Published 3 hours ago • Updated 3 hours ago NBC Connecticut The Occupational Safety and Health Administration released a report Wednesday blaming a lack of safeguards at the Veterans Affairs Hospital in West Haven for the deaths of two men in a steam explosion in November 2020. The two men, 60-year-old Euel Sims, an employee of the VA Facility Maintenance Service, and 36-year-old Joseph O Donnell, a contractor working for Mulvaney Mechanical Inc. were killed as they worked on a steam boiler at the West Haven facility on Nov. 13. One local veteran was overcome with emotions after being walked through his house that had been fully renovated over the weekend. Retired Staff Sgt. Dave Caillouet is a former 19K battle tanker soldier who served many years at Fort Hood in the late 1990s and early 2000s. He lives alone with his faithful companion and service animal Otto, a 90 pound, robust black lab with the energy of a much younger dog, according to a news release from Surplus Americans, the nonprofit leading the renovation. Surplus Americans, a San Marcos based veteran nonprofit founded in 2016, were the leaders on the renovation in connection with many other local organizations including the Fort Hood Area Habitat for Humanity. Founders Hunter Bruce and Jason White started the nonprofit immediately after the closure of The Brass House, Jazz and Blues Club, in Austin, where they both worked for many years. ....

(AP) An ousted chief of staff at a Veterans Affairs hospital has been selected as the chairman of the West Virginia Republican Party. Members of the state Republican Executive Committee chose Raleigh County Republican Executive Committee Chairman Mark Harris on Saturday in Charleston, The Intelligencer and Wheeling News-Register reported. Harris was selected over Conrad Lucas, a former state GOP chairman. Harris replaces Melody Potter, who resigned in January, citing the health of her parents. Harris was chief of staff at the Beckley VA Medical Center from 2017 to 2019. He was dismissed after an investigation into sexual assault charges against a former doctor. Jonathan Yates was sentenced in January to 25 years in federal prison after pleading guilty to charges that he molested three male patients and violated their civil rights. ....
