Is a good starting place. We are back to where we were on march 31, before we started this dramatic increase in the number of cases. Theye still watching questions, how fast is that number continue to go down . First, does that number continue to go down . That would be a nightmare if it picks up. How fast does it come down and how low does it go . Before we started this, you had a couple hundred cases. When did we get back to a couple hundred new cases going in . The overall hospitalization rate is down, number of innovations is down of intubations is down. Even the number of new covid cases is down. Still not good, still 1000 new cases yesterday, to put it in focus. That would normally be terrible news, it is not terrible compared to where we were. This is just terrible news. 367 deaths. Horrific and there is no relative context to death. Death is death. Families. Passed, 367 what i have been working from day one is to make sure people understand the facts of what we are dealing with. This is a unique situation. Unless thecannot act action. Ully support the what we have done here, government could not do. It was a pure function of what people did. Been, give people the facts, and if they have the facts, they lack responsibly. But they have to have the facts. They have to buy into the plan. And it really is yet individual decision. Who is taking care of your health . You are. We are mutually dependent and that what i do can affect your health. But it comes down to giving everybody the information so people can make their own decision. The great achievement in this period has been that when people get the facts and they trust the facts and understand the facts, they do the right thing. That is a lesson that i hope people remember after this is over. But we still have to remember the facts. We talked about reopening, talk about reimagining. Lets start to put some meat on the bones so people understand. The federal guidance from the cdc is before you start reopening the state, the Regional Hospital rate must be in decline for 14 days. The federal government leaves it up to the states, but they also give guidance. In this case, i think the cdc guidance is right. We are monitoring the hospitalization rate, monitoring the Regional Hospitalization rates. In this state, it is a diverse state. Upstate regions are like states in the midwest, even out west. We have different hospitalization rates. We look at the rates across the state as well as across regions. We will reopen in phases. A regional analysis on what we call our economic regions that we have been working with the state on and those regions have been working together on economic policy, so to analyze the regions, which are existing theitions, but look at regional analysis, make a determination, and then monitor whatever you do. Phase one of reopening will involve construction and manufacturing activities. Within construction and manufacturing, those businesses that have a low risk. There is a range of construction activities, a range of manufacturing activities. Those businesses that pose a low risk within them. A business byd be business analysis, using the matrix we discussed, how essential a service does that business provide, and how risky is that business . Howou reopen that business, much risk are you possibly incurring . How important is it that that business reopen . That matrix will be guiding us through phase two. In phase two, when we get there, we need businesses to do that analysis. They have to think about how they are going to reopen with this new normal. What precautions are they going to take in the workplace . What safeguards are they going to put in place . Very much going to be up to businesses. Then we are going to leave two weeks between phases so we can monitor the effect of what we just did. Take an action, monitor. Two weeks, that is according to intubation the period of the virus. You can see if you had an effect where you increased the rate of infection, which you would see in hospitalizations, testing, etc. Everyone understands the overall risk that you start to increase activity, infection rate goes up, two weeks to do that monitoring. Outlines ofbroad the reopening plan. Then you get into cap yachts. At isap yacht one caveo you cannot do anything in one region that would increase the number of visitors to that region. You have a multistate region in lockdown. It is possible you open something in syracuse, open something in the north country, where you now see license plates coming in from connecticut, new jersey, from downstate, all coming to that area because they have been lockdown and they are looking for they have been locked down and they are looking for an activity. All of this is done in a multistate context with our neighboring states. Most relevant downstate. Downstate is obviously the most complicated situation. That is new york city, westchester, surrounding suburbs. Coordination is vital there because the new jersey, connecticut, new york city area is very intermixed, people are going and coming, they live in one place, they work in the other. That correlation is important. You get upstate, coordination with massachusetts is more important in some areas. Some parts, pennsylvania coordination is more important. Coordinating with those states that our neighbors to that region. Downstate, we have to coordinate the main activities. There are gears that enter mesh, you cannot turn one gear without turning the other gears. That is how you strip gears. I keep trying to explain to my daughters. Transportation, parks, schools, beaches, they are all coordinated activities. You turn one, they all have to turn. City,s true for new york westchester, they