For months and months and months. Were going to open our country again. And the cure cant be worse than the problem itself, right . I started by saying that, and i continue to say it. The cure cannot be worse than the problem itself. We got to get our country open. This country was not designed to be closed. So weve had the greatest weve ever had, and then were paying people to stay home . Think of it. Were paying people not to go to work. How about that . How does that play . And they want to go to work by the way. They dont even want they dont want money. This country is great. But were paying people. We have to get back to work. Thats what im saying. Joining me now, dr. Lipi roy. Shes an msnbc medical contributor and an internal medicine physician. And dr. Schaffner, a professor of infectious diseases. I see lipi. I dont see dr. Schaffner up yet. Lipi, you and i talked this morning and you said maybe donald trump needs to come to new york to see how things are. He vacillates between saying he said probably two hours ago there would be a lot of death this week and next week. Its going to be terrible. Then he skips right back into we got to get back to work. He seems to have a fundamental misunderstanding of the seriousness of this infectious disease. Hi, ali. Its good to see you again. Total disconnect, and its completely conflicting topics, right, and points. And its confusing people that are really strong supporters of him who are just really listening to him. Look, ive said this to you before. Ill say it again. As Health Care Professionals, we have one sole agenda, and that is the health and safety of the public, of our patients. We do not care what political affiliation, religious affiliation. It just doesnt matter. We care about your health. So, yeah, i get that urgency that he has actually of going back too work and getting the economy back. I get that. But, look, let me share a story. Many years ago my father had quintuple bypass. Thats open heart surgery, which is major surgery. Imagine if after the very next day or the next hour after the surgery, he said, oh, i want to go back to work and go running. Im like, yeah, you cant do that. Youre not ready. Right now our society is not ready to be out roaming around in public because we have a pandemic. And as weve said before, the data shows that the only way we know, the most effective way of reducing transmission of this disease for which we have no treatment, cure, or vaccine, is physical distancing. Thats why the Health Care Professionals are strongly making this point of physical distancing and staying home. I saw your tweet the other day. You gave blood. There are blood shortages around the country because the number of blood drives have been canceled. However, how safe is it to give blood, and whats the blood situation right now . Yeah. I am so glad you brought that up. Almost every day im talking to people and reminding them of a Blood Donation. So ever since we started this stay at home recommendation, strong recommendation, blood drives i think almost 3,000 blood drives were canceled. So its important for the public to remember that one in seven people who enter a hospital need blood, right . People with sickle cell disease, trauma victims, g. I. Bleeders. Many, many people need blood. So, yeah, so when i donated blood, i went to a Donation Center right here in new york city. There were hardly anyone there other than staff. They were checking my temperature. They kept us distance, wearing masks. It was very safe from a public standpoint. So i really commend that facility, and i think most Blood Donation facilities, i mean they practice good hygiene at baseline, so theyre obviously ramping up at a time like this. Dr. Schaffner, i want to ask you about getting back to work and workplace safety. Half this country is not taking seriously the idea that they need to employ that sort of thing. But the bottom line is as much as the president doesnt send clear messages, i think there is an understanding from all of us that at some juncture, many of us will have had coronavirus and survived and perhaps there will be antibody tests. I believe there are some out now already to determine that weve got antibodies and perhaps some resistance, and we will have adequate Protection Equipment for those of us who have to work. How do we start thinking about how that return to work looks . Ali, are you talking to me . This is bill schaffner. Yes. Yeah. I think the way that looks the way that looks, ali, is in a gradual fashion. We will first have to look at the data, just as dr. Roy says. Its very important that we not send people back to work and then have immediately another wave of infection. We must determine what we do by what the virus does, and we can determine what the virus does if we all engage in social distancing. We dont want the virus to get from one person to another, so we must keep ourselves separate. And yesterday the cdc gave us Additional Information in addition to staying home, going out only occasionally, washing our hands, we all ought to be using face protection at the same time. I actually went to the supermarket this morning. I had my own face protection on, and i was a little disappointed there wasnt a big sign outside the supermarket that said customers, we would appreciate you all wearing face protection in order to protect not only yourselves but the good people who serve you in this store. I think we all need to do that. As a doctor, you understand that the confusion here is that the cdc says we should. There are still