On Monday, the department released the survey on its website and its social media pages. It initially asked respondents if they were over 75. It now asks if they are over 65. The survey states that when the vaccine is available and appropriate based on IDPH guidelines, survey participants will be contacted to schedule a vaccination appointment. The survey is for Madison County residents only.
