Physicians Health Plan of Northern Indiana and Premise Health will build a primary care center in Fort Wayne, Indiana, that will be exclusive to the health plan's fully-insured members in the area. Members will gain access to a care team that comprises physicians, nurse practitioners, a behavioral health specialist and medical assistants.
