Roofing specialist fined after worker’s death
A roofing company has been fined £75,000 over the corporate manslaughter of an employee.
Hugh Thomas Jack, 59, fell through an unsecured section of roof while carrying out cladding work for H&M Engineering & Roofing Specialists.
The incident occurred at industrial premises in Carryduff, County Down, Northern Ireland. The work being undertaken at the time of the incident involved the over-cladding of an existing roof structure with new sheeting.
An investigation by the Health and Safety Executive Northern Ireland (HSENI) investigation revealed that the company had failed to make a suitable and sufficient assessment of the risks to the health and safety of their employees. ....

A roofing company and its director have been fined £75,000 for what a judge described as “gross negligence and slipshod practices’’ which led to the death an employee.
Director Stephen Heaney pleaded guilty last month, on behalf of Ballymena-based H&M Engineering and Roofing Specialists (NI) Ltd, to the corporate manslaughter of Hugh Thomas Jack four years ago.
The 52-year-old also admitted failing to ensure the health, safety and welfare of employees, failing to make appropriate risk assessments, failing to plan and supervise work safely, failing to take suitable measures to prevent a person from falling and failing to report to health and safety authorities within a reasonable time of Mr Jack’s death. ....

On Wednesday morning, an Occupational Health and Safety (OHS) inspector from the Department of Employment and Labour visited a construction site at Da Gama Court, an apartment complex on De Lorentz Street in Cape Town, and issued the contractor – Levenbach Building & Roofing – with a “prohibition notice” that work may not continue until working conditions had been resolved.

Daily Maverick sent the department queries about the precarious safety conditions of workers on the roof of the apartment complex. The OHS inspector found that Levenbach had not complied with the requirements of the Construction Regulations of 2014, and the 75-year-old company was served with a prohibition notice. ....
