So as you can see, the percentage of the old old as you in the orange and yellow boxes in this chart grows very quickly. Again, if you think back to the chart in terms of the amount of per capita spending that increases as people age you see we are looking at a growing number of individuals who, in fact, will be the most expensive portion of the Medicare Program. At the same time we have fewer young people coming in and we have fewer people that are paying payroll taxes to essentially support the program. And, of course, among those who are the most costly and juliet touched on this issue, are those who are dual ebl jiblligible. But Rising Health care costs is also an issue for the beneficiary. In Rural Communities, and we think the need is largely met in that program, not only through federal funding, but through state and private sector funding, but we do allow people to come in through our outreach funding to get at the same issue. They could come in for outreach funding or Network Funding for a request in the budget and do the same thing in the sense they can develop a program that seeks to purchase supplies and put them in the communities. The remaining need out there, it can be met through the outreach program. And the hospital Improvement Program youd continue is a 25 Million Program . Yes, sir. That in the current Year Spending 25 and preparing another 25 next year . Correct . The 20 this year for similar purposes goes away in the president s budget . Yes. These are the goals that the secretary announced. Really focus on the dark circle on the page. The dark circle is the percentage of medicare fee for Service Payments in the alternative payment models. In 2011, they were zero. No medicare payments in the model. In 2014, at the end of 2014, about 20 of the 362 billion of the medicare feeforService Payments, excluding Medicare Advantage. But the numbers are similar. 20 were alternative payment models. The goal is by the end of 2016 30 of the feeforService Payments will be in the new alternative payment models that work and 50 by the end of 2018.
