Where people want to be to meet, to laugh, and its just an amazing place to be. Theres a number of different exhibits. Theres food, wine, cocktails, and the idea, again, is to give people an opportunity to enjoy what really is, you know, one of the great civic faces in america. When you look from the polk street steps, and you look all the way down the plaza, down market street, daniel burns design, this was meant to be this way. Its really special. The city approached us off the grid to provide food and beverages at the event as kind of the core anchor to encourage people who leave a reason to stay. Its really vibrant. Its really great, just people Walking Around having a good time. This formula is great food, interesting music, and then, we wanted to have something a little more, so we partnered with noise pop, and they brought in some really fun games. We have skeeball, we also have roller skating lessons, and weve got a roller skating rink. If youre a passion jail skeeball player like me, and youre deciding whether youre just going to roll the ball up the middle or take a bank shot. Our goal is to come out and have fun with their neighbors, but our goal is to really see in the comments that its a place where people want to hold their own public event. I think this is a perfect example of all these people working together. Everybodys kind of come together to provide this support and services that they can to activate this area. Theres no one agency or organization that really can make this space come alive on its own, and its really through the collective will, not just of the public sector, but both the public and our business partnerships, our nonprofits partnerships, you know, neighborhood activists. I really like it. Its, like, a great way to get people to find out about local things, cuisine, like, its really great. Its a really good environment, really welcoming. Like, were having a great time. We want to inspire other people to do this, just using a part of the plaza, and its also a good way to introduce people if theyre having a large scale event or small scale event, well direct you to the right people at the commons so you can get your event planned. Being a San Francisco based company, it was really important to connect and engage with san franciscans. How great is it to come out from city hall and enjoy great music, and be able to enjoy a comtail, maybe throw a bocci ball or skee ball. I find third thursdays to be really reinrig rat reinriggating for me. Whether youre in the city hall or financial district or anywhere, just come on down on third thursdays and enjoy the music, enjoy an adult beverage, enjoy the skee ball; enjoy an adult playground, if you. My name is leslie mccray, and i am in outside beauty sales. I have lived in this neighborhood since august of this year. After my fire in my apartment and losing everything, the red cross gave us a list of agencies in the city to reach out to and find out about various programs that could help us get back on our feet, and i signed up for the below market rate program, got my certificate, and started applying and won the housing lottery. This particular building was brandnew, and really, this is the one that i wanted out of everything i applied for. And i came to the open house here, and there were literally hundreds of people looking at the building. And i in my mind, i was, like, how am i ever going to possibly win this . And i did. And when you get that notice that you want, its surreal, and you dont really believe it, and then it sinks in, yeah, i can have it, and im finally good to go; i can stay. My favorite thing about my home, although i miss the charm about the old victorian is everything is brandnew. Its beautiful. My kitchen is amazing. Ive really started to enjoy cooking. I really love that we have a gym onsite. I work out four days a week, and its beautiful working outlooking out over the courtyard that i get to look at. It was hard work to get to the other side, but its well worth it. Im super grateful to the Mayors Office of housing for having this for us. Hi today we have a special edition of building San Francisco, stay safe, what we are going to be talking about San Franciscos earth quakes, what you can do before an earthquake in your home, to be ready and after an earthquake to make sure that you are comfortable staying at home, while the city recovers. The next episode of stay safe, we have Alicia Johnson from San Franciscos department of emergency management. Hi, alicia thanks to coming it is a pleasure to be here with you. I wonder if you could tell us what you think people can do to get ready for what we know is a coming earthquake in San Francisco. Well, one of the most things that people can do is to make sure that you have a plan to communicate with people who live both in and out of state. Having an out of state contact, to call, text or post on your social network is really important and being able to know how you are going to communicate with your friends, and family who live near you, where you might meet them if your home is uninhab hitable. How long do you think that it will be before things are restored to normal in San Francisco. It depends on the severity of the earthquake, we say to provide for 72 hours tha, is three days, and it helps to know that you might be without services for up to a week or more, depending on how heavy the shaking is and how many after shocks we have. What kind of neighborhood and Community Involvement might you want to have before an earthquake to make sure that you are going to able to have the support that you need. It is important to have a Good Relationship with your neighbors and your community. Go to those community events, shop at local businesses, have a reciprocal relationship with them so that you know how to take care of yourself and who you can rely on and who can take care of you. It is important to have a batteryoperated radio in your home so that you can keep track of what is happening in the Community Around and how you can communicate with other people. One of the things that seems important is to have access to your important documents. Yes, it is important to have copies of those and also stored them remotely. So a title to a home, a passport, a drivers license, any type of medical records that you need need, back those up or put them on a remote drive or store them on the cloud, the same is true with any Vital Information on your computer. Back that up and have that on a cloud in case your hard drive does not work any more. In your home you should be prepared as well. Absolutely. Lets