Yes, thank you president walton and good afternoon, supervisors. Over the summer we saw a lot of excitement, tourist particularly from europe were everywhere. The championship parade made San Francisco shine. And our neighborhood were past our residents and visitors but with all of that positive movement, were also seeing downtown continue to recover far too slowly. Yet, San Francisco is not alone in dealing with the impact, our Current Office attendance is on par with los angeles, san jose and new york. This is a change that is playing out across the country. Here in San Francisco, we need to do more to draw out our recovery and plan our future. San francisco is a city that has risen to the challenge before and we can again. We need to acknowledge five areas, vacancies and diversifying, enhancing vibrancy, maintaining a clean and safe city, increasing and improving access to downtown and growing our labor force. We have already made some progress in these areas. For example, together we passed a budget bringing festival and other activations downtown, supporting Small Businesses and recruiting and maintaining Police Officers and ambassadors on our street. Those are positive steps but we have a lot to do. We need to be creative and be open to a wide range solutions. We think to think differently. This is about the future of our city. The reality is that so many of the programs and services we provide to help those most in need, the priority so many of us care about are funded, by taxes by our downtown area. If we dont take steps now and if we dont do things differently, we will lose out on that funding. And well be forced to make budget cuts. Thats not what any of us want. Its not what our residents want either. They want us to Work Together to create a stronger more resilient downtown that supports workers, Small Businesses and economic not just for our region. Letsz be open to ideas. I know many of you are doing work in this area and interested in coming up with Creative Solutions and i believe we can do this together. And there is hope for our city. People want to bet on San Francisco. Just last week, supervisor doersy attended a ground breaking. 333 new homes, 25 of which will be affordable and nine floors of office space. That does not happen if there are people in our future. We need to show that commitment and work. We have our policy and in the coming weeks, more ideas will come out. Thank you, president walton and thank you for being here for question time. This month is transit month. And i enjoyed celebrating Public Transit with you at last weeks rally on the steps of city hall. I believe that there is no economic recovery without a full and complete transit recovery. Fortunate, thanks in large part to pressure from transit riders, activist and operators and this board of supervisors we stop line elimination sxz reverse fair hikes during the pandemic. Despite the progress, we remain in a Public Transit emergency. Services lacking, and rider ship is low far below prepandemic levels. Nowhere is the lack of service more notable than during the evening and night time hours. And madam mayor is this, what is your plan for restoring evening and night time maoun service . Muni service. Thank you, for the question, as somebody that grew up riding the system, i know what muni and coming out of this pandemic, we know that it needs to be built on a foundation. That does not mean that things will look the same as they used to, people are not traveling the same way that we were in 2019. We need to prioritize Service Based on our equity needs to match the way that people use it now and to support our economic recovery. Were so happy to hear from constituents that is reliable and people can count on it. That is what is most important. But we need to be honest about our limitation. We dont have enough operators and mechanic nicks to run more service now. There was a hiring freeze and weve had more retirement and natural attrition than we can keep up with. I asked sfmt a to streamline the process so we can hire more operators as soon as possible. I know what a great job being a muni transit operator is. A lot of people i grew up with, are working there every day. Thats why were promoting this opportunity, through job fares and Workforce Development team. Facing historic challenges, these last two years was rider ship dropping dramatically, we should be proud making data driven decisions when providing Critical Services all throughout our city and our neighborhood corridors. Even though we have lots to be froud of, we know that we still have more work to do and i look forward to working with the board and communities and make sure that we build a Transit System for the future. We are not there, we got a lot of work to do and im hopeful that we can get there. Thank you, mayor reed. Supervisor preston, do you have a followup question . Thank you, president walton. Thank you for your response and your work on Public Transit. I want to followup more specifically about night time and evening service. I want to underscore just how dire the situation is right now for people trying to use Public Transit in the evenings and at night in our city. These are workers trying to get home and tourist trying to enjoy our restaurants and bars and music venues. They depend on Public Transit availability outside of the 9 to 5 hours. And our system is not responsive to their needs. 