Adrenal insufficiency (AI) is a life-threatening clinical condition caused by either primary adrenocortical failure (PAI) or secondary adrenal disease (SAI) due to impairment of the hypothalamic-pituitary-adrenal axis (HPA). Mortality among patients with AI has been reported to be more than double that of age- and sex-adjusted control populations, but there are not sufficient data for exact mortality rates.
The most common cause of death in AI is adrenal crisis, especially in patients younger than age 40. In patients with PAI, mortality is associated with an increased rate of infections especially respiratory. Furthermore, supraphysiological doses of glucocorticoid and/or fludrocortisone replacement therapy are associated with increased cardiovascular mortality and bone fragility.