some governors who arent sure. The president himself says hes not going to wear one. Part of this is the inconsistency in the messaging about wearing face protection. Ah, youve just broken up. Im afraid i cant hear you. No problem. Let me ask that question to dr. Roy. Same issue, dr. Roy. Yeah. The president said himself, right next to the cdc saying that you should wear face protection, he said, well, i dont want to be meeting dictators he actually used that term in the oval office at the resolute desk with a face mask on. I dont know why dictators came to his mind first, but the fact is there is an inconsistency around the messaging about that. I would imagine that if people like you and Public Health professionals and dr. Schaffner all said, hey, protect other people. Wear the face masks, wed all get face masks. Yeah. So let me drive home a key point that dr. Schaffner just made. He used the word data. Look, again, i get it. I get the confusion from the public. Theyre getting what seems like mixed messaging. But, look, from the Health Care Professionals, the Public Health professionals, we are truly looking at the data. And just as a reminder, were dealing with a novel virus. Were truly getting realtime data, information, what the science is showing, and we are then sharing that back. So the information i wouldnt say that its conflicting. Its evolving, right . So, remember, there was a time when we said, its okay, you can stay home. Oh, you can still go out. Now were saying, no, do try to stay home. Regarding the masks or the face coverings see, were not even saying masks. Were saying just cover your face because new data is now showing that maybe even talking can transmit the virus whereas before data showed that it was only sneezing or coughing which transmitted the virus. So, again, its about applying, learning the data as were going along and then disseminating that to the public to keep everyone as safe as possible. Bill schaffner, can you hear me now . I can hear you now, ali, yes. Bill, how safe are hospitals if people are thinking that theyre sick . How safe is it to go to a hospital . There are normal procedures that have to occur at hospitals too, not just coronavirusrelated stuff. What should people be thinking about . Well, i think hospitals are really being very, very meticulous. You cant get into our Medical Center without having your temperature taken. We are limiting visitors. Were being very, very rigorous about that in order to protect our patients and to protect our Health Care Workers. And of course we have at the moment in nashville enough, but just enough personal protective equipment. Were already concerned about what may happen next week and the week after when we expect to get many, many more patients. Dr. Roy, phil murphy, the governor of new jersey, has decided that he is going to lower the states flags to halfstaff indefinitely to honor the coronavirus deaths. And part of the issue here is that there are people who are losing their loved ones either to coronavirus or because they cant get to them in a hospital without being next to them. There are funerals that are not able to go forward in the normal fashion. People are being buried on their own. This is a whole different thing that were not dealing with. As a society that has not culturally entirely come to terms with death, this is now another element of this that people have to deal with. I mean i cant believe were having these conversations in the United States of america in 2020. This isnt the 1800s. Were not reading a novel from dostoyevsky from the past. Its really unfortunate. Yeah, end of life care, these are conversations remember earlier this morning, you and i were talking about thinking two, three, four steps ahead. Well, we really need to start thinking about how were going to have conversations with our loved ones about end of life care, about just advanced planning, writing wills. I think i remember hearing a young 28yearold doctor, a resident, i believe, resident physician, saying that she just wrote up her will. You know, we need to start having these conversations. Here in the west, were not as accustomed to talking about death even though its a natural part of life although the fact that a pandemic is causing these discussions now, its not ideal. But at the same time, this is something that we really all need to be talking about. And, you know, the Palliative Care community, which is involved with the relief of suffering, Palliative Care is a subspecialty of medicine, you know, theyre really trying to focus on not just withdrawing care. They dont withdraw care. Its maybe treatment but never care. And care is offered by many different ways, managing pain, agitation, cough, and all these other symptoms. So, yeah, we need to start having these conversations because people are dying in droves, and its unfortunate, but we got to act. Dr. Lipi roy, dr. He wrote a letter. The letter was a fivepage letter from a captain, and the letter was all over the place. Thats not appropriate. I dont think thats appropriate. Its not appropriate. Earlier today the president weighed in on the navys decision to relieve Navy Captain Brett Crozier of his command after he raised alarm about the spread of covid19 on his ship, the uss Theodore Roosevelt. We also got more information today about the circumstances on that Aircraft Carrier with a crew of nearly 5,000 United States sailors. 