The devastating toll being felt by front line workers in the United States, desperately trying to save lives during this pandemic. Has recorded its worst day ever for covid19 deaths, a staggering 3,100 fatalities were reported on wednesday. That is 20 higher than the previous record set on april 15th. And with a potential vaccine still months away for most americans, many thousands more could die by next march. The head of the u. S. Centers for Disease Control and prevention did not attempt to sugar coat the grim forecast. Were in that range potentially no, starting to see 1,500 to 2,000 to 2,500 deaths a day from this virus. So yeah, the mortality concerns are real. And i do think unfortunately before we see february we could be close to 450,000 americans who have died from this virus. And more than 100,000 americans are so sick theyre in the hospital. Thats also a record. Some u. S. Medical facilities say they have run out of beds to handle the rapid influx of new patients. U. S. Regulators will review pfizers application for an emergency use authorization on december 10th. If it gets the okay, the Vaccine Distribution could begin almost immediately. We get the latest from cnns nick watt. Reporter a global first, the brits just authorized the biontech covid19 vaccine. December 10th, an fda panel meets, u. S. Authorization could come within days. And then. Distribution to the American People becomes immediate within 24 hours. We expect if all safety and efficacy approvals are granted, those doses will arrive on december 15th. Modernas vaccine is about a week behind. The plan, vaccinate 20 million americans in december, 30 million in january. 50 million in february. By march 1st. We will have potentially immunized 100 Million People which is really more or less the size of the significant at risk population. The director of the cdc warns the next three months will be the most difficult in u. S. History, as hospitalizations set a record at 100,000 plus and deaths soar above 2,600 a day. Thanksgiving and Christmas Travel will make this even worse, yet more americans ignore Public Health advice. What needs to be done here, and wheres all this going, do you think . Yeah, its hard to know what to ask of people anymore. I mean, we have been pushing throughout the fall, headed into these winter holidays for people to really listen to Public Health recommendations, but very strict about facebook wearing and social distancing, and yet people have been gathering and of course we all have fatigue. But i think all we can do is try to take some measure of hope from these incoming vaccines and ask for people to do more of the same, but this time with hope and a very specific time line in mind, and im hoping that the promise of these vaccines, sometime in the new year will give people the extra energy they need to really buckle down and try to get us through the virus. Between now and then, we need to save as many lives as possible. Yeah, lets just try to do that, as you say, and doctor, yesterday, the uk approved Pfizers Covid vaccine. They are ready to start vaccinating next week. They have been criticized for moving too fast, even though they insist corners were not cut here. Whats your response to that . Well, it really does seem like things are expedited rather than rushed, i think is really the right terminology. I mean, were not very far away from the same here in the United States. Were looking at getting vaccine pushed out to Health Care Workers as early as later this month, and i think it seems like the studies have been done with tremendous care and rigor, that the safety data had been incredibly reassuring. I havent seen cause for alarm, but a lot of reassurance that the benefits outweigh the harms that have been observed, not just with the latest trial but phase 1 onwards. Indeed, and the u. S. Isnt far behind the uk. Vaccinations could start right after the december 10th fda advisory meeting. The plan is to potentially vaccinate 100 million americans by march. How do you think that distribution will play out . What are the major challenges you see Going Forward with this . Well, distribution is an extremely complicated thing, and were still looking to see how this is going to play out in the United States. There are many many questions that have not been answered. How is this going to be coordinated across states . How are we going to manage things like even storage of the vaccine . Of course the Pfizer Vaccine requires these ultra cold refrigerators. These are not standard issue for Major Medical centers. If anyone has them, it will be larger hospitals. How are we going to get it to rural areas or sempimply places that are enough driving distance that we wont be able to get batches out to them, and then of course the huge Public Health messaging system. Theres a lot of vaccine hesitancy. We have to get people interested and willing to take the vaccine when we work out the delivery issues and not only receive one dose of the vaccine but come back and receive a second dose. There are tremendous challenges. This is an incredibly expensive process to get vaccine out and administer how to store, administer the vaccine and educate the public and make sure people have the knowledge and understanding and the confidence to receive the vaccine, so we have tremendous challenges ahead of us, really more questions than answers and a lot of work to do over the coming months to make sure this very successful vaccine in the Clinical Trial setting is successful when we get it out there. Even when a vaccine is approved and available, there are Many Americans who remain skeptical the injections are safe. In an effort to reassure the public about safety and encourage widespread vaccination, three former u. S. President s say they are willing to get the shots for everyone to see. Barack obama, bill clinton, and george w. Bush have volunteered to get vaccinated on camera as a way to build public confidence. U. S. Infectious disease expert echoed concerns about the uks vaccine approval. In an interview, fauci said the uk didnt examine the trial data on the vaccine as carefully as the food and Drug Administration is doing in its review. We really scrutinize the data very carefully to guarantee to the American Public that this is a safe and efficacious vaccine. I think if we did any less, we would add to the already existing hesitancy on the part of many people to take the vaccine because theyre concerned about safety or theyre concerned that we went too quickly. Its almost a dammed if you do, and dammed if you dont. If you go quickly, people are not going to want to get vaccinated. They did not do it carefully. Well be there very soon. While western countries prepare to vaccinate millions of people, russias Health Minister says more than 100,000 russians have already been vaccinated so far with the countrys sputnik v vaccine. Front line Health Care Workers, teachers and several top level russian officials have been among the first to take the vaccine. All this comes as russia just reported more than 28,000 new covid cases on thursday, it highest number of cases in a single day. Cnns Matthew Chance joins us now from moscow. Lets talk about the vaccine, what the latest on its safety and efficacy because there was much concern in the initial stages, wasnt there . There was some concern expressed by those outside of russia, the trial group of people who underwent the early phase trials was not significantly big enough. Wasnt