You honestly dont know, you dont know how many people have been tested . Because a private vendor shipped most of those 1. 1 million shipped from a private vendor selling to their customers and those entities that use the tests do not have to report back to cdc. Were trying to set up a reporting system where they would have to do that. Here are the numbers as best we know them at this hour. The death toll in the u. S. Is 26. The number of cases has risen overnight to 732. Thats across 36 states and washington, d. C. Globally more than 4,000 people have died. And there are now more than 113,000 confirmed cases. This morning all italy is on lockdown. The countrys borders effectively closed, 60 Million People barred from traveling there for weeks. Here in the United States, local officials are moving to mitigate the impact of the pandemic. That means social distancing. Santa clara county, california, has banned gatherings of more than 1,000 people and the securities and exchange commission, the s. E. C. Told employees to work from home and this is going on in the middle of what seems to be a financial crisis. Ohio State University just became the latest college, look at this map, to cancel in person classes, despite as far as we can tell having no confirmed cases on campus. Schools from coast to coast are closing this morning to clean the classrooms. Boston just canceled its annual st. Patricks day parade, a big deal there. Pearl jam, madonna, miley cyrus canceling concerts and were keeping a close eye on the mark hes after the single worst day on wall street since the 2008 crisis. Things are looking up, the stock futures pointing to a big rebound. The markets appear to be reacting in part to President Trump floating the idea of a payroll tax cut and other Economic Relief to offset the impact of the coronavirus. Were going to begin, though, with the Health Concerns this morning. Joining us, cnn chief medical correspondent dr. Sanjay gupta and dr. Celine gounder. I want to start with where you think we are this morning. The most important developments over the last 12 hours. I dont know if it is santa clara declaring in gatherings, more schools closing, what is it . I think the social distancing measures that are going into effect sort of being determined more at the local level, not coming from some sort of national recommendation. I think thats something that is really alerting people to what is happening here in the country. Obviously a lot of people hearing about italy and being reminded that in italy, back on february 2nd, there were sorry, february 20th, there were just a few cases and no deaths and then a couple of weeks later you have a couple thousand people who are infected and around 50 deaths. So how quickly this can change in a place and how social distancing measures may impact that i think is something that a lot of people are paying attention to. In addition to your excellent interview, john, with secretary azar, about the lack of testing still. We have been talking about this for weeks and constantly comparing our lack of testing or decreased testing compared to places like south korea where they have been doing thousands of tests a day. We still, according to our calculations at cnn, total in the country have done fewer than 6,000 tests still. So i think those are the some of the headlines people are hearing about today. Secretary azar was just on, john just interviewed him as you heard. So if we dont know how many people are getting tested, if the hhs doesnt know, cdc doesnt know, then how can we know how many people have the coronavirus . Well, there are probably different reporting procedures for did you test positive versus were you tested by i have a hard time believing that lab core and quest which have completely automated reporting systems, i can log in as a provider online and pull up a patients results. Im sure they can provide a total number of this many tests run, this many tested positive, this many tested negative, thats not a complicated thing to do. Wouldnt that be helpful for the government to know, to know what the coronavirus looks like in this country . Absolutely. We would want to know where are you seeing cases, where is testing being done, where is it not being done. That would help us target resources to the right places, especially in terms of hospital preparedness. We want to know where the cases are and were now hearing of n95 respirator shortages which makes our jobs impossible to do safely in the hospital. Talk to me about schools, sanjay, ohio State University is just the most recent school, college, big university, to cancel classes for a period of time. But there are middle schools, junior highs, high schools across the country closing as well. Why, given that we know that younger people are not at high risk here. They are not the ones getting very sick. Yeah, no, it is a good question. First of all that is true that younger people, thankfully, you know, seem to be insulated to some extent, protected from getting particularly sick from this. We dont know entirely why. But as we also talked about, they can still carry the virus in their bodies, they can potentially still shed the virus and be a source of infection. So i think there is really two reasons, first of all, those are large gathering places, big schools and big universities. And, you know, if there is an infection in an area like that, it can spread throughout a large gathering of people like that. And then they can go home or they can go be with a Vulnerable Group of people, and possibly spread the virus along. So this is really about trying to break that cycle of transmission and schools, you know, at least closing schools or closing them for a period of time and disinfecting them well could be a strategy to help break that cycle of transmission. And again, it is being decided on an ad hoc case by case basis. There is no federal mandate around this, but i think a lot of schools are being proactive in this regard. There are also schools where if someone tests positive, the school is more reactive to