44 of the crew has been tested for covid19. So far, there are 155 positive cases. More than 1,500 sailors have been moved ashore. Joining me now, dr. David shulkin, former secretary of Veterans Affairs under the Trump Administration and former president and ceo of beth israel Medical Center. Hes a doctor, but as the former head of the v. A. , david, you were familiar obviously with military people, with veterans. This captain crozier was looking after the interests of his seamen. He sent a letter, and he was fired for that. And the president has made it very clear that him raising that alarm was a problem. Theyve given some sort of false excuse about how it wasnt chain of command stuff and he sent the letter to the wrong person. But he raised the alarm for his sailors and got removed from his command for that. Yeah, well, ali good to be with you. These are clearly extraordinary times and were seeing historic acts everywhere with Health Care Workers just like in the military. And i believe leaders deserve and have a responsibility to protect those that serve. And captain crozier had at the time 137 sailors that were infected, many others at risk, and they needed to be protected. Doing that, i think the captain im not exactly sure what steps he took before he went outside the chain of command. But what i do know is that he stood up for what he believed in. He stood up for his principles, and thats the type of person i want in command of a ship like this, a person who is willing to put their job on the line to stand up for what they believe is right. And frankly, that person deserves our gratitude and our respect. And frankly just look at the reaction that his crew gave him on his sendoff. In my mind lets actually just play that because in case our viewers havent seen it, it changes your view for a moment about the world being a good place. Lets just play what the sailors on that ship did as captain crozier exited the ship. [ cheers and applause ] [ chanting captain crozier ] yep. Thats pretty incredible. Yeah. Ali, in my mind, that speaks very loudly. There is no greater affirmation of a good leader than having the gratitude and respect of the people that you serve. And you can see the way that they felt about him, and thats the type of captain that we want leading our troops. People that the troops believe in, the sailors believe in and, you know, this is a person who really has served their country and was doing what they believed was the best thing even if they knew it would cost him his job. Lets talk about another thing that happened late last night, david. The Inspector General for the Intelligence Community was relieved of his job by the president of the United States. The president said he has lost faith in him. There was no allegation of any wrongdoing. The assumption here is that this is the person who brought to congress attention, to adam schiffs attention as the head of the Intelligence Committee at congress, the fact that there was a complaint about this whistleblower, the ukraine situation, and that the white house wasnt handling it properly. For that, for doing what many people believe was his job, hes now been relieved of his job. Well, it certainly isnt easy to be in washington these days, to stand up for your beliefs, you have to be willing to put your job on the line. And thats part of what goes with public service. But we are seeing a clear pattern thats repeated time and time again of people who are brave and stand up and do what they think is right dont necessarily last very long in washington these days. And it is increasingly hard to be in public service. And at times of crisis like this, we see how important it is that we have people who are experienced and competent leading at the top levels of government because frankly we as citizens rely on these people. And its a really tough job right now. Tell us what youre hearing, sir, from the medical community about the state of the pandemic. You were in charge of two hospital systems, one in new york city, one outside of new york city in new jersey. What are you hearing about the state of testing, the state of staffing, the state of supplies . Well, it still is difficult to get tests. While there are increasingly more tests available, many people still arent able to get tests when they should be getting them. But the critical issue remains shortages of staff, and increasingly we see more and more Health Care Workers getting sick and frankly not having the type of protective equipment that they need. And, you know, we just need to do more. You know, if we talk about this being a war and we have a wartime president , we need a surge. We need to see people around the country who arent using ventilators and protective equipment to send them to the places that need them like new york, new jersey, new orleans. The fact that were getting ventilators from china rather than from different parts of the United States is really almost unbelievable to me. This is the time that we need to show those parts of the country that we are a single country, that we can help and protect our fellow citizens. And im hearing desperation out of my colleagues in new york, and we really need to see a bigger, broader reaction than what weve been seeing. That was President Trump earlier talking about his decision to remove the Intelligence Communitys chief watchdog Michael Atkinson. Atkinson was responsible for flagging the ukraine whistleblower complaint which eventually led to the president s impeachment. Joining me now, Yamiche Alcindor and frank figliuzzi. Yamiche, let me just start with you. He said it was a fake report. He said he was not a big trump fan. But, in fact, there was