The Hills of Headwaters Collaborative Ontario Health Team has appointed Alexander Lim as its transformation lead.
In December 2019, the collaborative became one of the first 24 Ontario Health Teams.
Lim holds two masters degrees, one in public health and another in health law, and has 10 years of health sector experience at the regional, provincial and national levels.
As Transformation Lead, Lim will lead the creation and implementation of an organizational framework which supports improved models of care that are patient-centered.

The Dufferin-Caledon Ontario Health Team has introduced a Transformation Lead to the Hills of Headwaters Collaborative Ontario Health Team.
Established in 2019, the Hills of Headwaters Collaborative was one of the first of its fellow 24 Ontario Health Teams across Ontario dedicated to enhancing the delivery of health care through new models. Ontario currently has 42 health teams covering 86 per cent of the population.
A Transformation Lead creates and implements “organizational framework” in order to improve the treatment and care for patients and providers with models that are said to be more patient-centered and efficient.

Exactly one year ago, the Hills of Headwaters Collaborative was announced one of Ontario’s first 24 health teams.
The team is made up of health care organizations and providers who dedicate their work to improving the overall well-being of residents in Dufferin-Caledon.
Together as a unit, they provide the community with “family and specialist physicians, housing and long-term care facilities, mental health and addictions providers, home and community care providers, the hospital as well as long list of other health and social service providers.”
Ontario Health Teams were introduced as a new model of care that brings healthcare providers to work as a team.
