The new St. Augustine Community Based Outpatient Clinic just off State Road 207 in St. Augustine is expected to become operational on or about June 27, according to Melanie Thomas, a spokeswoman for the North Florida/South Georgia Veterans Health System.
The facility, at 100 Deerfield Preserve Blvd., will be called the Leo C. Chase Jr. Department of Veterans Affairs Clinic in honor of St. Johns County’s first solider to die in the Vietnam War.
Administrators plan to begin moving items from the temporary clinic at 195 Southpark Blvd. about a week before the opening date.
The new building, which broke ground in November 2019, will offer primary care, physical therapy, mental health services and specialty care to area veterans.  ....