30 of our current muni lines have significantly earlier end times than before the pandemic. Many of these lines, they ran before the before the pandemic until midnight and now they end at 9 00 p. M. Or 9 00 am. This has less people waiting at the curb and more often forced people to either drive or spend an hours wage on a taxi or tnc. This is key part of our economic recovery that has not be received the attention it deserves. So my followup question is this, more specifically what is your message to the restaurant or theater worker who gets off at 11 00 p. M. Or to the janitor who finishes their shift in the Morning Hours . Or to the bartender who cannot use Public Transit to get home after a long night of work. Youre preaching to the quaier here. In terms of all the people that youre talking about. Many of those people are people that i know who are struggling in the city. But when you talk about maoun service as being dire in this 9 to 5 thing that is not accurate. But part of what i mentioned in my comment, is the fact that we need to focus on that muni is reliable and not stretching it to thin so its inconsistent. The fact that sometimes the run is running and sometimes it isnt. And inconsistent see is something that were dealing with every single day. But the truth is, as i said, we do not have enough operators and mechanics to run service right now. So were focused to use data, trying to restore the lines that you know, people are most interested in using and trying to make sure that the service is efficient. I dont think its as problematic in certain areas and times as what you have, mentioned especially seeing so many of our buses, along San Francisco, on the weekend and in fact on the evenings in the evening that have been empty because i pay very close attention to that. But more importantly working with tomland and sfmta, i play attention to the data. And our goal is that its data driven. Its not entirely accurate in terms of how youre portraying the problem . Thank you for being with us for question time. Given the expansion of shelter beds and other investments solving homelessness. At the time, you answered that you believe they would. And since then, my office and i have worked closely with you and your team. And we have certainly seen significant improvement from conditions at the height of covid in 2020. But the status quo in open drug sales and use and people who are in danger to themselves or others roaming the streets is clearly unacceptable. Residents and merchants reach out to my Office Every Day describing scenes of open drug use that are unsafe for everyone involved. And recently the castro merchants threaten to hold fees if conditions did not improve. While im grateful for the opportunity to collaborate with your office and your genuine care for this neighborhood which i know is real, at this point i dont feel i can honestly tell my con constituents in the castro that we are where we want to be. Madam mayor, what can i del them . Can we make this better in a reasonable timeline . If so how . And if not, why not . Well thank you supervisor, manned dman. I understand the frustrated of the residents of castro. Im frustrated too, and part of my frustration is because were adding new shelter beds with 1,000 shelter beds added over the next three months. Thats nearly 75 of our goal of our goal 200 plus for those who serious Mental Illness and Substance Abuse challenges. These individuals are struggling and they need more and they need to make it clear that this is an option that they need to take. The change is going to can have to come in two ways. Better coordination so that somebody is not accepting resources and not allowed to disrupt the neighborhood, were working on that now. For example the department of added health has added staff that are working every day in the castro specifically and with the Police Department and homeless out reach team. With this coordination, we will identify when somebody has clearly rejected our help in a different intervention required. Individuals to disrupt the neighborhood. And i know, you know that better than anyone else, we need changes to our state law while care court will add a new tool for certain individuals. Our browder conservative reform failed to make it through the legislation of this year. But we will try again next year. In addition we will continue to bring resources for all of our neighborhoods. And we will continue our work like the castro merchants to discuss how the city can toader coeder nature and continue to partner effectively with them going forward. I look forward to working with you on it. And i know its frustrating and i know its not enough, but we dont exactly have all the tools necessary to make the difference. Chair and i were able to Work Together to bring it under control. This was to 2026 but because, we were not exactly sure how many shelter beds would benefit those in need as well as and for that reason, during covid they had to reduce the number of people that were served at this location. But know as we have the facility. Some of the reasons were not 100 satisfactory but we wanted to give time to serve the levels. Because of all that, we shortened the original contract amount. Originally the contract was not able to be found. They found it the day of the hearing. So we were happy about that. But we also learned that the contract was not had not gone through the board approval process. This time it was put out for a competitive process. We felt comfortable about that but wanted colleagues to know that were doing all we can to bring accountability. So we shorten the amount of contract of june 2024 and require the department to come back with more updated information and more accurate data. Thank you, colleagues. Thank you, supervisor safai. I have a drafting request hearing and memberium. Today im calling for a hearing for the department of Public Health treatment on demand report. I want to thank supervisor stefani, doersy. We still have not gotten ourselves there. Even worse were not measuring and grappling with the gap from the system. Meanwhile parallel opioid have skyrocketed 474 . Treatment on demand reports mandated by proposition t are due by february first every year. This is not a random date, it was chosen to coincide with the process. This will be my third treatment on demand hearing. Back then, it was not taken the obligations seriously and submitting 1 to 2 