that saying were going to close down for a period of time, make sure the school is disinfected before we bring people back. It is the ad hoc federal response that is confusing to people. I hear people all the time, are they supposed to be taking Public Transportation or not . Here in new york, there are now more cases in new york state than anywhere else. The head of Port Authority, the huge Mass Transit System of new york city has just tested positive. Are people still supposed to be taking Public Transportation . Where are the instructions for people . Well, we heard coronavirus dot coronavirus. Gov announced yesterday. They are i think compiling a lot of that guidance there. It is a lot of the things we have been talking about. The hand washing, the social distancing, that sort of thing, but i do think one question i have in my mind is taking the new york city subway, it is hard to maintain sort of that two meter distance for people, maybe not so hard now because a lot of people arent taking the subway, some of those things are difficult to do. There is also on surfaces, those arent being wiped down every time people get on and off the train. I too take the subway. I look at the surfaces in a different way now than i used to. Again, we dont know what were should we all know what were doing or is this just the fog of war as we heard from dr. Fauci say yesterday . I think thats part of why this is different from the flu, for example, because there is so much we still dont know. We really are learning and going on by the seat of our pants because this is something new. This is something different. And there is still a lot that we need to learn. These are the some of the things theyre let me read those. Things like wash your hands and avoid crowding, great advice, what people should be doing, but tom bossert, Homeland Security adviser in the white house before, sanjay, said to the extent, a lot of this is like a light from a star, were now seeing something that was actually emitted some time ago. Yeah, yeah. And, look, i dont want to belabor this point, the fact we havent tested might make that light from the star further away here. The star may have already died and were just now seeing the light from it because were that far behind in testing here. What he also wrote in this oped for the washington post, i think is really important point, with regard to what they call these npis, nonpharmaceutical interventions, social distancing mechanisms, they can be effective. We have seen evidence of that historically, going back even to the spanish flu of 1918, two cities, philadelphia and st. Louis, he writes about, st. Louis had much lower rates of the flu compared to philadelphia because they took social distancing measures early. If you get beyond 1 , so on santa clara, city of around 2 million, if you get beyond 20,000 people infected, the social distancing, you know, measures, dont have as much of an impact. I see here, it says disinfect surfaces like door knobs, tables and handrails regularly. Im going to take that on for the new york city subway. I spoke to a guy earlier this week, a big money manager, he said the only thing that will stop this volatility is seeing peak cases, a stabilization in the numbers here in the United States. As you guys and i know pretty well at this stage, we look a fair distance away from that at this stage. Were also waiting for what President Trump can say about possible measures to support individuals who have been hit, impacted by the coronavirus outbreak, payroll tax cuts, perhaps some shortterm loans as well for small businesses. Joining us now with updates is new yorks Governor Andrew Cuomo. Governor, great to see you this morning. Do you have updated numbers for us . What is going on in new york . Were updating them as we speak, alisyn. And well get them all together so there is no confusion. It is up about 25 since yesterday. Okay, thats troubling, obviously. Also troubling is that the head of Port Authority, which is new york citys obviously huge Mass Transit System has tested positive for coronavirus. And so what is the message there . Should we stop taking Public Transportation . If he contracted it, can the rest of us contract it . Well, first, the head of the Port Authority rick cotton, who is doing a phenomenal job, and hes a great friend, the Port Authority in new york supervises the airports. So he spent a lot of time at jfk airport, just at that time when all the flights were coming in, if you remember, and getting that initial situation under control, what flights were coming in, what people were being tested, et cetera. So he was really in the ground zero of the coronavirus outbreak. And hes remarkable guy, hes a hands on guy. We have a hands on government. And he was there himself. And he got exposed, our guess is, during that situation. But it is not emblematic of Public Transportation or anything else. Well, yes and no, governor. The reason i ask is because instruction seems so scatter shot for what the public is supposed to be doing. Im sure you know families, i know many, who dont know if theyre supposed to be flying right now and taking their family trips. Some are canceling them because, again, i understand what youre saying, that rick cotton is a hands on guy, but he was at the airport, a lot of us go through airports. Should we be going through airports right now . Yeah. Couple of points, alisyn, first, rick cotton was at the airport at the very beginning when people were coming back who were infected from countries all around the world. We didnt even know who was infected and who wasnt infected. In general, the lets remember the bottom line here, the vulnerable populations should be taking precaution. We have this mass fear, mass hysteria. It is the vulnerable population, senior citizens, yes. Think twice before getting on a plane. People with preexisting conditions, yes, think twice before going to a large gathering, a person with a compromised immune system, yes, think twice before getting on a flight, going to a gathering, Public Transportation, et