all have to be coordinated. We are working with local businesses and leaders to do that. On the phase the reopening, does my business reopen what is your business . How would you do your business . In this new normal . You normally have people in the Conference Room or go to do that, you have people in workstations that are right next to each other. Do you plan on reopening that way . A onesided equation. Businesses, you develop a plan on how you would reopen given everything we now know. If you have a plan i will show you in a moment that takes into consideration these circumstances, more essential, lower risk. The way a business will open will determine the risk. They cannot determine how essential their service is, but they can determine how risky opening them up opening their business would be. Also, we need them to be creative and think outside of the box. We have been speaking with Business Leaders across the state. But some people even need a new economic model. We want to bring sports back. This is an activity that people can watch on tv. What sports can you do without an audience . What sports can you make work economically where you dont have to sell a seat in the stadium or in the arena . How do you do driveins . Hown types of businesses do you do different types of businesses that could work in this environment . They have to be creative and think about it. Downstate, which does have particular needs, we need some activities in downstate new york. You cant tell people in a dense, urban environment through the summer months, we dont have anything for you to do, stay in your apartment with the three kids. That does not work. There is a sanity equation here also that we have to take into consideration. Special attention for Public Housing residents, special attention for low income communities that paid a higher price for this disease than anyone else, we talk about racial disparity, africanamericans and latinos, we are increasing testing, we will have more to say on that this week. But low income communities need more assistance through this. I also need to focus we need to focus on the basics. We need more food banks, more food assistance, there are people who are struggling for food and childcare. We have to make that more available and i want to bring in , in a coordinated way, philanthropies. A lot of people want to help. Philanthropies represent the positive force in our society. I want to make them part of this in a partnership and we are doing that. ,ooking through all of this with partners in the Business Community and health care community. Andave an Economic Strategy a Public Health strategy. It has to work with businesses who are creative and more thoughtful, and Health Care Professionals who learned an important lesson with what we went through. I dont want to lose that lesson. In terms of businesses thinking about the new normal, think about it in terms of people, how are you going to protect your people . What are you going to be doing differently with your employees . Your actual, physical space . What does the physical space look like when you be open in this new normal when you reopen in this new normal . How are you cleaning . What is the hygiene . What is the access . What is the screening . How do you move people . What is the travel and transportation . What processes can you put in place to make your business less risky . How can you train people . How can you communicate about this disease . Can you do testing in your workplace . These are all factors for businesses to consider that want to reopen quickly. It is is between governmental decisions, in partnership with business decisions. I think every Business Leader gets, you cant go back to where you were, we have to go back in light of the circumstances that have developed. This, midst of all monitor the Public Health impact. All the progress we made by flattening the curb, we could lose that in a matter of days if we are not careful. It is important that people understand what that means. This gets a little technical, but it is worth understanding. If i can understand it, anybody can. They talk about the rate of increase of the spread of the infection. R0 factor. It is basically straightforward. Less thanson infects one person, that is the first category of r0. Up is one person infects one person who then infects another person. To one to one to one to one. The worst situation is one person infects two people and those two people in fact two people. That is fire to dry grass. Now you are in bad shape. That is where we were when this started. There before we even knew we were there. Insight that the disease came from europe to new york because it was already out of china by the time we realized it, it went to europe, went to italy, went to the lombardi district, got on a plane, came to new york, and was here earlier than we knew and it was spreading earlier than we knew at a higher rate. To the onebe down person infects one other person. We cant really go beyond that margin. Right now, we are at 0. 8. One person infects 0. 8 of a person. One person is infecting less than one person. That is good news. At that rate, you see the virus declining. Upstate, interestingly, it 0. 9. Statistically, very close. But upstate, the infection rate is one person infects 0. 9 . Downstate, one person infects 0. 