If you look over his right shoulder, there is a periscope there, and its a prismatic periscope that sticks up through that space. In the periscope is a lens and he uses that to fire the machine gun. His periscope comes up from the inside, and that is how he is looking. One of the things you will notice, their field of vision is fairly limited.
