutsd on but the former fda chief dr. scott gotlieb says he thinks we may need annual covid boosters for a while. do you agree with that, and a lot of people are thinking when does this end? when does it stop? >> on the up hand dr. scott gotlieb is the former fda chief and also a board member of pfizer. the other point i'll make about this we don't know what endemic covid looks like. we are still in pandemic covid, so i think there's a lot to learn about the evolution of this virus under endemic conditions, when of course, we're not gripped by this but it's something more akin to the flu and cold, something seasonal that some group of people will get but it's not taking over the society the way that omicron is right now and we have a lot more to learn about where research on vaccines goes from here. i would really like to see a pan covid vaccine and that's a potential scientific possibility. we're not there yes. i do think it's a little early to talk about annual shots. that might be the case, of course, but we just don't note conditions endemic covid right now. we don't know where vaccine research is going so i think we get through this moment and then look up and say, okay, what is and how is this virus behaving? how is it shaping our illness patterns in this country and beyond and twhaen do we do to protect ourselves from there? >> new york has been hit very hard. it seems like everybody knows someone at least who has been infect federal their family wasn't impacted but the good news the positivity rate for this state just dropped by a full 10% from 23% a couple weeks ago to around 13% positivity now. that brings a lot of silver lining and glimmer of hope for people here, but surgeon general is still warning that we shouldn't expect to see similar trends in other places just yet. let's watch. >> there are parts of the country, new york in particular and other parts of the northeast where were starting to see a plateau and in some cases and early declining cases. the challenge is that the entire country is not moving at the same pace. the omicron wave started later in other part of the country, so we shouldn't expect a national peak in the next coming days. the next few weeks will be tough. >> given that, where do you expect the country will be past this omicron surge? >> well, you know, in the part of the country where i am here in michigan, we've always been hit by surges about a month after the largest cities in the country, places like new york or seattle or boston, and so what you're seeing here is that though larger cities who are hit first may be beyond their omicron peak, a lot of rest of the parts of the cup truckers where, by the way, fewer people tend to be vaccinated or boost federal they are vaccinated in those parts of the country, the worst may be yet to come, and so there is a prolonged american experience even if the larger cities may be over it, so, you know, when are we going to be over with? when are we going to start seeing that end of the upside down "v" in communities like michigan or in the south? probably in the next throw to four weeks, and until then it really is critical for folks who haven't gotten their booster, i'm going say this again. please do. it's the best thing can you do to protect yourself. we've got to batten down the hatches and do what we can to protect ourselves and our loved ones and hope that we can get through this together. >> yeah. full disclosure. i got covid a couple weeks ago, and i was boosted. i didn't even realize i had it until i got tested because i had a slight runny nose and was with somebody -- in close contact with somebody who had already tested positive. i credit that to the booster and, you know, hardly feel the impack. i was so grateful. feel very fortunate that that was my covid experience. dr. abdul el-sayed, thanks for your expertise. i wouldn't wish this on my worst enemy, a mom speaking about her 1-year-old's son experience with covid. baby crew who has down's syndrome was diagnosed back in december and spent a month on a ventilator back in december leaving his parents with heatache. his mom amy crosby is joining us now. amy, wow, you've gone through so much. i can only imagine how is you and how is your son? how is crew doing? >> him. thanks for asking. we're actually doing very well. our son was able to be extuba t'd and is continuing to progress and he's on a single canula and he's really made a lot of stride the last couple of days. >> such a cutie as we look at pictures of him here in the hospital. it's heartbreaking and heartwarming at the same time because he's just so precious. i know you've been through a lot. he's been through so much. you're still at the hospital. you've said you wouldn't wish this on your worst enemy. walk us through what his covid battle has been like. >> so at the beginning it was pretty basic, a little cough, a little mild, you know, not even a fever. he did really well and then all of a sudden just one night he all of a sudden was having more issues to breathe and i knew it was time to talk to our pediatrician and then things just very vastly progressed until we ended up having to be transferred down for a possible backup of something where he might need be to be on a heart and lung machine so omaha had a bed for us. we were able to come down here and they truly have saved our son's life. >> i can't believe he was on a ventilator, somebody so tiny having to experience that for nearly a money is my understanding, and i know you were taking so many precautions given he couldn't be vaccinated and because of his pre-existing conditions. do you know where he may have gotten covid? >> yeah. we do have a decent idea where he maybe had and it was honestly kind of a fluke thing. possibly a family member who wasn't sure they were symptomatic, previously vaccinated. we mask with everybody, but we had maybe lesbian that had for a little bit period of time. we really aren't sure. we can't say that that who that was. that's the only thing we can think of. but i just think it's so hard to know sometimes, too, but he hasn't been out in public or anything like that. >> so for parents towns, you know what, to look for, i ask this question. how did you, you know, figure out that he wasn't just, you know, battling a cold or something but it was -- he was sick enough and needed to go to the hospital? >> so for crue and with just some knowledge that i have, too, just his breathing was more work. i can tell he was more fussy, not able to be held and normally for him i can get him calmed down and you can just see in his respirations it was becoming harder to breathe and we do have the means of oxygen and monitoring at home and we saw his needs going up, his oxygen needs going up as we were turning it up higher and higher in order no have him oxygenated at an adequate level. >> and you saw firsthand how hospitals are really struggling with staffing, right?
