Illinois State Police (ISP) reported a woman from Palatine was killed in a crash involving five vehicles on the eastbound Jane Addams Tollway (I-90) Thursday afternoon, April 27. According to ISP, Carolyn Van Slyck, 40, was pronounced dead at the scene after her vehicle was rear-ended around milepost 66.75 near Schaumburg at approximately 3:23 p.m.
