emphasizing they're trying to reach out to passengers before they come to the airport so they know there is a cancellation or they know there is a delay, they won't come. so they're trying to reduce the bottlenecks. all of this is due to the covid numbers, employees calling out sick and to weather problems. looking ahead there in the forecast, there is some troublesome weather across the country in the coming week, and i think the numbers of covid cases are also rising slightly in places. if you put all that together, and with new year's coming, you would think there would be more travel as the week goes on. probably we're in the middle of this. i don't think we're at the end of it, we're not at the beginning of it, obviously, so we're probably in the middle of this. the best advice is to check with your airline before you go to the airport to make sure your flight is taking off, when it's taking off and if there is a delay or cancellation. >> this study of a fourth vaccine dose in israel and where it fits in the picture of covid challenges facing that country now. >> here in israel the start of a ground-breaking study of a fourth dose of the vaccine, medical staff at an israeli medical hospital are being given that fourth dose and researchers will be looking very closely to see what effect it has in the fight against omicron and other variants. now, this comes about a week after an israeli medical panel recommended a fourth dose for people over 60, for people with compromised immune systems and for medical staff. that recommendation is still awaiting final approval from israeli health officials and the fact that this trial is going ahead suggests the israeli government may want a little bit more information before it does move ahead with that fourth dose. meanwhile, israel is looking at potentially easing those very tough travel restrictions that were brought into place when omicron first emerged. right now foreigners are banned from entering israel unless they have special permission, and israel has a very long and growing list of countries that are labeled red, which means israeli citizens cannot travel to or from there. the u.s. is on that list as is most of western europe. we've kind of reached that ironic phase in the cases in israel where it's getting high enough, there is enough omicron domestically where they feel there is not much point keeping those travel restrictions in place. i will say they seemed to serve their purpose in that they did slow down the arrival of omicron in israel. israel seems to be at least one or two weeks behind both the u.s. and western europe in terms of rising case numbers, but those case numbers are going up. one of the many israelis who is in isolation right now is israeli prime minister naftali bennett. he was actually in a cabinet meeting yesterday when he was informed that his 14-year-old daughter had tested positive, so he was rushed out of that meeting into isolation. he has since tested negative, but he's going to stay in quarantine as a precaution. he's in good health and he continues to do business from his home just outside tel aviv. lindsay? >> dr. gander, i think it's safe to say health experts around the world will be keeping their eye on that fourth dose out of israel. do you foresee a time when here in the u.s. maybe health care workers could be encouraged to get a fourth dose, or maybe just not everybody yet? >> what it is we're trying to do with vaccines, many of us are becoming increasingly concerned that the focus has been too much on preventing every single infection. it's going to be very difficult, without getting too much into the weeds of the virology and biology of that, it's going to be exceedingly difficult to prevent every single infection with our vaccines. however, we can prevent severe disease, hospitalization and death. that is, after all, why we vaccinate against covid. we do not vaccinate for the common cold. really keeping our eye on the ball here as to why we vaccinate, i'm really not convinced you're going to need to give fourth doses to the general population any time soon. >> you know, dr. gander, we are coming out of one holiday weekend. we're heading into another. people may have tested before gatherings, before seeing loved ones friday or saturday, but if anyone is sticking around through new year's, what should they do to make sure everybody is staying safe? >> it's the same advice we were giving leading up to thanksgiving, leading up to christmas, now leading up to new year's, which is, first of all, get vaccinated, get boosted if you have not already done so. and then layer other measures. so be sure to wear a mask if you're indoors with gatherings of people, open your windows and doors to optimize indoor ventilation, get yourself heppa air filtration to improve air quality even further and make use of rapid testing. what we've learned from omicron, you probably want to test as soon as possible prior to an event, not a couple days or an hour before, but as soon as possible prior to an event. if you're positive, use your common sense. do not attend the event where you may be spreading the infection to others. >> doctor, you talk about wearing masks, but there are a lot of doctors who think those cloth masks we've all been wearing are almost ineffective against omicron. how much do we know about this right now and what do you say to those people who can't get their hands on an n-95 or nk-95 right now? >> there is really two reasons we wear a mask. one is to protect ourselves, and also if we happen to be infected, maybe we don't know it yet, it protects others from our spreading the virus onto others. there is no question that cloth masks do provide ample protection in terms of our spreading the virus onto others. we measure the effectiveness of these masks in terms of how well they filter out tiny particles. those n-95 masks are called n-95s because they filter out about 95% of these tiny particles in the air. kn-95s are filtering about 50% to 80% of those tiny particles. surgical