page reports. End of story nothing to see here folks. In the years since then, these reports have gone from bad to better though lots of important questions remain unanswered by the 2022 report. Main concern is that we dont have the process despite the ordinance requiring to include an assessment for demand of substance of treatment and a plan to meet the demand. I understand they highlight connecting people to non medical Services Within 48 hours while they wait for a spot in a 90 percent residential treatment. But a 5 to 7day wait for program is not treatment on demand. We need to better understand just how many people are losing as an unable to provide treatment on bhand. And how many are necessary is impossible as the system fails people with addiction. People with duel diagnosis and injustice involved clients. But the report does not plan to extend nor address the policy and budget changes. Last week supervisor stefani joined supervisor dorsey, i support the area and making San Francisco long process treatment will be critical to the success of any such plan. At the time, the citys failure to implement also serves as a cautionary tale. Policies and plans are not enough. Change will require persistent focus with market and from the public. This hearing is part of that project, treatment on demand is more important than ever. Im also submitting a letter of inquiry that by april 1, 2023, the completed analysis of how San Francisco uneven economic recovery will impact the Business Revenue base and recommend priority to ensure revenue resilience. The covid pandemic has had profound impact on where office work is performed. Remote became the norma long trend towards more Flexible Working arrangements dwaoen businesses and their employees. Last and resent indicator that our post Pandemic Recovery is falling behind our other peer cities. San francisco post pandemic downtown recovery dead last in the nation according to a resent report at uc berkeley. Major tech employees are looking elsewhere forehead quarters. Rates returning to the office and air travel all continue to drag well behind prepandemic norms. The long term persistent are still not entirely clear but the implication on tax structure which is heavily and increasingly dependent on revenue are significant. Business taxes are the largest source of revenue and any disruption can have a profound impact on future budget. We need to consider how those shifts will impact and prepare for those long term shifts. So todays inquiry to analyze these potential impacts and report back on the magnitude of the economic trends. And that this board and the mayor may consider as we emerge from the pandemic. I want to extend special thanks and our chief economist ted for helping me and my office think through these issues. I also want to acknowledge supervisor safai hearing on downtown recovery as well as supervisor stefanis prior letter of inquiry in this area which im hoping will help inform the analysis that were asking for today. Next, i have mentioned that i think its past time that we remit remote and today im introducing a motion to do that. Up until 2020, its funny seeing the various different, there is a little applause and some head shaking, today im introducing a motion to do this. Up until 2020 this board conducted all business inperson and without remote except by allowed by the brown act and sunshine ordinance. Over the past two plus years we have provided out of necessity as gathering were not possible until last summer. I want to commend calvillo during this entire time without missing our step. And while the clerks is prepared to have these meetings, we have to recognize that continuing for remote comment including callers who may have no relationship to San Francisco comes at a cost both in terms of resources but also in terms of our ability to conduct business effectively. We resumed at our board and commited hearings, in april the city administrator lifted indoor mask facilities and hearing rooms while in session. And its been over a year since the state lifted and its been over a year that we have met in this chambers. I believe we have more than demonstrated our ability to conduct inperson meeting safely. This motion would rescind the motion we passed in 2020 to initial remote buildings and would require supervisors to be present. Only as necessary to enable the participation of people with disabilities. Colleagues i want to note that last week, we renewed for 20 days to continue allowing participation remote. Im introducing this motion today so we can have the space and discuss and where what the future of remote participation and proceedings should look like and make the hope for these on going resolutions. I look forward to discussing this more in the coming weeks with all of you. And then fourplex, as you recall, we vetoed the mayors fourplex. I want to thank you for the legislation, that you put in. On how to allow more housing in a Single Family neighborhoods. As i said then, im not done with this conversation. I believe that we believe strongly. Those buildings will not be nearly enough to get to the 8 2000 units that the state expects us to build by 2031. A version which is still pending at the committee. And that is discussion around streamlining. While many will weigh in on streamlining, it strikes me that neither of the measures will make it available to build fourplexes. Streamline and approval proj edmontons for those projects is going to be important. As critical as for larger projects. Hcds letter announcing a review calls out the need to streamline for permitting project of five or more units. I also want to note that sb9, the housing streamlining bill that the mayor massaged apply to duplexes and in some limited cases, current homeowner remain onsite for three years after instruction. So even if youre okay with the state taking over our local zoning, which i am not, sb9 is p going to