The lack of followup coordination and quality and continuity of care, many of these is troubling and unacceptable. The most troubling issue described in the report was the awareness of the ongoing scheduling and challenges that many facilities face. Furthermore they had opportunities to address the systemic culture of inappropriate scheduling practices. They did not act to address the inappropriate practices of manipulation of wait time data. This lack of accountability was engrajed by the decision to waive the fiscal year 2013 annual requirement for the directors to certify compliance with the scheduling directors.