masks do a little less well at maybe 30% to 70%, and cloth masks, maybe about 30%. so they are all achieving some level of filtration, some level of protection against the virus. you know, a cloth mask is certainly better than nothing, but if you're able to get a surgical mask or a kn-95, or best yet, an n-95, that will certainly protect you better. >> all right, dr. celine gander, we appreciate all that expertise. i want to bring in chief medical officer of the system. doctor, thank you for being with us today. >> thank you for having me, lindsay. >> you said pennsylvania's health system was in the brink of collapse. what are you anticipating the next couple weeks as we get into this post-holiday potential surge? >> we have a critical situation right now in terms of capacity. our hospitals are currently at 106% of capacity. we have icu-level patients being taken care of in other places besides the icu. we have tremendously long waits in the emergency department. we're looking for that to get worse before it gets better. >> are you looking at a scenario where non-covid patients might need to rethink coming to the hospital if it's at all even possible to avoid it. we have ways to protect the patients as well as the staff. i think the people who don't have covid are sometimes overlooked here because they're what i call the forgotten victims of the pandemic. we can't get to them quickly enough. people who have heart attack, strokes, whatever, we're not able to get to them quick enough because of the overcapacity of the staff we're running. >> what do you mean when you say we're not able to get to them quickly enough? does that mean they're waiting a long time for an ambulance because there is not a bed open? what does that look like in practical terms? >> it depends on the day. let me go back. pre-pandemic, if you came to the hospital by emergency, the handoff to staff would take about 15 minutes. now we have days where it's taking upwards of an hour and a half. that not only delays the care that the person arriving in the ambulance is getting, but it also keeps that 911 unit out of service that much longer so the next person calling 911 experiences a delay of getting things started by first responders. >> what does that mean for people who need not emergency, but kind of emergency procedures. we're talking about heart attack patients, we're talking about cancer patients. what does this surge mean for people like that? >> so to be clear about this, we have stopped all non-emergency surgery as a way of providing additional capacity in the hospitals. so there are people who need heart surgery, there are people who need cancer surgery. they don't need it today but how long can we prolong it? how many tomorrows can it be postponed before it becomes a critical situation? those are things we're looking hard at every day and those are the people being impacted by this. >> that because there aren't enough beds or there's not enough staff? >> so both. we can create additional beds in places where there have not been beds before, but we can only do that when there's adequate staff, and our staff is fatigued, our staff is frustrated, and we have staff that is really at the brink. so finding the additional staff to take care of those beds can be the most feared challenge of all. >> when it comes to covid patients, what are you seeing in terms of how many are unvaccinated? >> about 90% of people in the hospitals have not been vaccinated at all. if you look at the people who have some vaccine but maybe not the booster where the most recent dose was more than six months ago, it goes up to almost 100%. it's a good 90% of the patients in the hospital have had no vaccine, and the unvaccinated covid patients are taking up about a third of our total hospital beds across our system, and they're taking up 50% of our icu beds. >> that said, what's your message to folks? what do you hope they hear coming away from this interview? >> what i hope they hear is a couple things. number one, the worst is not upon us yet. we have not seen the people who got sick at the thanksgiving table. we'll see those in the next two or three days. but new year's eve is coming, so if at all possible, exercise judicial use of the resources. wear your mask if you do go out, please consider staying home, please consider that masking and social distancing are the most effective measures we can do at all. finally i would just like people to stop and realize that they have a responsibility not only to their own family but to everyone around them, and unvaccinated people do create a risk. and we just need them to get vaccinated and to wear their masks. >> before i let you go, what do you need in the coming weeks? deliberations of the trial of elizabeth holmes. she is charged with 11 counts of wire fraud and conspiracy to commit wire fraud over the allegation that she intentionally deceived investors. the jury has been in deliberation over 14 hours after an 11-month trial. here is gadi schwartz. >> reporter: the jury has not taken any lunch yet today, but so far no questions from the jury. if they do have questions or they reach a verdict, there will be a 30-minute heads-up for everyone, including elizabeth holmes, who will regather in the court. when a judge was asked to hear a recording, everyone was brought back into the courtroom and listened to some phone calls that were secretly recorded by investors of elizabeth holmes preaching about theranos. another time the jury asked the judge if they could take the jury instructions home. the judge said, no, it has to stay in the jury room. think about that for a second. everyone thinks this is a cut and dried case, but 39 pages for jury instructions. binders and binders full of evidence, statements and seven days of testimony from elizabeth holmes herself. that is a lot to go through for this jury as they work to determine if holmes intended to defraud patients and investors in her false advance of this miriotic blood test with just a prick of the finger, or