Queenslandâs deputy coroner has found the former Child Safety Department ignored key medical information in the lead-up to the separate deaths of two babies in 2015 and 2016 which could have saved their lives.
One, an eight-week-old Logan boy, was found with his mother slumped over him in her lap in August 2015 after an âincorrectâ assessment he did not need protection under the department.
The Child Death Case Review Panel would later state it was alarmed by the âpoor and completely inadequateâ response by the department in one of the cases.
