The man, Craig Richards, received both doses of the Pfizer and Moderna vaccine at a West Lebanon, New Hampshire, vaccination site, WMUR, an ABC affiliate out of Manchester, New Hampshire, reported. Richards, who received his first COVID shot in March – the Moderna vaccine –  got his second dose - the Pfizer vaccine -  on Tuesday morning, when the error occurred. He said, You ready for a poke? I said, Sure, and he poked me, Richards told WMUR. As soon as he poked me, he looked down at my card, and I think he realized he just gave me the Pfizer.”

Kroger told WWBT, an NBC affiliate out of Richmond, that a small group of individuals visited the Little Clinic in Mechanicsville to get vaccinated with the COVID vaccine and instead received an “empty syringe,” which was previously thought to have been a saline shot. In a statement to the news outlet, Kroger said it was an “honest mistake.” The company continued by saying, “All impacted customers were contacted and have now received the COVID-19 vaccine. We apologize for this oversight and the inconvenience caused for these customers.”

Victor Smith went into shock after receiving two doses of the Moderna COVID vaccine back-to-back within a four-hour period instead of weeks apart in a case of mistaken identity, the Cincinnati Enquirer reported. Smith, who is currently recovering from the accidental double-shot dose, received his first shot of the COVID vaccine at the Fort Hamilton Hospital on Jan. 22 and was scheduled to get his second shot of the Moderna vaccine on Feb. 25.