as big as other pharmaceutical companies. You have to balance that with the fact that the results accrued from the early trials were Peer Reviewed in a prestigious medical journal, the lthe lancet, and the people that were part of the trial reported a strong antibody response. Theres not a lot of sign ticie data that backs up the skepticism for the russian vaccine but a general concern that the process is not as transparent as it is in other countries and with other pharmaceutical firms. Having said that, the russians are confident in their Scientific Production that they are stepping up vaccination the vaccination program. Yesterday, vladimir putin, the russian president called on the government to start large scale vaccinations across the country. He said that 2 million doses of sp sputnik v has been produced and there are millions more in the next several weeks and months. Honestly, sanjay, yes. Every decision we make is risk versus reward. And when the Company Announced early data showing 94 efficacy, yassar was been confident it was worth it. Folks not getting the vaccine and actually infecting with covid, those effects lasted longer and they can be life or death. These are early days and the two vaccine front runners in this country, pfizer and moderna, use a type of genetic sequence called mrna. Its a technology that is never before been used in humans outside of a Clinical Trial. Mrna stands for messenger rna, it carries the instruction for whatever protein you want. In this case, the spike protein, the virus uses to enter our cells. These vaccines require two doses, one to prime, one to boost. A few weeks apart so the body mounts what we hope will be a lasting immune response. One of the biggest concerns now is that the side effects that yassar is describing, fatigue, muscle pane, fever and chills will deter people from getting that second dose. Maybe 10, 15 of the subjects immunized have quite noticeable side effects that last no more than 24, 36 hours. Do you worry about the impact of the vaccine on you longterm. I give it a lot of thought, and the only thing that gave me some calm was trying to research the actual vaccine, trying to understand how mrna vaccines work. We understand this for sure. You cant get infected from this vaccine because the vaccine doesnt actually contain the virus. And even though these are genetic based vaccines, they dont alter our dna. As far as those side effects go, that may be a good sign. That means your immune response is working for you. You should feel good about that. It shouldnt be any difficulty, knowing youre in a better position to fight off this awful virus. For now, yassar is looking forward to his next appointment, on december 10th, the exact day the fda might authorize the first vaccine. I put my name down because i felt so helpless. Its public service. I have to do it because i think mask scale vaccination is really the only realistic way out of the pandemic that were in. Dr. Sanjay gupta with that report. Biontech ceo spoke, about the vaccine in the uk, and what it means for next year. Heres part of the interview. We will deliver up to 50 million doses to the different patients in 2020. So that means a significant proportion would also go to the United States. Of course it depends on when we would be able to start the rollout. So technically, we are prepared to start the rollout. It depends on the regulatory approval. One of the things we have been hearing is the logistics specifically of your vaccine, having to be stored at around minus 100 fahrenheit. There are some who have called it a logistical nightmare. How do you plan to mitigate that in the future . Yeah, so this is the early phase of pandemic supply, and of course the early phase is a logistical plan for all of us, not only the storage and transportation of the vaccine but getting people to the right place at the right time, and ensuring that they can come back for the second vaccination. We are in the moment working in analyzing other transportation temperatures including minus 20, we are evaluating what other vaccine is tabled at 228 degrees for a longer time and what we also do is we are working already on a Second Generation of formulation which might allow us even transportation at room temperature. We will see what would that Second Generation be available. First off, what is important is that in the next few months, we will get Additional Data supporting us, the tr transportation of the vaccine at minus 20. Thats the first change. The Second Generation will be available most likely in the second half of 2021. Meantime, some social media platforms have put policies in place to combat misinformation and conspiracy theories surrounding covid vaccines. They have their work cut out for them as englands medical chief explained just a short time ago. Of course at the moment, they are trying to stamp out some of this misinformation. Its a very complex process. Its a little bit like having bugs in the yard. If you push them away from one yard, they will just go to the neighbors, and of course different platforms are all neighbors online. Whats happening is that a lot of them, and some are them, you know, its communities of moms, concerned parents, theyre being pushed on to other platforms where theyre reforming, feeling that they have been chased because theyve actually got a point, and then kind of coming back in a stronger way and uniting with other Interest Groups like yoga lovers, anybody whos got this kind of more holistic view of health, and therefore thinks that maybe, a, their kid done need a vaccine anyway or, b, this is all so rushed and the science is so much still in development that they have a right to be concerned. Thats what they feel. Well, front line medical workers across the u. S. Are struggling to keep patients alive and keep themselves from burning out. I mean, youre saying this day in and day out. On a personal level, its hard. We lean on each other. The holidays have been rough for a lot of us. Were not seeing our families. Were doing everything we can to keep the public safe, and so its extremely frustrating for us when people are not doing that. Here on the front lines, this virus is incredibly real. According to sources cnn has spoken to, and diplomatic sources as well, the u. S. Is drawing down some of its staff but also worth noting this is happening over the holiday season. So basically a lot more people are being allowed to take leave, and they are expected to return back to business more or less as usual by mid january. But of course, yes, the anniversary of the killing of Qassem Soleimani is on january 3rd, and there are concerns surrounding that, concerns thatch most likely been amplified because of the killing of the Iranian Nuclear scientist that took place last week. And following that, that is really what prompted Irans Parliament to pass a bill that allows it to immediately begin enriching uranium at 20 . That is higher than the pre2015 Nuclear Agreement levels and to block inspections if sanctions are not lifted. So you also have tensions that are rising on that front, and the way it does tend to sadly play out when it comes to tensions between the u. S. And iran, well, they happen in the proxy battlefield of iraq. That of course is putting iraqi officials on edge, and the last thing that that country needs is to once again find itself stuck between its neighbor iran, and the country meant to be one of it allies, the United States rosemary.
