Refinery fire three years ago. The United States chemical safety board released the third and final report on the 1212 fire that endangered 19 workers and sent throughs of people to the hospital for medical treatment. The report also identified shortcomings in the chevron safety problem and emergency response. Chevron has been fined more than 2 million for the fire.
