Stayed in the Adipose town of Marsh Harbor thinking he could ride out the storm with his 2 dogs but hurricane Dorian destroyed his home while he was still inside of it and I want to warn listeners that this is a wrenching story Alex thank you for talking with All Things Considered You're welcome 1st of all Are you Ok Were you injured or hurt during the storm. It's a miracle I'm alive still. So I understand when the storm came you went to a central room of your house that you thought would be safe can you tell me what happened then yeah so few hours before that the I hit I could hear my house tearing apart around me I happen to get cell signal so I called my sister and screaming begging her to tell me where the eye is because I knew my house wasn't going to hold up yeah I could hear it foaling down the portico over my head. The water was rushing under the door it was seawater at. The door broke apart and I could see the you could see daylight and I just so confused I didn't understand why then I realized the house was torn apart and then. Properly saw the ocean coming through I couldn't believe it you sent us a video of the dogs on the console table and you were trying to speak calmly to them and this huge wave of water just comes pounding again yes so I shut the door and I go outside the waves are like 78 feet above me crashing over my head how did you survive that so. Few days before. Every problem is to get a life and I said Yeah but I did it and I was removing a doctor ladder just it wouldn't get damaged in the storm and and that was removing . Life that was floated up to me a life vest just came to you that's incredible. So I thought What the hell let me just grab it it was an extra one and I threw it in the garage. And when the garage was gone the only thing that washed up to me when I was trying to leave the house was that life vest put it on you Alex I'm so sorry for what you've been through I just hear our own this was. So I managed to. Climb on the side of the rock wall my house. Right across the foundation of what used to be my neighbor's house and then to the next house I knew they were the only people on my street and they'd open the door about 30 seconds before it was the I the storm screaming begging my friend Chris Appleby please help me. Be without me but I couldn't because there was no cell signal and then we got into this is going through bunker and then the 2nd half of the storm to about 5 minutes later and how long did you have to stay there before your evacuated to have to face. I know that you are still wrestling with your own experience but as this storm hits the Carolinas and millions of people are under evacuation orders there what message do you have for them there there is no words to describe the power of the ocean and I usually blow this stuff off and don't be ridiculous it's going to be fine. Hurricane Dorian is battering South Carolina and it is expected to continue traveling northwest up the coast Alabama is not in the path of the storm and yet this week President Trump has tweeted more about Alabama than about South Carolina he has said repeatedly that Alabama was projected to be in Dorian's path he went so far as to hold up a map that had been modified with a marker to show the storm headed to Alabama and. Not only Florida Georgia aerator was going towards the goal that was. Going to originally projected So why is the president still talking about a state where the storm is not headed even after being corrected by the National Weather Service small Let's ask n.p.r. White House correspondent Tamara Keith Hey there hi hi Why is the president focused on Alabama so this all started with an error President Trump was at fema getting a briefing on the storm on Sunday and he said something that may have been true a few days earlier but it simply wasn't true at the time that he said it and it may be you know a little piece of. A great plays a role of a hell of an ability to be indoors. At least in very strong warnings and that it could be this is a game of unfortunately. The storm that we're looking for is easy to false and now the National Weather Service Birmingham quickly tweeted that Alabama would not see any impacts from Dorian and then it sort of snowballed in the way that things snowball in this administration so there was news coverage of the president's odd mention of Alabama that the president started tweeting in response to the news coverage that all led to that weird incident in the Oval Office with the president whole holding up a modified map which of course led to more coverage which led to the president tweeting about it today he tweeted what I said was accurate. All fake news in order to demean So although there are parts of the us that right now are being affected by this hurricane in a very real way the president has turned this into a conversation about Alabama and about the media yeah and the modified map you could it was the circle where the storm was headed was was just extended to include a little softies chunk of Alabama So what what how is the president explaining this what is the White House saying well he is insisting that Alabama was part of the original forecast and I went up to Stephanie Gresham's office today the press secretary she showed me yet another map that showed some winds possibly affecting Alabama but even if Alabama was predicted to get some strong winds or effect early on by the time