We are finally seeing the light at end of the pandemic tunnel. Businesses are reopening and the cities are rolling back the restrictions and the inperson learning is resuming inle schools across the country, and there are several reasons to be optimistic and the nations death rate just hit a record low for the year, and the cases are at the lowest point since october 12th, and more than 100 million americans are now fully vaccinated. So it sounds like a lot of good news, but if you are one of them who has been vaccinated, you are still wondering how can you go about safely . We will talk about travel, social events and much more in a moment, but first, break down the latest headlines with dr. Peter hotez who is dean of medicine at baylor, and dr. Callie from John Hopkins School of medicine, and dr. Peterson as well. So, now, dr. Hotez, we have heard that americans dont have to wear a mask outside as long as they are not around crowds, and you can see the breakdown in the chart here, and so where should vaccinated americans be wearing the masks and why in some places it is still required . Well, fredricka, one reason that there is so much confusion, because there is two answers to each question of what america look like in july and august when the transmission is way down, and not for now while there is a significant level of transmission, so while we head into the summer and hit 60 to 70 vaccination rates the transmission will plummet, and life is going to look like it did in 2019 with the one important exception of International Travel, because we have not done a good job of vaccinating the rest of the world, and International Travel is going to be treacherous, but that is the message to be given that we are only talking about the interim guidance right now until we get deep into the summer, and then things will change for the better. So for now, you have to act as though there is a significant level of transmission, and yes no masks outdoors, and masks indoors in the workplace or interacting with large crowds and even outdoors, because that message needs to get through that there is two answers to this. I see that already. So dr. Gounder, and if the cdc is saying that you need to wear a mask outside if it is crowded, now defined what is crowded . So, i think that i would defer to the Supreme Court justice who said that you would know pornography when you see it. But generally, if you are not able to stay away from somebody else within six feet, you are in a crowded area. And now, you have worked with the president s commission on transmission, and you saw that the president and the Vice President and House Speaker wore a mask, and so is this a missed opportunity or sending mixed signals and messages to the nation . Well, i think that there is a mental checklist that i use when think about these situation, so if you are vaccinate and everyone around you have vaccinated, you can go back to prepandemic life, but the problem is that we dont know who around us has been vaccinate and not, and while the president has been vaccinated, he had no way of knowing for sure that everyone in that room had been vaccinated. And so s when you are around other people who might not be vaccinated, we really revert to what is called the 2 of 3 rules. The three things are outdoors, wearing a mask and remaining six feet apart. You want two of three of those. And there is some additional factors that help sort of sort out how risky the situation is. One of which is geography, and are you in a place, or a county for example that has high levels of local transmission. I believe there is a map on the screen now that shows that where we are right now, and so the red and the Orange County ares are risk of getting covid and the counties in blue are lowrisk. So if you are higher risk as the president was in washington, d. C. , i would be more conservative. And so finally, i would look at in your vaccination status, do you have symptoms . Do the people around you have symptoms, and have you or they been exposed to somebody that you know has covid and that is also going to up the risk level in that situation. I feel like for so many, and me included, i feel like the mask is a security blanket, and i am wearing it all of the time except for inside of the house, and maybe it is hard to part with it when the time is right. So dr. Cyrus, a new cnn poll finds that 45 of the people have not been vaccinated, and so how should you talk to someone who is hesitant about getting vaccinated, and sometimes that conversation happens right within the family, right . And families are strained and friendships are being strained now, and how do you that conversation . Yeah, i think that you are highlighting how divisive this issue has come, and one about the Public Health and do you want to stay alive and not get sick. So if you are going to talk to somebody, try to not go in it as if you are going to convince them. If you need to take a walk or a few deep breaths and take a second, and try to be calm so you are not defensive and not reactive and the goal is to just understand. That you cant force people to change their minds, but what you can do is to understand why, what makes them hesitant and what would make them want to get the