75 . That is where we are across the state. If we keep the infection rate although one person infecting one person, that is where the infection rate continues to drop. That is where you will see the curve dropping. We have to stay there. How do you monitor it . You have three basic tiles. Number of hospitalizations, which you see every day, which i show you every day, number of hospitalizations. You can see that by region. Number of positive antibody tests. This is why testing is important. We are doing Antibody Testing around the state and regions. Antibody testing tells you how many people have been infected. Little bit of a lag because you only have the antibodies after you have had the virus, but it tells you on a lag basis how many people have been infected. Is the diagnostic tests, which are positive or negative. Yourtell you what infection rate is. Those are the three dials you are watching. Valve,e your hand on the the activity valve. You open the valve a little bit, phase one, watch those dials like a hawk, then you adjust. Factor,called the rt rate of transmission factor. What is the rate of transmission of the virus . We are now at 0. 8. You cannot go above 1. 2. 1. 2, you see that number go back up again. We will be sitting here, talking that showing you a chart showed up, down, then up again. Thats what happens if the rate of transmission gets to 1. 2. Wes is a balance that collectively need to strike. I want to get back to work, my kids want out of the house, i need to do something, i understand. Intelligently,t and that is this is the definition of intelligence in this context. I dont want to just reopen. We learned a lot of lessons here. Painfully, but we learned a lot of lessons. How do wewe take the lessons we learned, take this pause in life, and say, when we reopen, we are going to be better for it . And we are going to reimagine what our life is and we are going to improve for this pause . Level, you hadl time to decompress a little bit or compress for a different set of factors. I think everyone went through a period where they analyzed their life and what they were doing and when somebody pulled the rug from under you and you wind up in a different place, you see life differently. I think that is true for most people. After that, how do you what have we learned . How do we improve and how do we build back better . It is not about return to yesterday. There is no return to yesterday in life. It is about moving forward. It is about taking your experience and what you learned and bringing it to a positive effect. To end on i want sharing a story that taught me a lot. There is a tunnel in new york called the l train tunnel. It is a tunnel that connects manhattan and brooklyn. Train0 people use this and this tunnel. Group0 people is a larger than many cities in this country have. They had to close down the tunnel because the tunnel was ,ld and it had problems everybody looked at it and said, we have to close down the tunnel. 400,000 people could not get to work and they had all these complicated plans on how they were going to mitigate the transportation problem in different buses and cars and , the wholeorses alternative transportation. This went on for years. Everybody said, you have to close the tunnel and it would be close from 15 to 18 months. When the government says it will be close from 15 to 18 months, i here 24 months to the rest of your life. But that was the plan, we will close it, rebuild the tunnel 15 months to 18 months. This was going to be a massive disruption. I heard a lot of complaints, i people, cornell engineers, columbia engineers, we go down into the tunnel and we look at it and the engineers say, there is a different way to do this. They talk about techniques that they used in europe and they say, not only could we bring these techniques here and we would not have to shut down the tunnel at all. We could just stop music at nights and weekends and we can make all of the repairs. And we can do it with a partial closure for 15 months. To this new idea was an explosion. Have a meddler, i did not an engineering degree, they were outside experts, how dare you question the bureaucracy the bureaucracy knows better. Thunderstorm of opposition. But we did it anyway and we went ahead with it. We rebuilt the tunnel and the thanl is now done, better before, with all these new techniques. It opens today. And the proof is in the pudding. We went through this period of, i dont believe it, this is interference, it opened today. Not in 15 months, but in only 12 months of a partial shutdown. It is ahead of schedule, it is under budget, and it was never shut down. I relate this story i relay this story because you can question and should question why we do what we do. Way . O we do it that i know that is how we have always done it, but why do we do it that way . And why cant we do it a different way . Why not try this . Why not try that . People dont like change. We think we like change, but we dont really like change. We like control more than anything. So it is hard to make change. It is hard to make change in your own life, let alone on a societal, collective level. But if you dont change, you dont grow. If you dont run the risk of change, you dont have the benefit of advancement. Not everything out there has to be the way it is. Wildst went through this period where people are Walking Around with masks, not because i said to, but because they understand they need to. How do we make it better . Period to do just that. And we will. Realitymake it a because we are new york top and smart and disciplined and unified and loving and because we know that we can. We know that we can. We showed that we can. Questions . It seems to be a philosophical approach to reopen, but when can people expect dates and hard facts . Gov. Cuomo you have them there. 