the president was talking about it he was talking about days old information. The cust question I suppose is does this matter I mean this would not be the 1st time that the president has bent the facts to suit his purposes right there was the crowd size it is inauguration his claims of voter fraud there are a number of cases like this where President Trump says something that's not true or not accurate and then just digs in the question is you know what does it matter do we need a shared set of facts in this country and what happens if it is about something that's more matters more than a silly map right where people are trying to figure out with the should evacuate that type of thing thank you very much Tom thank you White House correspondent Tamara Keith and now to a photo that President Trump tweeted last week it came from a u.s. Spy satellite and until the moment it was posted it was highly classified N.P.R.'s Geoff Brumfiel reports on why the president's tweet was unprecedented. The photo is of a launch pad at Iran Space Center it shows the remains of a rocket The blew up on the pad last Thursday and it is so sharp you can read the lettering on the pad I called up Bruce Clinger a former CIA officer now at the Heritage Foundation the photo may be on Twitter but he doesn't want to talk about it even though I'm no longer in the intelligence community I'm still bound by the pledges that I took when I joined the intelligence community so just talking about the president's tweet makes you kind of nervous a bit. If Clinton had tweeted this he'd be in a lot of trouble certainly anyone else who revealed it would be you know sitting in Leavenworth prison serving out of prison term the president of the United States is actually the only person who can't end up in jail because he has absolute power over classification Steven Aftergood studies government secrecy at the Federation of American Scientists the classification system for national security information is not based on a law it derives from the president's own status as commander in chief of the armed forces the rules are laid out in very detailed presidential orders the entire system is run by the executive branch and President Trump is the boss he therefore has the authority to decide unilaterally what will be disclosed what will be declassified and what will not pass presidents have used his power sparingly President Clinton authorized the release of some satellite images during the Balkans war in the 1990 s. News I'm Mary Louise Kelly and I'm Ari Shapiro a new report from a government watchdog offers some sobering news for student borrowers the government accountability office reviewed a program that Congress created last year to forgive the student loans of public servants but the g.a.o. Found that so far 99 percent of requests for loan forgiveness have been denied N.P.R.'s Corey Turner is here in the studio Hi Corie they are 1st tell us about this program and why so many people were rejected Yeah I think we need a quick history lesson 1st Ari So Congress created this program called public service loan forgiveness back in 2007 it was a promise to student borrowers basically said if you work in government or nonprofit as a teacher or 1st responder social worker you make steady payments on your student loans then after 10 years the Education Department would forgive whatever's left here's the problem the 1st several years this program was in effect the ed apartment and loan companies they really did a terrible job of managing it and explaining it to people so borrowers were often given no advice or sometimes they were given wrong advice to the point that in recent years now we're seeing thousands of borrowers come out of the woodwork and say Wait I thought I was in track and instead they realize they don't even qualify for a loan forgiveness that was a 2007 program but now we're talking about a program from last year how does that relate exactly so what we're talking about now is an effort by Congress last year when they heard all of these frustrated borrowers saying wait you've got to help us Congress decided to create a kind of fix a limited expansion that relaxed some of these really rigid rules they set aside $700000000.00 to help pay for it and it's this expansion Ari that the g.a.o. Just found in its 1st year is denying 99 percent of requests if the whole point of the expansion was to get more of this money to more people why has it been such a failure has the question. And luckily the g.a.o. Report offers a pretty clear. Trail of breadcrumbs the simple answer here is that the vast majority of the requests that are being denied 71 percent they're being denied based on a technicality people aren't filling out the right form if 71 percent of people aren't filling out the right form my guess is the problem is not with the 71 percent of the people who well yeah I talked to a lot of borrowers who are brass tax people they have budgets for their families and they could not figure this out they were baffled one of the people I spoke with Jonathan Barnes of Chicago he told me he believed he had done everything right when he applied for this expansion but then he got a rejection letter that said this in all capital letters you know I wouldn't be considered for the permit this time because it said I hadn't applied for public student loan forgiveness it was all very confusing but at the end of the day it