cetera, those people, yes, general population, no. And also, alisyn, if we can, earlier interviews that i thought were very interesting. Remember what were doing as a society. There is only two ways to stop the spread of the virus, right . One is massive quarantine closedowns. Thats what china did italy now. Italy also. So people dont interact with each other. Thats strategy a. Strategy b is massive testing so you find people, you find who is positive, and then you isolate those people. Our country does not have the same capacity to do the massive testing that the other countries did. China, south korea, they were testing like 10,000 people per day, per day. Right. Were nowhere near that. What is our answer . So thats our challenge. Well, look, what im saying our answer is, we have to say to our federal government, you have to increase the Testing Capacity dramatically. And faster than we are now increasing the Testing Capacity. Lets look at china, lets look at south korea, their tests are not as good as ours. Okay. Even if thats true, they were doing 10,000 of them a day. And were doing a fraction of that. But, governor, i want to ask you and identifying the positive i want to ask you about Something Else we can do, one thing in our control and we see communities around do it, it is harder in new york, but thats the social distancing. Boston has just canceled its st. Patricks day parade. Should new york do that . Thats one of the things were looking at. And, look, in new york, we also have a cluster in Westchester County which is outside of new york city, and that is very intense cluster. So were taking more dramatic actions in that cluster, right. You calibrate your response to the time. And the facts and the circumstances in that place at that time. So parades, et cetera, we look at that on a daily basis. You might do that . Youre saying it is possible that we might see that happen here, the canceling of the st. Patricks day parade . Well, yeah, alisyn, what you will see is the numbers will continue to increase because they have increased. Our testing is lagging the reality. Were not testing enough people to know where it is. The more we test, the more positives youre going to find. People will think oh, my gosh, it is going up. No. It was going up anyway. You just have testing now that confirms what has been going on. That number will go straight up over time. Im worried that thats going to continue to fuel public anxiety which already is ahead of the virus, because, remember the bottom line, we talk about all this stuff, the testing, the quarantine, if you are not a member of the vulnerable population, alisyn, dont overstress. But senior citizens, elderly, the conversations i have with my mother, yes, that is a different conversation than the conversation i have with my daughters, or my colleagues. And we have to keep that info and then in the meantime, this country has to get better at testing and faster. Understood. That is good advice, governor cuomo, thank you very much. Nice to see you. And if he cant get the full support of working class white voters in the state of michigan, thats bad for him for tonight. These are the states hes been trying to make the case, these are the states in the rust belt i can win, not only in the primaries, but in the general election as well. But in addition to that, in addition to just being the bridge, i do believe if he does get it and if he wins the election, lets hope he does or any democrat does, he can also be transformational in that he empowers the next generation. He has to listen to the base and engage the base and i do believe it is super tuesday, i will be bold, learn nothing from last week and make bold predictions about that is going to happen. But it seems that joementum is on his side and if bernie didnt take michigan or washington, it is going to be tough, going to be tough for bernie. I think we have thus far painted a pretty disheartening picture for Bernie Sanders tonight and Bernie Sanders fans and supporters, but anything is possible. I mean, thats what we have learned. So there may have been people even voting before the latest revolutions in this cycle. Yes, but i think so i think vote by mail helps bernie in Washington State. This is the most important thing to politicians, the thing they do often least well, when trump was coming on, people like me, certainly republican establishments, any mock trump and they also mocked his voters, a critical error. If the democratic establishment or the people supporting joe biden are laughing at the young people particularly, and also the working class folks, but im really interested in listening to the young people, hes losing the young people. You just mentioned Harvard University, which we learned since last night, also the ohio state, San Francisco state, the university of california, san diego and amherst college. We learned are all now moving their classes online. Just adding to the list of schools making this move. Social distancing is the priority at schools across the country as they try to prevent the spread of coronavirus. In Washington State, the university of washington has suspended classes for the rest of the quarter asking instructors to conduct classes and exams remotely, with plans to resume normal class operations march 30th, pending Public Health guidance. Several universities are taking similar steps, canceling in person classes at least temporarily, including Stanford University and northeastern university, seattle and San Francisco bay area campuses. On the east coast, princeton, hofstra, fordham, barnard and columbia universities are temporarily suspending in person classes. It became more real. Reporter columbia universitys president , the quarantine person is not a confirmed case of covid19. It was a way in a different country or before it came to america, this is out of our this is beyond us. But now it is here. Reporter Elk Grove Unified School district in sacramento county, california, also canceled classes and activities for the week due to virus concerns. This is a time