was she a dreamer, believing that the company was on the edge of a resolution. that's the core of the argument that's been made, that's been presented to the jury. now it's a wait and see situation as we wait to see what the jury is going to do. just four months combat global warming at a major u.s. summit in glasgow. >> we remain with the eyes of history upon us. >> reporter: he continued with his domestic agenda, halting the keystone pipeline and halting energy releasing in an alaska wildlife refuge. also keeping his word when appointing a diverse cabinet and repealing the ban on transgender personnel in the armed forces. >> transgender personnel, if qualified in every other way, can serve the government in the united states military. >> reporter: on capitol hill, after months of negotiations and democratic infighting, a scaled-back $1.2 trillion infrastructure bill is signed into law. >> america is moving again, and your life is going to change for the better. >> reporter: while the second half of the president's economic agenda, a massive social spending and climate plan, is now in limbo with no clear path forward along with other major priorities still stalled in congress. >> free the vote! >> reporter: like voting rights. >> people need the people's act. >> reporter: police reform. creating a pathway to citizenship -- >> the promise of protection for immigrants, nothing has changed. >> reporter: and making roe v. wade the law of the land. >> abortion still needs to be legal. >> reporter: with much still unfinished, the president will now take the country into a new year of change and challenges ahead of the critical 2022 midterm elections. monica alba, nbc news, the white house. >> let's dive a little deeper into this. i am joined by eugene daniels, white house correspondent for "politico" and msnbc contributor, and susan page, washington bureau chief for "usa today." susan, if there's been one constant during this presidency so far, it has been the pandemic. on one hand you have two-thirds of the country evacuated. on the other hand, caseloads are as high as we've ever seen. if you had to pick, would you say the administration has succeeded or failed here? >> there are things the administration can brag about when it comes to trying to deal with pandemic, certainly more of a message than the trump administration had, one that relies more on science, the success in getting vaccinations to millions of americans and then boosters, but i think you would also have to criticize the biden administration for not seeing, for not being prepared for this new variant, for not being prepared for these rapid tests people are needing these days. i think you would have to say it falls in both categories, achievement on some hand, a disappointment on others. and it is, of course, the number one issue for joe biden, the issue that is most responsible for his election. it is job 1 for his administration on the day he was inaugurated and today, lindsey. >> eugene, when we talk about the president's other goals here, we got infrastructure passed with the american rescue plan, but build back better has been paused and we know it's on the back burner for now. every democrat says we'll get it done, that it has to be pared back for now. what do you think this bill is doing? >> i think a lot of it depends on what joe manchin has said. viewers have heard this man's name more in one year than in his years in congress combined, because he controls a lot of the agenda. one senator that can kind of derail everything is bad. so one of the issues of this presidency, and has been for the last year, the administration is going to get hit from both sides, right? from the republicans, and especially a progressive wing who wants more on climate, police reform, wants more on climate change, voting rights, on protecting democracy. in this build back better agenda fight, you see the same struggle. you see progressives saying we want more child tax credit, we want the leave back in there. that is one of the things this administration has found is that a lot of the things that they're struggling with that are hitting their own numbers are out of their hands. that's a struggle when you came in with such high hopes. >> eugene, the withdrawal from afghanistan has been marked down, really, as one of the toughest parts of the biden era so far. the airlift did rescue more than 100,000 afghans with tens of thousands now resettling here in the u.s., but there were those images of those planes coming in and swooping and getting people in. a lot of people said it could have been handled differently. we're going to have much more on what's going on in afghanistan in just a little bit as well. but should the white house be amplifying what was accomplished there, or do you think they prefer to keep it out of the conversation? >> i mean, i think in afghanistan there is still a lot going on and hardly anything good, right? you're hearing about millions of children who have been going hungry because of a possible famine that's coming. when you talk to folks that live and operate in that country. so i think what they're hoping for now is they're going to be proven right at some point with afghanistan. this is an administration that was constantly -- when they make their decision, they did this during the campaign, they kind of stick with it, and the president said over and over again history is going to prove him right. we will see, right? it's one of those things in the first year of his presidency stands out as one of the biggest things that they bungle. now, they have a lot more in the messaging. i was in the audience with the other president when they talked about the pullout from afghanistan, and he said we weren't going to see those helicopters coming from saigon, but we did see people holding onto airplanes falling out of the skies. had it been clear to the american people this would be something that was going to be hard, that would be a struggle, that would be a bit bloodier than the country wanted