was another big giant stop sign that I felt was underneath a bureaucracy that was unwilling to help Ari obviously the bad news here is he's not the only one who feels this way but the good news is that some folks including Jonathan potentially may ultimately qualify for this through what more Congress will action are just filling out the right form or what well some of them by just filling out the right form frankly some are going to require more congressional action g.a.o. Made a couple of recommendations to add number one they said streamline this process obviously it's not working you need to make it simpler They also said the department has got to improve its messaging explain a borrower's how it works how you qualify and what you need to know before you go through it and honestly the department told me told the g.a.o. Look we agree with these recommendations and we're working hard a number of these efforts are already underway as N.P.R.'s Corey Turner thank you thank you with the new n.f.l. Season starting tonight concerns about head injuries and footballers expected to ramp up again and now the discussion is expanding to women's soccer after the women's World Cup researchers are preparing to study how a lifetime. Time of head impacts could affect women including having the ball N.P.R.'s Tom Goldman reports using the head to redirect a soccer ball or to score a goal that's an integral part of the game especially as players become more skilled but in an era of increased concussion awareness heading is fraught with potential risk and the science exploring that risk hasn't been inclusive we really needed to expand this research to include women Dr Robert Stern studies chronic traumatic encephalopathy that's the degenerative brain disease known as c.t.e. He and others at Boston University have focused a lot of their attention on c.t.e. As linked to head trauma in men who play tackle football but next month he'll start working with former female soccer players some well known on a study called Shine it stands for soccer head impacts and neurological effects I. Mean I'm like right there are. Only as it goes on time I want but I think former national team star Michelle Akers now 53 was the catalyst for Stern study she's concerned about her peers and wonders whether certain mental lapses are early signs of soccer induced brain problems including c.t.e. Acres and former u.s. Teammate Brandy chest pain spoke on c.b.s. About their involvement in the study Akers said she now regrets what she estimates to be at least 50 headers per game during her career I would not be having a 1000000 balls like that there is no way on earth I would do that again Robert Stern says prior research shows a relationship between the amount of heading and cognitive and even chemical changes in the brain enough so that in 2015 the u.s. Soccer Federation banned heading for kids 10 and younger So how do we had the ball we look with what. Ok youth coach Dan Pingree has led his Seattle United club team through its 1st year of heading by the time they start playing games this fall most of the girls will be 11 meaning no more band at a practice on the University of Washington campus Pingree runs a refresher drill upper body straight. Ahead. I said easy right to their forehead good don't bend over Ella bend over Ella what Pingree doesn't want to see is heads wobbling neurologist say girls can be more prone to concussion because they sometimes have weaker neck muscles that cause the head to flop and the brain to shake Pingree also trains his kids to keep their elbows out creating a protective buffer to help prevent smacking skulls when someone else is flying in the head the same ball. Shahram salaries daughter Hannah plays on the Seattle United team he says the new study isn't creating extra concern among parents he hasn't sensed the kind of concussion hysteria that scripts some parents of young boys playing football I will say that I know parents who have kids that are older teenagers and what I hear from them is more of a concern about a.c.l. Injuries and injuries more than head injuries we just don't see a whole lot of that at that maybe maybe it's just the age of the kids in 2017 research by the American Academy of Orthopedic Surgeons found a high school soccer playing girls did have a significantly higher rate of concussions even more than boys who play football the study points to Henares as part of the problem that's where Dr Stern now is turning his attention with an open mind I'm one of the people who does the bulk of this research and I'm not really sure exactly what leads to c.t. And how it's manifested and what the risk factors are but stern stresses repetitive head impacts of any kind even ones that don't cause concussions are not good and he hopes the 20 women signed up for his soccer study will help science inch closer to understanding the risks of playing the games we love Tom Goldman n.p.r. News this is All Things Considered I'm Ari Shapiro and I'm Mary Louise Kelly when doctors and nurses become addicted to opioids and they get caught they have to follow strict treatment guidelines to get their licenses back often the means they're not allowed to use the so-called gold standard of treatment medication such as methadone Suboxone on n.p.r. Selena Simmons Duffin has more here's how this played out for Dr Peter Grinspoon he got addicted to vie couldn't it med school and still had