to listen to your body. Reporter bill de blasio saying there would be a high bar for closing the citys Public Schools, but there could be specific targeted closures. New york Governor Andrew Cuomo putting it this way. For all schools, were going to set a policy that if a student tests positive, in a school, that school is closed for an initial 24 hour period. Reporter Public Schools in scars dale and Westchester County will be closed until march 18th after a faculty member at the middle school tested positive for the virus. Now, when it comes to Public Schools here in new york that could see disruption or staffing issues due to coronavirus fears, the mayor says the city has resources for schools in case of additional needs. They have a reliable reserve of substitute teachers and other staff that can take on other roles as necessary and are looking at how they can ramp that up if they need to. You deal with this every time you walk around the office, people have questions, anytime you go to anything, people are asking you questions, because they want to know. It is affecting so many peoples lives. Sherry asks, how long does it take to develop symptoms . Typically if people between the time theyre exposed to develop symptoms is around five days. But as we talked about, it can be as long as 14 days, which is why that incubation period, the quarantine periods are around 14 days. They want to wait that long to make sure someone hasnt developed symptoms. Around five days is the average. Kristen in arkansas, people keep comparing the coronavirus to the flu, what is the main difference . There is a lot of similarities. Both respiratory viruses. Theyre both primarily spread through respiratory droplets, people cough or sneeze and those droplets then can be transmitted to someone else or get on a surface. Unlike measles, where the little viral particles can attach to dust and stay airborne for a while, neither flu nor this coronavirus seem to behave that way. They both have incubation periods like i mentioned five days average for the coronavirus, flu a little shorter, three days which is why it tends to spread a little bit more quickly, big difference, one we talked about for some time, the fatality ratio with flu is around. 1 . So many people get the flu, which is why in this country you already had some 16 to 18,000 deaths. Coronavirus fatality ratio so far predicted closer to 2 , so that is a lot higher, still low, but 20 times higher than flu. I want to ask a followup to sherrys question. She asked how long did it take to get symptoms. Want to ask how long do you stay sick because i understand there is a new answer to that question. Yeah, thats right. So for people with mild illness, Recovery Period typically is two to three weeks. For people with more severe or Critical Illness to recover it is closer to six weeks. So thats important for a couple of reasons. Thats that can be miserable for the patient for a long period of time. They can still be contagious during that time. And as we talked so much about the impact on the healthcare system, you know, it is not just a few days in the hospital, a few days necessarily of intensive care unit care, it can be a lot longer than that. Thats something that hospital officials have to think about in terms of their planning for new illnesses. Chelsea in kansas wants to know, are pregnant women at higher risk . There has not been a lot of data so far on pregnant women. Again, the largest studies on this novel this new coronavirus out of china, we do look to sars and mers, the middle east respiratory syndrome, to try to get some includes because, alisyn, those are both coronaviruses as well. We know women at the time theyre pregnant can be their immune systems can be somewhat diminished, they can be more at risk. Overall they may be more at risk, but there is no evidence that theyre passing the virus on to the fetus during pregnancy. Right. So monique from california writes, we keep hearing that the majority of people are surviving coronavirus. How many people have contracted it, and have recovered normal health. This will be hard for you to answer, given all the testing questions. Right. It is hard to answer because we dont have full vision on exactly how widespread this is. What most people are pointing to, guys, is the is the largest study of some 70,000 patients out of china, and this is where a lot of the numbers that we have been talking about are coming from. About 80 of people will have mild illness. Out of that study, there was 80 of them did recover within a couple of weeks. 14 had severe illness, and it took longer for them to recover and it was about 5 that had more Critical Illness. Now, there were no deaths reported in the mild or severe illness groups, which is about 95 of people, all the deaths did occur in people who had Critical Illness, which kind of makes sense. But all of that is to say that most people do recover. It can be longer to recover, depending on the severity of your illness, the vast majority of people who contracted this virus anywhere around the world have eventually recovered from it. Sanjay, i have one more question to build on kristens from fayetteville, in terms of the symptoms, is this like flu where you see vomiting, achiness, or is this more chest, coughing, respiratory distress . Seems to be more the latter, alisyn. Again, we looked at the study very carefully, there was a small percentage of people that had more of the gastrointestinal type symptoms. Thats possible. But a lot less likely. And ill take it a step further, when you really dig into this data, it did tend to be more lower respiratory symptoms, particularly in the patients who had severe or Critical Illness. What that means is more likely to feel just winded or short of breath going up a flight of stairs or a block if you had runny nose, or a sore throat, that was less likely to be coronavirus. Not that they couldnt overlap, but less likely to be coronavirus if it is upper respiratory. Really helpful. The last five minutes a window into sanjays life these days. I know.