vaccine, and then conversations to listening and empathizing, you might slowly be able to change their minds and at least in our field, change is a dynamic process, and sometimes the people are sometimes into the idea, and sometimes they are not into the idea, and meet them where they are at, and continue to have the conversation, and folks are just, you know, it is really tense, and you have to change tyour mind and get it doe right now, and maybe that is not the right way. And it is prickly if it means getting in each others space, and they have the heels dug, and they wont get together or it is going to be unpleasant when they are. Which is unfortunate, because a lot of relationships are suffering. Yes, it is tough. Well, this is yet another sign that perhaps the country is returning to some semblance of normalcy, but the pandemic remains dangerous. Dr. Hotez and dr. Celine gounder are back to offer their advice on vaccinations. And talk about the social events like the derby and tell me whether you think that people should be attending or avoiding, dr. Hotez and if i have been vaccinated, is that safe to attend the arena that has 100 capacity . Yeah, i probably would not do it at this point, and they would not have a problem doing it this summer if you are feeling compelled to go to a Sporting Event even if you are vaccinated given that there are a lot of people surrounding you infected with covid. You want to wear a mask. By the summer as the transmission is way down, then you will be in a much better position to go to that event. And dr. Gounder, if i have been vaccinated safe to go to a wedding with the Indoor Service and outdoor reception . I would go to local Community Transmission rates, and looking at the map in the country and high levels of transmission and what you see and read on this map, and as more people are vaccinated, we are seeing more and more of the country is blue, and the risk is minimal, and you can attend the wedding more safely, and until then attend to the 2 of 3 rule, and outdoors, masks and distance, and so when you are outdoors at the very least, you need to be wearing a mask and for the indoor wedding or ceremony, doubling up masks and social distancing. Hmm. Dr. Hotez, so if i have been vaccinated, is that safe to fly to my grandsons graduation in another state . Well, yes, it probably is, and you want to wear a mask on the plane. And in certain Family Events are important, and itll be safer as we head towards the summer. But, the flying is relatively safe, but, if you are vaccinated, but there is quite a bit of transmission especially at airports, so you want to be careful to be masked the entire time at the airport and masked the entire time at the plane. Again, these are the questions at home, and dr. Gounder, if i have been vaccinate, is it safe to hug an unvaccinated Family Member . Again, go back to look at the map, and are you in a place or the Family Member is in a place with highlow risk with the transmission, and hope that the transmission drops across the country with the more and more vaccination that the map turns blue, and in the meantime, stick to the 2 of 3 rules, and if you are hugging somebody outdoors and you are both wearing a mask, that is fine. But i think that you need to be following the guidelines for now. All right. And dr. Hoe tez, people are tryg to make plans and fourth of julie is one of them, and if i have been vaccinated, is attending a Fourth Of July barbeque okay . Yes, i believe it is, because once we hit the vaccination rates by then, it should be okay by then. I would give it a vote yes. If there is still a lot of transmission and people, then you want to wear a mask, but i am hoping that we might not have to do that folks are going to be excited to make these plans. What about whether it is safe to go on the cruise if i have been vaccinated . Right, again, the same issue, you cant be sure that everybody on that cruise has been vaccinated. It depends where the people are coming from. Usually the cruise has people from all over the world, and you have to believe that is a highrisk situation, and two of th three, masks, six feet apart and vaccination. And what about at the gym a workout . Well, i would say it is doable, and as the transmission is down in the summer, we wont have to have masks in the gym. And dr. Gounder, some high skeerls who have been vaccinated and what if my child has not been vaccinated, and is it safe for them to go to prom . That is the situation of following the two of othree rules. If they are going to the prom, assume an indoor component. So they really need to be wearing a mask, and i would be concerned, because it is hard to maintain six feet apart with that prom date. So, you know, i would prefer to see the 16, 17, 18yearolds vaccinated before celebrating that prom. All right. Dr. Hotez, last one before this commercial break, and what about if i am vaccinated, but my kids are not. Should there be places that i dont take them, dont bring them . Well, just remember right now, when there is a lot of transmission, any place you bring them indoors, there is a risk of transmission. So, i would try to limit it at this point. So, you know, taking them to mcdonalds and places like that are still better