15 you have may 15. The pause is statewide until may 15. Then you have the cdc guidance that says total hospitalizations declining for 14 days. 15, what regions have seen a decline for 14 days . We assume we will see a decline in the state for 14 days. Regions of the state have seen a decline for 14 days . That is where you will start the conversation to get to phase one in that region. That would be more likely able to open sooner would be the upstate regions. You take central new york, the north country, the mohawk valley, those regions have seen lower numbers from day one. You would talk about phase one opening there, which is construction and manufacturing. At, dont doe anything that will ring people in from all across the board. From newhave people york city and massachusetts and connecticut coming in. Also, construction by manufacturing is a business by business analysis, not all construction is the same, what are those businesses doing to incorporate the safe procedures . You what see that more likely in the upstate area as if those numbers continue to decline the way the cdc recommends. Downstate will be more complicated. You cant do anything in new york city that you dont do in nassau, westchester. You cant do anything and downstate that we dont do in coronation in coordination with connecticut and new jersey. Coordination does not mean total consistency, but it does mean what a nation. We have to know if we are doing something different. We have had issues where certain activities were open in connecticut and you saw many new york license plates in the connecticut parking lots. We opened beaches in new york city, but new jersey does not opened beaches, you are seeing new jersey people at our beaches. Be in lockstep, but before we do anything, i want to make sure we know what we are doing, because you will see people react to different activity levels in different communities. See manufacturing and construction start to come online inserted upstate regions after may 15 . Gov. Cuomo with certain precautions after may 15, yes. [inaudible] gov. Cuomo schools are necessary for a largescale business we opening. You cannot really get to a maximum phase two without opening schools. Question on schools is going to be, do you you open for the rest of the school year . Then there will be a question, many local School Districts are talking about Summer School. They are contemplating Summer School to make up for some of the lost time. You have a real problem. This Remote Learning is going to be one of the lessons we learn. Motor learning is great in concept Remote Learning is quite in concept. We had to jump into it with both feet and we did not have a chance to scale up for it. Many of School Districts we have 700 School Districts, many are saying they want to do Summer School. They want to make up for some of their class time. That, we have to feel out as we go. On many of these things, im not comfortable getting too far ahead of ourselves. You want to talk about a two week window . I think that is an intelligent window to talk about. Anybody who says, im going to tell you what will happen a month down the road i would not believe that person. Districts are trying to fit together their budgets. There is a blood basis cut coming. What guidance for them . What should they expect . They need to put together a budget. You said 20 . Is that the right number . Does it have a number . Gov. Cuomo what number should they do . Be 20 or additional reduction. May 1 wasy form the first checkpoint. On may 15, we will know where we are with washington and whether we have more money. We will release a plan by the middle of may that gives time for the School Districts to make their budgets. Between now and may 15, we will have more clarity in a financial plan. We are letting you know, this is where we stand, this is where the revenue picture is, those are the facts. By the time they put their budget votes together, it will have those numbers. [inaudible] your executive order puts the School Budget vote after june 1. Do we know when they will be . How they will be done, by mail, i born . Gov. Cuomo we have not gotten there yet. I heard a lot about businesses and private sectors, but have you been in touch with the state workers, Union Leaders . Have you gotten pushback, we dont want to be the guinea pigs or the first ones to our workers are the first ones out at work . Ofe you laid out a plan social distancing as state Office Buildings . I dont see it in the presentation. Gov. Cuomo when you talk about opening a Government Office that is not now open, we are part of that same analysis. We would not open any Government Office that does not have all those social distancing measures, the configuration of the workplace, etc. We would do the same thing we are asking businesses to do. Are you planning anything . Gov. Cuomo we are planning that. We come back in phases, reduced workforce, more space, etc. We have certain functions that are more essential than others also. Same thing im asking business to do, we are doing. It would be easy for me to say, every state agency is essential. But there are every state agency is essential, otherwise we would not have it, but there are some that are more essential than others in a shortterm basis. Divisioniness, every is essential, i get it. , do you have more essential divisions than others that you could bring back and do special distancing in your workplace . Here, that is implicit, but let me make it explicit in case im wrong. The big factor is what people do. That has been the factor from day one. Spend so much you times going through the fact . I need people to understand so they actually do it. Were not do it. Work not do it. I gave them the facts on why i think the need to wear a mask. If people dont wear a mask, do we have the ability to force people to wear a mask, 19 Million People . No. Understand it and if they understand it, they will do the right thing. If they think my fax dont add up, then they dont wear the mac dont wear the mask. On this phase one opening, monitor, monitor, monitor, that assumes they are going to be responsible in their behavior. That they can now go out and go to a new store that opened. Ok, if they are wearing a mask and they are using cleaning their hands, that is one scenario. If they go to work and wear a mask and keep social distancing and dont get sloppy, dont get undisciplined, that is one situation. That behavior changes, im telling you, we are talking about a window of 0. 