no allegation of wrongdoing against this Inspector General for the Intelligence Community. Thats right. What we saw was the president lash out at Michael Atkinson for effectively doing his job. He said he was a terrible person and he should not have elevated this whistleblower complaint. But the president did say in particular what the i. G. Did wrong in terms of his actual duties. What the president did say is that he didnt agree with the fact that he should have been impeached, and he again repeated the claim that he thought it was a perfect call between ukraine when of course we all know that the president was at least attempting to get ukraine to look into joe biden, a political opponent, and look into whether or not there were any allegations of wrongdoing there. Frank, whats your take on this . Yeah. So first lets call this what it is. During a press conference today, during a national crisis, the president of the United States just lied to us. This whistleblower complaint was not a fake whistleblower complaint. The facts were borne out in the impeachment hearings, and the president was impeached in part as a result of this very valid whistleblower complaint. But im going to go a step further here, ali. Based on what ive talked about even on your shows on this network and written about on nbc think, from my experience in the Intelligence Community, we know that Intelligence Community reporting was pressing the issue of a Global Pandemic even as corona broke out in china, intelligents agencies were warning the white house its coming. We need to prepare. My great concern is that this firing is beyond just revenge, which would be bad enough, but also anticipating that there are whistleblower complaints likely in the pipeline saying, hey, we briefed the white house on this date and time, this person on this date and time about pandemic preparedness, about corona, and that were probably looking at a situation where the president is trying to suppress whats coming forward in the whistleblower pipeline. Yamiche, what do you make of the timing . I happened to be on air last night when this happened. It was in the 10 00 p. M. Eastern hour. We had those remarkable unemployment numbers out yesterday. We had massive problems with the government getting its Small Business loans out. It was a busy, busy day in which everybodys minds were elsewhere, and the president did this at 10 00 at night. Thats right, and the president also is doing this at the beginning of what the president says is going to be possibly the worst two weeks of the coronavirus outbreak in the United States. So the president is also taking his action at a moment where americans are not only just distracted at times by the other things that are going on in their lives, people losing their jobs, people dying, but also are bracing for the worst. So the president did this very late on a friday night. Hes someone who understands tv, understands what prime time is and what it isnt. The timing here i think was intentional. That doesnt mean he was trying to hide it in some ways because obviously this is going to be a highprofile that everyone is talking about, and the president didnt shy away from the fact that he did this and defended it. So in some ways, even though the timing is questionable, the president was happy to talk about it today. Right. Frank, i spoke with adam schiff moments after we got the news about this last night. Hes the chair of the house intel committee, and he speaks about accountability. But the bottom line is while the president didnt outline anything that Michael Atkinson did wrong, it is entirely within the president s purview to remove him from his job. There is some sense that these Inspector Generals that work across government are meant to be a little bit independent, and theyre meant to be able to honestly say whats happening in those departments to ensure our safety. So on one hand, theres really nothing to hold the president to account for doing this. However, what does it do to the Intelligence Community that the person who sees that theyre doing their job properly has been removed . Theres a terrible message being sent here. If youre an employee of the Intelligence Community and you want to bring forward a valid complaint thats not being addressed by your management, you now have to think twice about whether its ever going to get any time and attention from the i. G. Or from the white house. And you saw an extraordinary statement come out from the doj i. G. , horowitz, about the firing of the intelligence i. G. And hes coming to bat for atkinson, saying, look, this guy was straight up and did his job. So youre going to see an outcry from those who are simply trying to do the right thing and protect the American People. Yamiche, i just want to ask you about something because there was this press conference you were at a little earlier. The president continues to talk about hydroxychloroquine as a potential remedy for coronavirus, and fauci continues to sort of downplay this, to say, i dont know whether fauci wants him to not talk about it or wants to say that this is incorrect information. But he keeps sort of saying, this is not what the president seems to be implying it is. We have to be very clear. There are no fdaapproved treatments for coronavirus, and there are trials going on for this antimalaria drug to see if it has an impact on the coronavirus. But what the president did today was essentially urge americans all over the country to take a non a drug that has not been tested, and the president went further to say that i might take it myself. The president has said he was tested twice for the coronavirus and has tested negative both times. But for some reason, the president was saying that maybe he will take this drug as a way of possibly preventing it. This is all very in some ways dangerous information. Health Officials Say that you shouldnt be taking this if its not something that is approved by your doctor. So the president is out there putting out this information that could be misleading and possibly even dangerous. Thank you to the both of you for your work. The fears of the shortages have led to inflated requests. We have some states and areas where theyre just asking for far more look, we had one state asking for 40,000 ventilators. 