to do that and have the Outdoor Dining at this point, because there is still quite a lot of transmission. Then the goal is to be a little bit more, you know, ill say firm, and say, could we please have some space, but if you have time to ease into it, and you notice coworker over the past few days has been lax, and i would approach it again, and use the eye statements, and like, i feel uncomfortable and a little bit up safe when you are over there with your mask off. So that is one way to do it and a followup that says, maybe i am going to be sitting over here a little bit further away from you, and separate this tension that you might be feeling about people, because they have different values and not wearing the mask and try to come at them as really is the goal is to get them to mask up, and focus on you, and explain how you feel when they dont wear the mask, and that way, you are not attacking them personally, and using motions that you are going through, and that might appeal to them as a coworker, and that depends on the relationship that you have with them and the goal in the immediate and the long or the short term. That is polite and lovely and i am vedica sud. Talking with your eyes . Thats what were all doing nowadays. Our eyes can say a whole lot. I get it. Next weekend families will gather for mothers day. How do you address a family who has not been vaccinated and wants to be in the company of Everybody Else and others who have been vaccinated . This is a really good question coming up with some of my patients and friends. As you mentioned we all have different boundaries when it comes to our sense of safety. You can get sick and die from covid. If someone not vaccinated is trying to come over to the house to celebrate mothers day, thats a pretty clear statement to say, we dont want to get anybody sick. If this is a Family Member of mine, we are finding folks are having to put down boundaries. Its tough. Maybe you dont see a lot of nos in your family and conflict is not your thing. Its something folks are finding more often to set that boundary and have that conversation. I would go back to using i statements. I would feel horrible if you came and got sick from one of us if you were not vaccinated. I would feel horrible if you came and our mother got sick. Were thinking about the safety of everyone, which means that maybe you dont invite them. If they say they will show up any way, how do you let them in the house. I have a friend who has a partner who says, you cant come with me or you cant stay in the room. If i love you and want to do this for my safety, they should understand that. People will have to feel uncomfortable for a while, it seems. There is still a lot to consider. Now that the pandemic is improving a bit, many people are adjusting to old routines, how should people address anxiety related to their lifestyle once again changing . I would say normalize anxiety. If you dont have anxiety getting back to normal life, thats probably abnormal. Of course you will have anxiety. We seem like were get back to normal but who really knows how this will all play out. Be uncomfortable experiencing a range of emotions and will come down to whats best for you. If you know you go to that baby shower you will have a panic attack, maybe its best for you not to go to that baby shower even if you use 2 out of 3 roles, and your Mental Health it comes down to. And how much freedom they can achieve being vaccinated and going back to normal. I have a friend named david just finishing up vaccinations and going to a restaurant for the first time tomorrow. Im pretty sure he will enjoy that drink hes going to have but probably nervous about the chef or waiters, are they vaccinated that didnt come up being at home getting takeout. Stick their toe in the pool and see how it feels. If its a little too much for you, that means the next time you try this maybe you stay masked the entire time. There will take Trial And Error and moving at your own pace and that depends how risk averse you may or may not be and is an individual process for folks and gets more complicated if you have Family Members. At the end of the day you have to prioritize your health and how comfortable you feel. That makes sense, so much anxiety, in the midst of it with changes in the pandemic and trying to exit it, too. For your kids real quick, they are feeling pintsized anxieties as well, maybe worried about is it safe for me to go back to school even though theyre telling me its okay without a mask. How do you help them . Talking about children. Normalize it. Feelings are normal. One thing that happened through this pandemic we all know were going through this, statistics showing compared to 2019 the rate of anxiety and substance abuse, even you especially have gone up 4 out of 10, not feeling comfortable going through something right now mostly going through the pandemic. Lets talk about what these feelings mean. If your child is saying Something Like this to you, you say, i feel anxious, too, what might make you feel more comfortable. Once you get an answer to that, what kind of risk would you take, going to school one day a week, come up with a compromise and be okay with whatever answer is given to you. I love it.
