View Comments On Tuesday around 4:30 p.m., a single-vehicle fatal crash occurred between Scott and Lafayette. Juan Carlos Hernandez of San Antonio, Texas, was unbuckled in the left rear seat when the truck he was riding in had a tire failure according to Louisiana State Police press release.  The driver lost control and the truck overturned in the median with the 29-year-old becoming ejected. He was pronounced dead at the scene by the Lafayette Parish Coroner s Office.  Two other rear seat passengers were unrestrained and transported to a hospital with moderate injuries.