be make it any easier to build smaller apartments in San Francisco. Which is why colleagues i believe its on us to come wup a local streamlining program that will make a meaningful difference in getting them built. Projects that would extend units on a lot where the proj eblgt is treating at least four units where there are no rent control units and where the existing building is not historic. For any project that meets this criteria and all of our other open space requirements, i think we should get out of the way and make it as easy as possible for these units to be built. Even as our discussions about fourplex continues. And finally, im asking that we adjourn todays meeting in memory of andrew tonkin, andy as he was known who lived in the castro was a presence in the San Francisco castro community. He was tragically a victim of hitandrun. He was 35 years ago, andy like so many good young men, built a new life for himself here among his chosen family in the queer community. Its a program to support and resife San Francisco Small Businesses as the city continues to recover from the pandemic. The program weighs the costs and other applicable fees. To date the program has enrolled over 1600 businesses since it began last november. We have heard from many of these Small Business thats are grateful for the City Attorney and looked at ways that we can improve mon the successful pilot. This renewal is critical for the recovery of San Francisco and to make sure that tourism continues to be a viable industry for the city. There will be a few changes in this renewal over the first original district. One of those that increase accessment rate by 30 . We hope to be 10 Million Dollars per year to incentivize business to built musconi center. The district has been passed on to the guest of the hotel or shortterm rental. Youll see that the assessment rate currently in zone 1 hotels which is the majority of the hotels in the central part of city, will go from 1 to 1. 5 percent. It will allow us to increase up to 1 in times of critical or unseen circumstances. This is an industry best practice that has been established since our system was established over 15 years ago. And shortterm rental house which use plat forms like airbnb and expedia. They have not had a voice in the past but it will allow shortterm rentals in a more active and proactive way. And will also modify the board competition to include a rental host to include the city controller and include the city administrator. This is important for San Francisco in many ways. But one of the reasons that big events are critical for San Francisco it caused compression. That means when these large events take up a lot of space, it encourages to expand throughout the neighborhood stay and other hotels outside of the Downtown Core and also in shortterm rentals outside. And really share the growth and the benefits of the tourism spending and job creation around the city. We believe that this tourism renewal is going to be critical for San Franciscos recovery. This has been great to share especially in the southside of the city which most of them are absolutely thrilled to have discovered for the very first time. We give our guest access to clipper cards so they can use muni and public transportation. We also show them how to stay safe. I give instructed about not leaving anything valuable in their car. And also arrange for them to drop off their bags early before they take off to do other things in the city. And also allow to keep their bags with us if they have a late flight home. Another idea that you can present, is this is a small amount of tax and anybody who does efficiencies in ar, does it, every time you touch a payment, youre losing money. So it would include a plat rate per year, like 500 dollars per year and sdr can devy up. So right now, at this point, i have like six different log in. Many of those are tourist and their dollars, their administration helps us support our access programs. Half of our field trips were free. And today 30 of our visitors come for free for San Francisco all program. As soon asing they show ebt, medical, medicare, wic card they and their entire family get to ride for free. The other benefit is our ability to influence the rest of the world at a time where tolerance, equity and inclusion is much sorely needed around the world. I often hear parents at our museum comment about how beautiful it is to see kids of all races and back grounds play, imagine and create together. Theyre building the world we want to see in the future. And if we can bring more tourist to San Francisco who can appreciate, able able to export those values to be beyond our neighborhood. Its literally a difference of probably, a multble or two or more. Thats an example of information that i do not have. Before i make any questions, there is a set of questions that i need to answer, nor decide to allocate funds. And these, perhaps some of these questions can be answered in what is referred to as the management district plan. I understand this is available in the Clerks Office in a physical form, could it be possible to make this in a digital form . I dont know if this information would be contained in the report. Past results generated by the tdi. How was the money spent . How were the goals achieved . I know there was an attempt to say that these things were done. We offer many signature programs. We have specialization in mono lingual care and only h. I. V. Stiff unit in the bay area. And we also have a specialization in memory care, next slide. All hospital in california are required to report many types of incidents that occur within the facility. In summer of 2021, laguna did just that. Two patients that had on day pass and returned to laguna and suffered drug overdoses. Luckily staff were able to reverse those overdoses and those patients survived and okay. In the midst of make thating report, they conducted a