it to be, i think his hit on those phone numbers wouldn't have happened so badly. >> susan, it has only been a year, but at the same time the next few months are so crucial in terms of turning these agenda items into wins. it's very possible the balance could shift in november. how crucial are the next few months? >> in terms of getting legislation through congress, he only has a few months, because we know the midterm elections will take over there. we don't know what will happen next november, but this is a guy that will take over the house and make it harder. we see presidents with power especially when it comes to foreign policy, but when you're looking at this domestic agenda, the legislation he promised to push through congress, he has a few months to go and then it gets much, much harder. >> eugene, do you think voters will remember the infrastructure bill, the american rescue plan, the extended child tax credit that's being set aside for now, do you think people will remember that at midterms? >> if they tell voters what he did for them, and they're starting to do that, going out and talking about the infrastructure bill especially. but if they don't attend those ribbon-cutting events, it will be hard to keep it in focus, and some things good for the administration and democrats in congress is that you have a whole eight, nine or ten months before people actually start voting, and a lot can change, right? who knows what will be the sticky point for voters, but democrats will have to do a lot better job and give them a lot of credit for things they have done for them. >> eugene daniels and susan page, we'll have to leave it there. one of the criticisms of joe biden's first year in office was the withdrawal from afghanistan, and we're seeing the aftermath play out in realtime. the taliban which now runs the country has two industries that they call unnecessary institutions in the country. they also stripped back rights of afghan women. somali is covering this story for us in london. what do the taliban say? >> reporter: the restrictions are on women, and they have now said that women traveling over 45 miles by themselves can no longer do so without a male companion. outside drivers are not allowed to pick up women who are not covered fully head to toe. the taliban also announced, as you mentioned, they dissolved the election commission, they also dissolved the administration for peace and parliamentary affairs. this is a rolling state institution, state institutions that would not only lend credibility to the taliban government, but also, lindsey, these are the institutions that the rest of the world, western governments, will be looking at when they even consider recognizing a taliban government, working with a taliban government, and big picture, this is making it harder for ngos to work with the taliban and get life-saving aid in the country and get life-saving aid to countries who are starving. lindsey? >> in other words, is this just the beginning as far as regression? >> reporter: you ask activists, you ask women's rights campaigners who have been saying this for the last year, the taliban is now showing their true colors. this is what is going to come. we have to remember that secondary schools for girls have been closed since august, that many women when taliban swept their power, they lost their jobs and still have not been able to go back to work. a spokesperson was asked, do you worry about the negative image this might send to the rest of the world? according to the associated press, they said taliban is real. the royal family marks their first christmas without prince phillip. how he was made crucial part of the (calm music) with downy infusions, let the scent set the mood. feel the difference with downy. and baby a fighting chance. christmas is over and so begins the rush in returns. the national retail federation for$6.77 billion of goods will get sent back this year, and you might be surprised to hear who bears the brunt of that cost. >> the consumer pays the price for a free return, the consumer pays the price for free shipping. there is nothing in this world, not even death or taxes, that really is free any longer. so when a consumer takes advantage of free shipping, that cost which is not free, has to be borne somewhere, and the retail setting is in the price of goods. >> it's us paying for it. another cost you may not have considered, the toll all this takes on the environment. joining me right now is nbc news correspondent shaquille brewster out of a warehouse that processes returns in greenville, indiana. there may be one or two things that i thought -- and i may or may not be right. >> i won't ask what gift you're referring to, i promise you that. that sound you just played stuck out because it is so frustrating. none of it is free. none of the free returns we hear about are not really free, especially when you hear about all the returned items. we're talking about $6.77 billion being returned, and many of the returns end up in places like this, not the original store shelf but the warehouse where you see each pallet. there are 6,000 of them in this warehouse alone. all of these are returned goods and this is aided by the company called restock, which takes the returns and helps resell them to third party sites. they're not going back on the shelves but to consumers in other ways. the chief operating officer, listen to what he says about these returns. >> we see things going back on the shelves in brand new condition. but the bulk of that, the rest of that, is resold. >> reporter: so many of those items never reaching that original shelf, and one other factor is so many more people are shopping online. with the effect of the pandemic, they're shopping online instead of going in stores. but we know that if you buy something online, you're twice as likely to return that package. you talk about retail sales and you have more in general. places like this reselling it all. >> what can we do with the impact on the environment here?
