A six year old boy, jack adcock, died as a result of a catalogue of errors at a leicester hospital. A serious infection was not diagnosed. A doctor, Hadiza Bawa Garba, was convicted of gross negligence manslaughter and later barred from practising. A nurse at the hospital was also convicted and struck off. Colleagues say dr bawa garba was working under intense pressure and low staffing was partly to blame.
