Sarah Rolenson, BSN, RN, Population Health Nurse for Mon Health Stonewall Jackson Memorial Hospital, was recently chosen as the ICARE December Employee of the Month.
Her nominator wrote, “Sarah has shown great commitment to SJMH but especially to her patients during the pandemic. Sarah took it upon herself to personally call every person she has had an interaction with to check on them, make sure they were okay, and also provide education as needed. She goes out of her way to meet the patients where they are and sees her job as more than 8 to 4. She completely embraced Telehealth to continue to meet the needs of her chronic-care patients. She has received many accolades from providers for the patients she has helped. She is known as a sincere patient advocate and plays a crucial role in care coordination while meeting patients where they are on their health journey.”