take a look at the kinds of things that you might want to have in your home. We have no water, what are we going to do about water . It is important for have extra water in your house, you want to have bottled water or a five gallon container of water able to use on a regular basis, both for bathing and cooking as well as for drinking. We have this big container and also in peoples homes they have a hot water heater. Absolutely, if you clean your hot water heater out regularly you can use that for showering, drinking and bathing as well what other things do people need to have arent their home. It is important to have extra every day items buy a couple extra cans of can food that you can eat without any preparation. Here is a giant can of green giant canned corn. And this, a manual can opener, your electric can opener will not be working not only to have one but to know where to find it in your kitchen. Yes. So in addition to canned goods, we are going to have fresh food and you have to preserve that and i know that we have an ice chest. Having an ice chest on hand is really important because your refrigerator will not be working right away. It is important to have somebody else that can store cold foods so something that you might be able to take with you if you have to leave your home. And here, this is my very own personal emergency supply box for my house. I hope that you have an alternative one at home. Oh, i forgot. And in this is really important, you should have flashlights that have batteries, fresh batteries or hand crank flashlight. I have them right here. Good. Excellent. That is great. Additionally, you are going to want to have candles a whistle, possibly a compass as well. Markers if you want to label things if you need to, to people that you are safe in your home or that you have left your home. I am okay and i will meet you at. Exactly. Exactly. Water proof matches are a great thing to have as well. We have matches here. And my spare glasses. And your spare glasses. If you have medication, you should keep it with you or have access to it. If it needs to be refrigerated make sure that it is in your ice box. Inside, just to point out for you, we have spare batteries. Very important. We have a little first aid kit. And lots of different kinds of batteries. And another spare flashlight. So, alicia what else can we do to prepare our homes for an earthquake so we dont have damage . One of the most important things that you can do is to secure your valuable and breakable items. Make sure that your tv is strapped down to your entertainment cabinet or wall so it does not move. Also important is to make sure that your book case is secure to the wall so that it does not fall over and your valuable and breakables do not break on the ground. Becoming prepared is not that difficult. Taking care of your home, making sure that you have a few extra everyday items on hand helps to make the difference. That contributes dramatically to the way that the city as a whole can recover. Absolutely. If you are able to control your own environment and house and recovery and your neighbors are doing the same the city as a whole will be a more resilient city. We are all proud of living in San Francisco and being prepared helps us stay here. So, thank you so much for joining us today, alicia, i appreciate it. Absolutely, it is my pleasure. And thank you for joining us thank you all for being here today and for your patience. Im london breed, mayor of San Francisco, and i am joined by president of the board of supervisors, supervisor norman yee as well as the director of the department the department of Public Health, dr. Grant pulled koufax and other members of the department of Public Health. We are here today because there was a very unfortunate situation that has occurred at laguna honda pick laguna honda hospital is a place that has taken care of so many vulnerable residents, people like my grandmother, were so many years. My grandmother suffered from dementia and she was at laguna honda for many years before she passed away in 2016. I want to say that the chair and the support and the compassion from so many of the employees at laguna honda meant a lot to me personally and meant a lot to my family, but these particular acts that have occurred, by few individuals will be addressed. Dr. Colfax will provide some of the details in terms of what has happened and we are already taking swift action to address those issues and i want to thank him and his department for being steadfast in the work to address these issues, in addition to providing new leadership to make sure that this doesnt occur again. We are better than this. We are committed to restoring the trust of the families and the patients of laguna honda and we thank you thank you all so much for being here, as well as, as i said, many of the folks who are part of laguna honda, we have work to do and we are committed to making sure that situations like this that have occurred dont happen again. At this time, i would like to ask president of the board of supervisors, norman yee to say a few words. Laguna honda is located in his district. Thank you, mayor. Im kind of speech list when i found out about this situation and im super i am super outraged with what has occurred. Like mayor breed, this is personal to me. I have had family members who stay there. I still have friends whose parents are being cared for. So for this to occur under our watch, i dont have the words to express how upset i am. When people send their loved ones there, they trust, they would like to be able to trust that they will have the best care. For the most part, at laguna honda, i would say that they have received that, but for these few Staff Members that have perpetrated these incidents , i am with dr. Colfax, this is a very quick, quick reaction, not only to find out what happened, but what is more important is that we really need to figure out how to prevent Something Like this from ever happening again because again, when one sends their loved ones there, they want to feel comfortable. They care. They do care what happens, and they have to trust us, and we should do the best we can. Because of that, we are going to call for a hearing at the board of supervisors to find out what exactly happened and what are the measures that will guarantee that this will never happen again. I want to thank the mayor for acting quickly and i want to also thank dr. Colfax for what he has done so far. Thank you very much. Thank you president yee and now dr. Grant colfax to provide more details about what has occurred. Good