series of different surveys to address findings related not only to that case but to other observations that they made while laguna. There were a series of surveys 5 to 8 followup surveys, getting down to around april of this year. In which there was one final survey where the state came to laguna and found no deficits. But they did find hygiene and the types of deficits that you may find in any facility. Where mitigating factors are put into place and they in turn give comment and at some point approve that plan. But because the clock had started six months earlier, there was no time to remedy the basic and indicated that they to end participation in the program due to not having each and every area by april 14th, 6month deadline that started six months prior in october. It meant that our our Funding Source medicaid funds to the tune of half a Million Dollars a day. Next slide. From the very first day of decertification it became clear what it would be, and that was to begin the daunting process of reestablishing certification with cms for medicaid and medicare services. We got to work on developing mild stones and an action plan that hadle need to do both from short and long term operational and structural and cultural changes at laguna to make sure that laguna was in the best position to meet the requirement. To that end, we brought in consultant in the middle of may. They were experts in operations and also in Public Healthcare operations. Consultants conducted an initial assessment, particularly in terms of itsz areas of non compliance with cms and made some initial recommendations and changes, many of which all of have implemented and in addition to the process of doing a second more comprehensive top to bottom assessment of laguna. We however, we are confident that with the expertise of these consultants combined with many of the staff who have been reployed that we will be successful when we submit for medicaid and medical recertification. So i want to take a little time in terms of many developments that have happened while you were on recess. You recall when the decertification rured, as supervisor millgar mentioned, it was submit for and mandatory transfer discharge and closure plan. You you recall that given the complexity of the patients almost 700 patients, it would take us 18 months to effectively implement the plan that they were requiring. Instead mandated that we do what should take ten months and only four months. In terms of assessments beginning making referral to see other Skilled Nursing facilities and lis charges to other either home or board and care levels of care. We implemented and you saw the results it had on the patients and staff but the entire San Francisco and bay area community. Luckily after two months of implementing that plan, cmf and the state agreed to a request by the city to we had discharged 57 patients during that 2month period. And during august, we were pleased pleased through the City Attorney office reach an agreement. And that the pause would also ensure continued funding to laguna through november 13th. And we saw that as a win, it allowed us to main the integrity of the hospital. But allowed us to stop fighting. On the closure plan trying to refer these patients but at the same time we had an equal amount of effort going through the recertification for fixing all of those deficits. So again, i have equated to the fav that we were fighting two battles. Again this helped us it allowed us to use the resources with a single towards fixing the areas along with our consultant and our staff. Next slide. We said our number one goal was our first big step. They made tremendous impact both in terms of bringing expertise to laguna but also resources to laguna. Next slide. You recall that we shared towards getting back into cms compliance and certification. The first was a mock survey and think of it as a pretest when were all in school. So they simulated and ending during the week of july 21, our first mock survey. They used the tools that the surveys are going to use. As planned that mock surveys many of these issues that had previously been identified by the state. Ppe, the ability of the nursing units to be in good repair. Subsequent to that and as to any survey, plans of correction, we worked with our staff. We had planned of correction summit. Along who said what do we need to fix these areas and come into compliance. We did that, submit both to the consultant who were acting as cms surveyors, they made comments some areas we had to change. Were moving forward with a little over 100 different plans of correction in order to address the various issues. Making sure staff is correctly. All of those things are somewhere within the 200 federal rules and regulations regarding facilities. Were implementing the plans of correction. You recall that we would implement and make sure that we will validate that theyre continuing to work. And then were going to do a second mock survey. Again we want to make sure that were going to be successful. So were doing a second pretext. And thats key, when we get the results of that will let us know are we ready to submit to cms and be successful and act in the actual cms certification. So from mock survey, they looked at all the areas of deficit. It was not a could rick lum that you can buy anywhere. To train and retrain. This all occured during the month of august. There were a series of four cohorts, weve trained over almost 1200 laguna honda staff, myself ive gone through the training so has dr. Hathway. Our physicians, our reporters, our dietary staff, anyone who is part of the care team is required to go through this. We have exceeded our goal. You see what the area of education and training were. Emergency services and reeducating staff on making sure that our patients are free from abuse. And how to deal with patience with Behavioral Health challenges. The first three weeks was training