40,000, think of it. 40,000. Its not possible. Hes talking about new york state by the way. President trump speaking today on what he calls inflated requests for supplies amid the Global Pandemic. Joining me now is Lieutenant GeneralRussel Honore, commander of joint task force katrina. General, you and i have it must have been a month ago you and i spoke, and the president had just finished saying and by the way, general honore, as you can see, has got his mask there. The president had just said the federal government is not a shipping organization. Thats not our job. And one of the points you made and one of the points every american saw in 2005 when they saw you in louisiana is that the one thing that the federal government and the military in this country can do is logistics, and this is becoming a massive logistics nightmare. Yeah, and you know with all the electronics we got and tracking of the supply chain, you would think if we really had stuff on the way, we would see the numbers. We go through an hours brief every day with very few facts other than maybe one or two numbers coming out. But it would be very easy to give the doctors and those in the hospital some kind of confidence to show whats in the supply chain or either put it on an online streaming where you can go and check it at the fema website. None of thats being done. Its all being done on faith. The bottom line, ali, if it was working, it would be okay to do something different. But the problem is its not working. Daily we see doctors and nurses coming off shift and protesting because they dont have the stuff they need to adequately protect themselves and their patients. Its not working. So we dont have much time. If this is the big week, then i would hope that tomorrow they will start off with a fema update to the American People to really see because we should be knowing by tail number now what aircraft are involved from china or turkey or wherever theyre buying this stuff and where is it going. I think people need confidence on the ground so they can continue to do their work. But, you know, theres two problems, general. Theres that problem about being able to track it and trace it and something we should be able to do in 2020. But theres also this problem that in these briefings, we get contradictory information. The president says, its not possible that a state could want 40 ventilators. Well, i would imagine that that is something that can be established, right . If new york wanted the 40,000, someone can say its valid or not valid but they should have some data on which to think it. Youve got Jared Kushner saying they just dont need these things. And, again, on what basis . You are a guy who deployed your troops on the basis of data and a needs analysis. That doesnt seem to be happening. There seems to be a lot of opinion about what people need and dont need. Yeah. I mean when we deployed over 220 helicopters in new orleans, the army, the department of defense, or the white house never told me we didnt need that many helicopters or we didnt need 20 ships. Nobody said, you dont need this. They sent it because they saw the need. One of the principles of logistics, ali, is anticipation. And we failed to anticipate. Thats why our stocks were low. Now weve got the problem with how do we get the critical supplies to where they need them. And this is right now, my hats off to all the doctors and the people that are trying to fix this. But were not getting the results on the ground, and this is the critical week. Its time to straighten up with the American People and let them know whats coming, and we got to start fighting Solutions LikeGovernor Cuomo and the governor in oregon and start crossleveling supplies because i dont think theyre coming in. Which, again, is something that the military can do. Andrew cuomo, the governor of new york, said that. He said, look, we shouldnt every state shouldnt have the maximum number of ventilators it needs at all times. We should be able to move them out because new york is going to hit its peak before iowa does and before utah does, and it is actually the military that we can depend upon, the National Guard and the military, to say, hey, lets mobilize. Lets get these ones out of new york. New yorks off its peak now. We have extra respirators. Weve got a map we can show you where these hot spots are around the country. Lets get them to where we need them. Absolutely, ali. We talked about this a couple weeks ago. Its called days of supply. Weve never gotten that count and seen anything that is accurate. A solution would be to put two National Guard soldiers in every hospital and go in with the quartermaster of each hospital and count whats there and figure out the days of supply so you can crosslevel. Theyre going to have to make some decisions