afternoon. Thank you madam mayor and thank you president yee and thank you all for coming today on such short notice. We are here to report something difficult, disturbing, and disappointing to the people of San Francisco. In february of this year, while investigating and unrelated Personnel Matter, the department uncover the possibility of patient mistreatment at laguna honda hospital. We immediately started an internal investigation and found evidence to substantiate our initial submission suspicions we contacted the San FranciscoCity Attorney to help us conduct a more formal and comprehensive investigation. We reported our evidence to the California Department of Public Health and to the San Francisco police department. And we just we dispatch the quality and safety Team Together all the relevant facts and to make recommendations. We know want to the people of San Francisco to hear directly from us about our findings and what we are going to do about them. We discovered that 23 residents were affected, dating back to 2016. Six Staff Members were involved in they are no longer employed by the department of Public Health. The misconduct we discovered included sexualized conversations with residents, verbal and physical abuse, neglect, privacy violations, medication errors, and the administration of nonprescribed substances to some residents in an attempt to pacify them. This behavior is not something we will tolerate. This behavior is not something i will tolerate. As a caregiver, i am outraged and heartbroken. There are patients that our patients were treated this way. I apologize to them and to their families. In response to our findings, we have immediately taken steps to ensure patient wellness and safety. We have removed the chief executive officer and chief quality officer of laguna honda hospital. We have notified all affected patients and their decisionmakers or their conservators. We have thoroughly examined all affected patients to make sure to ensure their health and wellness. We have informed all relevant regulatory Law Enforcement agencies, we have retrained staff in prevention and preventing patient abuse and we have improved drug security, dispensing, and testing practices. There is no evidence that these abuses continue to pass january of 2019. An extensive investigation and interviews with staff confirm that these abuses were limited to two units of the hospital and to a minority of patients in those units. Today, we are confident that laguna honda continues to be a good place for patients. Especially in the areas the patient care and safety. We want to ensure it continues moving in the right direction, so these circumstances never occur again. Our acting c. E. O. Will lead to the process of recovery from these incidents and we will fully engage our hospital staff and recommending and recommitting to our mission, values and work. We will continue to work in close collaboration with the California Department of health and our cooperate and are corruptly cooperating fully with their investigation. Additionally, the Health Department wonder take a turnaround plan for laguna honda , informed by external experts in longterm care and working in partnership with the community, patients and their families and Health Department leaders. Well submit our plan for moving forward to the mayor, to the board of supervisors and to the Health Commission within 60 days anyone with concerns about patient care or staff conduct is asked to call the Confidential Department of Public Health compliance hotline at 8557296040 855 7296040. Now, the director of the San FranciscoHealth Network and i will answer a few questions as there are still ongoing investigations, we may be limited in what we can say at this time, but we thought it was important the community hear from us directly and that is why were we are here today. Thank you. [indiscernible] all six employees have been removed from employment. There is an Ongoing Police investigation. [indiscernible] they have been removed from those positions and we have a new c. E. O. It is a Personnel Matter and i cant specify any further than that they are removed it from their positions as of today. [indiscernible] as i mentioned, we first became aware of these issues in february of this year, february of 2019, in one of the things that concerns me is that, as a culture, within the context of which these employees were working, that there was a culture of silence and thats why we need to make this change. We need to move from a culture of silence to a culture of safety. [indiscernible] we have reinforced medication prescription and security and i will let mr. Pickens provide you with more details. Thank you dr. Colfax. Im roland pickens, director of the San FranciscoHealth Network in answer to your question in terms of medication management, there are medication rooms on each of the patient units at laguna. Those rooms have computerized medication cabinets that hold the drugs. There are also or medication carts that are used to bring medications out of the room and onto the patient units. Obviously the persons the systems in place at that time did not catch what happened in terms of some of the inappropriate medication use. As dr. Colfax said, we have taken steps to remediate that right away and are continuing to analyse the process. There has been an increased monitoring and oversight by pharmacists at laguna honda. They have doubled their checks going from once a week to once a day, and they will continue as we bring in consultants to really help us analyse what has been the gaps that led to this behavior and what do we need to do to fill those gaps. [indiscernible] yes, thank you so much for asking the question. This is all about keeping our patients safe. Twentythree occasions patients range in ages from 30 to 100. [indiscernible] im sorry, say that again . [indiscernible]. Again, these were patients on what north one and north to and those two units specialize in dementia care. [indiscernible] controlled substances are available, so it is a mixture of controlled and noncontrolled substances. [indiscernible] thank you all so much. We really appreciate it. Thank you. This is a reminder to turn off all electronic devices. The Fire Commission a regular meeting, wednesday, july 10th, 2019, and the time is just after 9 00 a. M. Item one, roll call. [roll call] item two, general Public Comment members of the public may address the commission for up to three minutes on any matter within the commissions jurisdiction and does not appear on the agenda. Speaker should address the remarks to the commission
