Firefighter one went to the lieutenants bunk room and inform the lieutenant the man across the street was down and asked if the lieutenant would put them on the run. Firefighter one told the lieutenant he thought the address was the 1300 block of rhode island avenue northeast. Alvarado and others familiar with protocol say this is standard protocol as firefighters do not selfdispatch. The kid let the officer know. At this point you are a firefighter. You cannot jump into a threequarter milliondollar firetruck and take off without an officer. That officer, identified as kellene davis, told firefighter one to get an exact address. The report says that murphy did not return. It states that he returned to the kitchen and informed firefighter two and three there was a man down across the street, so i let the lieutenant know we should be going on the run. Firefighter one gathers his personal items and study books from his car and went to the bunk room. When the lieutenant went to look for firefighter one, he found him in the bunk room lying in bed studying. When asked why he did not return with the address, firefighter one indicated dispatch had an ambulance en route. But the ambulance was sent to the wrong address. The report says 22 minutes passed before mills was transported to the hospital, where he died. Its about as tragic as it gets. Alvarado says a culture of fear permeates the agency following a year of staff, equipment, and Response Time scandals. Fire chief Kenneth Ellerbe has been demoted twice for insubordination, and believes Agency Leaders have created an atmosphere of inaction in which a perfect storm of dysfunction formed over engine 26. The report author, the deputy mayor of public safety, insists in this case the fire chief Kenneth Ellerbe is certainly not to blame.
