Stevedoring company ISO Ltd has been convicted and ordered to undertake significant health and safety improvements in relation to the death of a Gisborne employee Shannon Brooke Rangihuna-Kemp.
It must also pay her family $100,000.
Rangihuna-Kemp, 29, an ISO Ltd stevedore worker at Eastland Port, died there from crush injuries after she was hit by a log that fell from a trailer load she was about to scan in a tally lane just before 9am on October 8, 2018.
A WorkSafe investigation found numerous routine hazards in the tallying process and that ISO Ltd already knew about the hazard of logs potentially falling from trailers, but had failed to take steps to alleviate risks and protect its workers. ....

Kokiri told police he was intoxicated or high on meth and was “not thinking”.
The now 20-year-old previously pleaded guilty to 15 charges of burglary, using a document for a pecuniary advantage, being an unlicensed driver forbidden to drive and unlawfully getting into a motor vehicle. ....
