This Order is made in relation to a complaint that Mrs Westerman in her role as supervisor of two Individual Voluntary Arrangements (IVAs) breached the Fundamental Principle of Professional Competence and Due Care when she failed to ensure appropriate progression of both IVAs as required by paragraph 16 of SIP 3.1.