81. 2. Light blow through that wind through reads. It depends on what people do. How smart, how disciplined they are through this process. [inaudible] so that you can set up a scenario that would work . Gov. Cuomo we will do that. We will show a workplace that is socially distant, people wear their masks, they dont go to a cafeteria, there is no, get, the transportation is accommodated, we will show that in what we do. To makecan the state do sure businesses are complying with regulations . Gov. Cuomo they have to give us a plan before we say they can reopen. Nowre talking to businesses , literally individual businesses. We have groups that have been set up that our meeting with businesses, talking to businesses. Secretary, a former has been doing this for weeks. Were at a briefing, dealt with your questions, ran away, i dont know that there was a connection between the two. Theyve been talking with businesses, coming up with individual plans. Even construction, manufacturing, what construction, how would you operate . Manufacturing can be anything. Operate your business in this new normal . That is business by business. How is the state how does the state keep track of new transmissions while phase one is in effect . Gov. Cuomo with the three dials. You have hospitalization rate, which you know now. We have a new system in place, every hospital in the state on a daytoday basis gives us their hospitalization rates. Two is the Antibody Testing. Which we are now bringing up to scale in a large number, statewide and regional. Antibody testing tells you how many people were infected with a two week lag, roughly two weeks to develop antibodies. Diagnosticial is testing statewide and by region. This is what we got last week when we tested, last week we were in buffalo and tested 3 of the population were positive. This week, 4 of the population is positive. Maybe that is statistically relevant, irrelevant. Four days later, 5 . Close down the valve. You have those three dials. Those three dials, hospitalization rate, Antibody Testing rate, positive diagnostic testing rate, they give you the rate of transmission. Which is, how fast is the virus spreading . What have they done in the regions gov. Cuomo thats why we have said, all of this is dependent on testing. Without testing, you have one dial. You only have the hospitalization dial. The hospitalization dial is a little misleading. It tells you, people who got infected in some period over the past two weeks or maybe three weeks, and who are seriously infected, so they had to go to a hospital. But thats all that hospitalization rate gives you. Test and diagnostic test give you a higher level of information by region. The Westchester County center was one of the areas one of the facilities that was crafted into a temporary hospital. If the infection rate is going down. Work has been completed. They have no patience, as far as i know. What you do with that facility, that does not appear to be much use for it now . Gov. Cuomo today, we are in good shape. Today, that would be a two statement. Do you know if that will be a true statement in 60 days . So what would you do with the facility . You are the governor. Gov. Cuomo but what would you do . Is there a possibility for transfer there are nursing home patients and residents, could you move people out of nursing home . Gov. Cuomo nursing home patients lets go back to square one. A nursing home can only provide care for a patient who they they can provide adequate care for. If they cannot provide adequate adequate care, they must transfer that patient. If they cannot find an alternative nursing home or facility, the department of health will find an alternative nursing home or facility. Have they can seize vacancies. There is no nursing home who says, i have john, i cant take care of john, i dont know where to send him call the department of health and we will find a bed for john in a facility that can take care of him. That is how it works. There was one story that said a nursing home said they wanted to send someone to the comfort, the ship, the department of health said they cant send them to the comfort. Yes, the nursing home cannot send a person to the comfort because thats not the agreement with the federal government, comfort only took referrals from hospitals. But forget the comfort. If a nursing home because of and says, i need to send i cant take care of this person. We will find a bed for that person. Doesnt that isnt that a mix message from the march 25 memo that you cannot deny admission just because of a suspected covid19 . Gov. Cuomo no, you were in that facility, you have covid, you are back in that facility. If they are going to care for , that facility must quarantine, must have ppe for staff, must follow these guidelines. Or they say, i cant take care of john. Im not equipped, i dont have the space to quarantine him, i dont have the staff, i dont have the ppe, i cant take care of john. Im going to transferred onto another facility or call the department of health and tell them to pick up john. But if you keep john and i am paying you to keep john