Hawaii Attack Boat CO Removed Amid Command Climate Investigation After Sailor Death

The commander of a Hawaii-based submarine was removed during an investigation prompted with the suicide of a sailor last month.

Charlotte (SSN-766), was relieved by Submarine Squadron 7 commander Capt. Michael Majewski “due to a loss of confidence in his ability to serve as commanding officer,” reads a late Wednesday release from the Navy.

A Navy official told USNI News the relief was due in large part to “command climate and leadership failures,” revealed in an investigation following the March 17 death by suicide of a sailor on watch aboard the submarine. ....

A Navy sailor died by an apparent suicide on Monday at the Pearl Harbor Naval Shipyard, the service announced.
Torpedoman’s Mate 3rd Class Petty Officer Manuel Julian Jr., 23, “died from an apparent self-inflicted gunshot wound while on duty” in the shipyard, the Navy said in a news release. ....

March 17 (UPI) An inquiry will begin in the death, by apparent self-inflicted gunshot, of a sailor stationed at Pearl Harbor Naval Shipyard, Hawaii, the U.S. Navy announced.
The unidentified sailor was aboard a submarine when the shooting occurred on Monday, the Navy said, adding that the Naval Criminal Investigative Service will investigate.

Out of respect for the family the name will not be released until 24 hours after next of kin have been notified, Cmdr. Cindy Fields, a spokeswoman for Submarine Force, U.S. Pacific Fleet, told Stars and Stripes. ....