that every employee did no matter what the job qualification. That was our for clinical nursing who provide training. Again rerevieed best practices. And what they said we need to employ an adult education. With all sat through the courses on the areas that we just discussed. But after that, we had to go through skills demonstration. We had to be observe washing our hands, and then sign off that we know how to do that appropriately. This is all staff. We have to physically, what we call dawn and dot, take on and take off ppe, the hospital gowns, gloves, protective goggle and masks, we had to go through that. And then we have to take a quiz, in addition to the training, we have to skas a written and verbal quiz that we were able to retain the knowledge. Consultants have told us that they know this will pay dividends. Next slide. So in addition to the key mild stones, the second mock survey that is coming on for october, weve done many additional improvements. We instituted 24hour a day. Le both our expert consultants but some of our own staff. They go around to all of the hospitals and observe staff making sure that theyre following all of those infebruaryingtion control rules. So theyre making sure that staff are hearing their gofz. They saw where staff were inappropriately hearing inappropriate staff. Now were having that real tief training. When they see non compliance, theyre able to see coaching and they say, youre not suppose to wear the gloves in the holloway. Can you tell me about that, why do you have the gloves on. So there is education but in addition, we monitor the rounds to see are we showing improvement is compliance reducing. And im happy to report that were seeing compliance. Theyve been part of the group to deliver the message how important it is to make sure that were in compliance. We also put together a severe minding. So when the team are doing their round, usually if they find, errors sometimes theyre not very significant. But sometimes they may find something really significant like a crash cart that has the supplies you need in a case of a heart attack or stroke. We consider that a severe finding. So we have a strike team that meets with the management to say you had a severe finding. Lets talk about why you had it. So weve got a lot of interactive communication, going on with our staff. To ensure that we have the best chance for success. Next slide. Initially and currently the communication we have from cms is because laguna became decertified, whenever we go for decertification, rather than holding us, theyre going to hold us to the 2016 rules that they adopted. In 2010 when laguna was built, it was okay to have a room with three patients that share one bathroom. They changed the rules that say you can have one room with two patients that chair one bathroom. What does that mean for laguna . It means that a total of 120 beds do not currently meet that 2016 requirement of cms. We were under the impression that we we would have to take the 120 beds out of our license. Thed way it works in california, every year hofpts have to renew their license by the state. Were in the process of submit ising our relicense now. Now when cms comes out they will have to make a determination, the beds that we submitted and license for. What this means is this is still unsettling issue. I wanted to make sure that youre aware that thats the route were taking regarding the beds. We keepsered the largest covid outbreak in laguna since the beginning of covid pandemic. August 25th we got to highest possible. Dr. Hathaway in addition to being the medical doctor served as Incident Commander for our command system which is a heightened level. So dr. Hathaway had extra covid19 precautions which had implemented that had since gone away but came back. We instituted that all visitors had to resume being test asked found to be negative before they can come into the facilities. We instituted enhances ppe. Not only wautz it okay to have k9 5, you had to have n95 and eye wear and goggles. So were happy to report from august 25 to yesterday were down to only 11, 8 . Only 8 cases of covid at laguna. We felt confident to get it under control but always want to be prepared. We opened up a second positive unit. We want to move them out so they dont infect other patients. We made plans to open a second unit but thanks to the work of dr. Path away, we didnt have to use that unit. Next slide. So as i begin to wrap up, again want to leave you with the thought that we certification is our main focus. And that is sending mock survey that will occur in october will tell us if were ready to submit and well then move forward accordingly. So that is my presentation and im happy to take any questions you may have. Thank you, so much mr. Peckins. I do see some questions. Supervisor melgar. Thank you, i im happy to see that this saved lives in the couple of months. I want to thank the folks that advocated for that. Our former City Attorney, louis and joe urban who is supervisor stefani constituent california advocate for nursing reform and our great panthers and dr. Teresa palmer who has been leading this fmgt i wanted to ask you a couple of specific things. We now have until november 16th. 13. 