an opiate addiction 5 years into practice as a primary care physician then back in February 2005 he got in trouble in my addicted mind frame I was writing prescriptions for a nanny who had since returned back to another country and it didn't take the pharmacist gone to figure out that I was not a 1000 year old nanny from New Zealand one day during lunch he says the state police and the d.a. Showed up at his medical office I started going home glad you're here you know how can I help you in there like doc cut the crap we know you're reading that scripts he was fingerprinted the next day charged with 3 felony counts of fraud generally obtaining a controlled substance and he got referred to his state's physician health program or p.h.p. They work with state licensing boards if you follow the treatment and monitoring plan they set up for you they'll recommend to the board that you get your medical license back the ph he's basically say do whatever we say or we will give you a letter that will help you get back to work so they put a gun to your head and Grinspoon says they gave him very little choice to avoid a criminal record he needed to spend 90 days at an inpatient center in Virginia I would use in the Jewish atheist to a Christian rehab place in Virginia didn't make any sense I was just sitting there listening to people recites the Lord's Prayer on hold hands they took me cold turkey off all my medications it was completely insane he says medication assisted treatment was suboxone or methadone was off the table for him those medications are similar to opioids and work by suppressing cravings to the abused drug position health programs he said. Has effectively banned the use of these medications in the treatment plans that they set up for physicians like him why on earth would you deny physicians who are under so much stress and who have a higher access they have free refills and they'd have a higher dictionary Why would you deny them the one lifesaving treatment for this deadly disease that's killing more people in this country every year than died in the entire Vietnam War Grinspoon recovered despite what he called several awful rehabs today he's a license primary care doctor and teaches at Harvard Medical School he also wrote a book about his experience with addiction called free refills Now this was over a decade ago but Dr Sarah Wakeman also at Harvard says most physicians health programs still don't promote medication in treatment for addiction the sort of general standard of care is the sun people the absence based residential treatment programs that don't offer medication treatment she just coauthored a piece in The New England Journal of Medicine which called this quote bad medicine bad policy and discriminatory I think the underlying issue is stigma and the sort of misunderstanding of the role of medication and this idea that a non medication based approach is somehow better than someone taking a medication to control their illness so what do the institutions getting blamed here physician health programs have to say about all this Dr Christopher Bundy runs Washington State's p.h.p. And the Federation of all the state ph p.s. He says it's true that these medications aren't often used but that's not because of stigma or ideology proceeding with caution is understandable warranted he says medication may be the gold standard of treatment in the general population but there should be an asterisk when it comes to so-called safety sensitive workers not just health care providers also pilots for instance he says the concern is that these medications can affect cognition so the idea of people caring for patients while taking something like people in our family makes some people nervous we only need one bad outcome involving a physician. So since he's disorder is back to work then immediately the piece is under the microscope he says then the criticism would be how could you let that doctor work on a medication that could have played a role in that bad outcome he doesn't know of a case where that's happened Wakeman and her co-authors also argue there isn't clear evidence that these medications do impair cognition still that fear could be what's driving the reluctance here Bill Kinkel is currently navigating all of this he lives outside Philadelphia I'm a nurse but I'm also a person and sustain recovery from opioid use disorder he'll be eligible to practice nursing again next fall after 3 years of documented sobriety and he's public about being in our coverage he even has a podcast when he was addicted he said he was under a strong impression he wouldn't be able to go back to nursing if he was in a medication assisted treatment program so he went to abstinence based rehabs over and over again and over and over again as soon as the rehab ended he relapsed a lot of those I overdosed and then had my wife not found me on the floor and been able to take care of me I very well may have died and all that possibly could have been mitigated had I got it or people who are going to methadone he doesn't fault the ph p.s. Or the licensing boards for these policies even though he thinks they put his life at risk he thinks stigma against those with addiction is so ingrained in our culture there's no one institution to blame what's important is that this changes he says and health professionals have access to all possible tools and recovery Selena Simmons Duffin n.p.r.