or johns family is paying you to keep john then you have to give john the appropriate care. Here is appropriate care for a person who is covid. Alec baldwin released a video calling on you to give come and see two people in the prison system. Do you have plans to do that . Gov. Cuomo we have been doing that. As far as you granting gov. Cuomo based on what . Based on vulnerable populations, pregnant inmates. Gov. Cuomo how about if they are violent and started their sentence just started their sentence . Arehat about prisoners who your the end of their term gov. Cuomo that, we have been doing. How can you help forms that are struggling upstate . Farms that are struggling upstate . Gov. Cuomo the department of agriculture is looking at that. These stores of dumping build, i dont understand what the economic forces are, where they have to much. We talked to the department of agriculture. If there is a way we can buy it we are talking about food banks in new york city for low income communities. And we talked about dumping milk in upstate new york. I dont get it. That is the exact issue we are looking at. If there is a way the state can purchase part of these four programs, for example these food bank programs, for example, we will do that. New York City Schools bought a lot of the product from upstate new york because we work hard to develop that relationship. Ofarently, the purchasing the schools is down or it is different purchasing. I dont know the specifics. Anyway we can fix that, we will. [inaudible] is there updates on the data . Wheezy Domestic Violence going up we see Domestic Violence going up. We see trends in alcoholism throwing up. We see some trends that say drug usage is going up. Mental health needs are going up. Stressunderestimate the that this situation has created. The abnormal circumstances that it has created i dont know if i have a job, im not getting a paycheck, the bills are still coming in, i see no light at the end of the tunnel, and im and imp in my home under tremendous stress and then i have this added stress of being in a situation that ive never been in before. Hat is a toxic mix when we talk about getting people out, some activities, a place where people can walk, i saything tongueincheek the senate index. Theneed to know sanity index. People need to know that there is hope, somebody is doing something. Then you need a relief valve on a daytoday basis so people have some relief in their lives. Saidst about everyone has this process has amplified how Important Health care workers are. [inaudible] gov. Cuomo that is a good point. I dont know if we can get to situationem with this. Im aware of it. I dont know how we can help those people through here. It is something we will look at. It would be difficult. [inaudible] as far as investigating to find up who was responsible, is that something that should be [inaudible] gov. Cuomo for sure. Did you think tomorrow, the democratic commissioners of the board of elections, should cancel the democratic primary in new york . Gov. Cuomo i have not express any opinion for them. Is up to the voa. Gov. Cuomo i am not that opinionated. You know me. You roadway end . You wont buy in . In . Ou wont weigh why not use the decline of the death rate as a bellwether to reopen instead of hospitalizations . Gov. Cuomo we look at both. We look at the death rate by region and we look at the hospitalizations. They basically track more or less. Do we know statistically what the variance is between the two . We look at hospitalization because that is the front and so you know the capacity on the system. 25 to as a ratio about 30 of all hospitalizations in due to covid end up in icu. 90 of those people end up on intubation and that rate of death is fairly high. Process thatentire factors in to the dial of reopen, not reopen. Gov. Cuomo remember the train comes off the tracks here. The train comes off the tracks when you overwhelm the health care system. Thats when every thing goes bad. That was italy. Thats what we were worried about. That infection rate keeps going up, you will have a death rate associated with that infection rate. First, you will have a collapse of the hospital system. You will have people in hallways , on gurneys who are not getting cared for. Ofts what we were petrified and thats why we boosted all the hospital capacity, these extra facilities. Thats why that hospitalization rate, 1000 people walked into a hospital today, that number of people walking in, that number intotoo high, you go emergency rooms, we go back to where we were, shoving equipment all across the system, shuttling patients across the system, that is the first indicator warning sign. My question is related to the opening. [inaudible] the cdc came up with the hospitalization rate. I dont know their methodology, to tell you the truth. We do use both. We look at both. What would you say to Business Owners who are being clobbered by this economically . What words of hope, which of comfort can you offer them . Date . S will be over by x gov. Cuomo nobody has asked state. Ate. Obody has x d dr. Fauci says there may be a second wave, it could come back in the fall. Nobody is giving anybody a date. Areshortterm, the numbers on the decline, everything we have done is working. The rates are all dropping. The hospitals did phenomenal work but they did not get overwhelmed and we coordinated the healthcare services. The policies are working. We are now looking at a relatively short period of time, 14 days of decline. If you dont have 14 days of , beine, nobody would say reckless and open anyway. That does not help any Business Leader. Incrementally, which is what everybody says. Construction and manufacturing.
