Suicide warning signs missed at Washington prisons, investigation finds
For the second time in less than a year, Washingtonâs Corrections Ombuds (OCO) is warning that the stateâs prison system needs to do more to prevent inmate suicides.
In a 15-page investigation scheduled for release Monday, the OCO found that two inmates died by suicide in 2020 after prison staff failed to recognize signs of mental distress and didnât follow suicide prevention policies.
âSuicides are preventable. There should be zero, that should be the goalâ said Assistant Ombuds Dr. Patricia David, the investigationâs author and the OCOâs Director of Patient Safety and Performance Review.

This report is the continuation of a 2019 investigation, which showed the Washington State Department of Corrections (DOC) had a 250% increase in suicides from 2018 to 2019. In 2019, five of the 37 deaths were suicide, whereas suicide was involved in only two of the 37 deaths in 2018.
Although the number of suicide deaths in 2020 returned to the DOC’s average baseline of two suicide deaths per year, the OCO “proceeded with an investigation into both cases to determine whether there were findings similar to those put forth in the prior report which potentially contributed to these two deaths.”

Roberta “Jean” McGeary, 89, of Globe Run Road, Petersburg, passed away Friday at her home surrounded by family. She was born in West Township, Huntingdon County, a daughter of the late Edward and Anna (Ewing) Sager.
On Dec. 22, 1954, she was united in marriage to Harvey McGeary; he preceded her in death on May 6, 1978.
She is survived by children: Louise Houck and her husband, Jerry, of Petersburg, Minnie “Suz” Showalter and her husband, Ken, of Williamsburg, Patricia David of Huntingdon, Kathy Collins of Alexandria, Edward McGeary and his wife, Dixie, of Petersburg and David McGeary of Petersburg; 12 grandchildren; 18 great-grandchildren; and nine great- great-grandchildren.