and going to have to do some crossleveling, more than just volunteering like oregon did. Were going to have to get some crossleveling between hospitals to be able to take care of new york, then deal with new orleans because the demand based on the governor of both states is going to be higher than what the federal government yep. Looking at that hot spot right now. You see the northeast. You see new orleans. You see chicago. You see detroit. You see hot spots in seattle. You see southern california, and you start to see them in other parts of the country. General, there are days i go to sleep thinking that i had a feeling when i first saw you in 2005 taking over joint task force katrina, and i felt like someones on the case and its getting done, and there are a lot of americans missing that feeling right now. There was a cant relieved of his command on the uss theodore roa roosevelt because he drew to the attention of his superiors that theres a coronavirus outbreak endangering his sailors. While youre here, i want to play for my viewers the response that his sailors gave him as he exited that ship, the Theodore Roosevelt for the last time. [ cheers and applause ] [ chanting captain crozier ] general, that gave me a moment of hope. It made me realize the people who are serving our country around the world and keeping us safe still know that the world is the right side up. Absolutely. Our great navy, if youve ever been on a carrier and seen what those sailors do to keep us safe around the world, that is a supreme sacrifice for the short quarters they have to live in. And the sendoff they gave to the captain is ultimate respect. And that captain did something that many people in washington never had to make a decision between the mission or the men. And as opposed to violating his mission, he looked out for his men because his Mission Still could be accomplished. And i hope that in the days to come, that this captain will be treated well by this nation because he made a decision many in uniform and in washington never had to do, and he put the sacrifice of his men first, and i hope the United States of america recognize that. The good news is the next captain of that ship is on that ship. The navy got a great process. The next person thats going to command it is on. That crew, theyre going to get this one. Go, roosevelt. Maybe in the midst of this terrible crisis while we go forward in anyway to prevent people from dieing, from getting sick, from losing their jobs, we take a deep breath and rethink the basic structures of American Society and how we can create a nation and government that works for all not just the few. Derek, a lot of people have no cause to think about the tb epidemic around the world in the United States, but like tv tb this infection targets more people in the general population, those dont have health care, those who cannot choose not to work and those who are sort of not in the formal infrastructure, and that is happening again with respect to coronavirus. Its getting its targeting the least among us in a way that none of us can handle it. Thats exactly right, and we can call it what it is. It will affect people based on their race as well as class. And the reality is its not just circumstance but policies and political decisions weve made in the past. So theres certainly a need to deal with the immediate vulnerability with the coronavirus, but Going Forward we can make sure we have resilient communities that are economically and physically secure and a set of policies we can implement to ensure that. And i want to just quote from the new york times, where it shows location data of those staying at home during the coronavirus. It says although people in all income groups are moving less than they did before the crisis, wealthier people are staying home the most especially during the workweek. Not only that but in nearly ever state they meaning the wealthier people doing so giving them a head start on social distancing before the virus spread. There are people i look out my window in manhattan and there were people still on the street people delivering things, doormen, people staffing the shops. In many cases these are people who are not able to economically able to make the choice not to do so. 100 . This gets at the crux of the issue. As derek says over the past half century weve made a series of decisions that have made our economy uniquely susceptible to this kind of economic crisis thats based on a health crisis. We created this because of inequality. Folks with professional jobs who can telecommute were able to go home, get on zoom calls and keep doing their jobs and getting paid. Yet so many workers with face to face contact, child care workers, home health aids, so many workers who are lowest paid are also the ones whose jobs have been hit the hardest. We got new date data yesterday on the job situation. Eight out of ten jobs lost was in a lowwage industry. This is really playing out across our city society. We know those workers dont have access to sick pays, these fragilities have amplified this into an economic crisis that is effecting the country in ways were going to be feeling for decades to come. I want to under score one thing derek said which is we have in our power to create an economy thats more resilient but starts by addressing these fragilies created by economic inequality. This is the time to do it. The history of remarkable prodwrams have been born out of crisis. World war i, the depression, world war ii is note worthy, this is our moment to do that.
