Officials are investigating the weekend death of an inmate at the State Correctional Institution at Greene.
William Kitchen, 55, was found unresponsive in his cell around 6:30 a.m. Saturday, according to a release issued by prison Superintendent Michael Zaken. The release indicated employees at the prison were conducting security rounds on a housing unit when they discovered Kitchen.
Staff and medical personnel responded to the unit, and Kitchen was pronounced dead by the emergency medical services personnel.
In accordance with state policy, state police were notified and will conduct an investigation of Kitchen s death. The official cause of death will be determined by the Greene County Coronerâs Office, Zaken noted.
Woman killed in Kauffman crash, 4 others injured

FAIRBORN A woman was killed and four others were injured in a head-on collision on Kauffman Avenue Tuesday night.
According to Fairborn police, an officer was patrolling the area in the 1100 block of Kauffman Avenue around 9:30 p.m. and heard the crash happen. The officer immediately rushed to the scene, and requested support from other units, including the fire department and EMTs.
The crash, which involved two vehicles, caused massive, disabling damage to the front ends of both vehicles. One car was carrying a male driver and a female passenger. The other was carrying three people: a female driver, and male and female passengers.

One person was killed and two others were injured in a crash Thursday night around 7:20 p.m. on State Road 59 and County Road 625 N.
Steelie Barbee, 52, of Jasonville, and two adult passengers, were traveling southbound in a 1996 Ford on State Road 59 when the vehicle traveled off the road. An investigation by Troopers Caleb Garvin and Ben Burris with the Indiana State Police did not immediately find a cause. The vehicle proceeded to travel 50 yards down an embankment before coming to an abrupt stop.
First responders who arrived on the scene rendered first-aid to Barbee and the passengers. The Greene County Coroner later pronounced Barbee dead at the scene while the two passengers were treated and released.

CARROLLTON A Carrollton man has been sentenced after pleading guilty last year to burying an infant in a shallow grave in rural Carrollton.
Ryan L. Wheeler, 36, was sentenced last week to five years in the Illinois Department of Corrections and one year of mandatory supervised release in connection with the death of 6-month-old Jabari Ellis in late 2018.
