Offering expert at-home nurse and community health worker support, this initiative aims to smooth the transition for parents from hospital to home, offering crucial guidance and care in the crucial days and weeks post-birth. With no eligibility criteria, it’s funded through various federal and state resources. Services provided include health screenings, education, and connections to essential resources.
