The Transport Accident Investigation Commission s released its report on a water taxi capsize in Stewart Island in 2019 leaving several passengers with hypothermia.
TAIC s Harald Hendel says conditions were worse than the skipper expected and steep, unpredictable waves flipped the boat.
He says the operator was likely under self-imposed pressure to operate in rough conditions.
Hendel recommended that Maritime Rules be updated to ensure all vessels are assessed for stability and buoyancy.