13th, im sorry, thats our new date. Im wondering if you feel well be ready. Or if we have to renegotiate what that terms are of that pause because you know, i certainty dont want families to be put in the same positions of having to scramble is that anxiety. That is the first question and the second question and i dont know if thats for you or dr. Hathaway. And thats staffing, ive been a supervisor of other people, and i know when you challenge folks to learn new things, it makes people unhappy. So im wondering how Staffing Levels are at laguna honda . Have we lost people . All of those things, those were my two initial questions and im happy after to turn the floor over to my colleagues. Great. Thank you for the question ill try to answer the first one. Im confident that were on the right track to making improvement better needed to be successful for recertification. Are we there . No were not, and thats what the first mock survey showed us. So were implementing the sixes now. Ill be in a better position to answer that question after we have a second hoc mock survey. That will tell us how quickly we want to submit. We dont want to submit and then fail the survey. I can tell you that the progress being made puts us in a good place to continue with the mild stones that we set out. Okay, and you said the second survey is scheduled for october. Thank you. How about the second question, regarding staffing. Oh staffing, as a supervisor youre aware particularly as you make changes and do trainings and retraining, particularly to staff that are used to doing it a certain way, you know, its a process. And the mayor was great when she came to her visit to laguna. She went to the Training Session and spoke to the staff and said, look i know were asking you to do things in a different way, but the rules that were in placed when laguna opened in 2010 are not the rules that are there now. So some of those things may need to change. Dont take it the wrong way when youre being asked to change your practice. Know its to keep staff and patients safe and maintaining regulatory compliance. The issue of staffing, is always a big one in any healthcare institution. In particular in hospitals. As somebody who started my career as nursing assistant on the floor, you never have enough staff. And also exacerbated when people are in leave. There are always challenges thats why we try to do per diem staff. I can say that we made strides. There was a new class of patient care assistance, there were 40 or 50 that just got hired. Were working to fill those vacancies. Again, theyre always competing interest. And right now, laguna has been put at the top of the competing interest. Thank you, so much ronen. Thank you, thank you for the very detailed informative report and supervisor melgar, thank you for your leadership. Im glad to see progress. This is more of a comment than a question. I want to thank you for keeping the 120 beds licensed that is a huge keviner of mine. Its not juflt in the city, its all over the country where its no longer for possible for business to see maintain board and cares, its just not affordable anymore. Losing 120 beds at laguna is is absolutely devastating, i workweekly to help assess and that is bed, which includes building new beds and getting opportunities recently and pretty much all of last year. And thats not even close to 100 beds. And there are thousands of people still on the street that are battling Mental Illness that can use Skilled Nursing facilities. So i want to put on the Public Record to cms to all leaders that have Decision Making authority over the relicensing, what alternative are you providing . It is incumbent to make sure that were running a top quality insolution. However if you shut down this facility, what is your alternative . Last time i checked you dont have one. You dont care of our patience, you dont provide Health Services for those that need attention. They dont provide an enormous amount of care. Smaf, we need to force them for every single bed. Cpmc has clodes down their Skilled Nursing facility. So truly what is the alternative and how cruel do you have to be to close down or even shrink the size of the facility, it continues to blow any mind. Thank you for the report, thank you for the progress. You know, cautiously optimistic that were moving in the right direction. And i really want it appreciate you dr. Pickins for all your work. And mr. Peckins. Mr. Peckins or acting director or ceo for this update and really glad to hear that the recertification is going in the hopeful direction. Really glad to hear about the urgents, when we had the hearing last time. So thanks for your work on that. Now that there is transfer to november 13th. You said there were some changes. Im wondering if these patients are going to be able to return. Thats good to hear. So youre saying that opportunity would not happen until after recertification . Yes, cms in the state have said while were in disclosure, we cannot accept new admissions. We cannot accept new patients, what about does that apply . That includes the ones that have been transferred. Okay. From your report, it sounds its been around Staff Development and staff education that sounds really positive and important. And you mentioned there were new patients assistance hired. Are those new positions . Or were these vacant positions . And how much of the recertification process is related to staffing . Inadequate staffing . Thank you supervisor mar. Those were position thats were not field. We were able to enlist the hr. So those were not new additional ftes those were that were not field at the time. I would say its pivotal. We cannot be successful if we dont have the staff to deliver the care. You know, there are like hospital ratios of staffing, based upon patient accuity and so we do meet those ratios but of course we staff to those ratios. When you have lastminute cancellations. We try our best to get somebody either through our per diem staff, asking some staff to work overtime or registry. But, if that is not successful then we do sometimes have gaps in having positions filled. Thank you, thank you mr. Peckins. I dont see any other colleagues on the roster. Im really glad to hear that the recertification sounds like its going the right way. But i want you to look at the elephant in the room long term. Which is can this model of laguna be i traditional nursing home and basically offloading platform for sf general continued because i fear if it does, well be back here with a similar problem. I saw this train wreck develop, we saw the er fill up. And ambulances would take the patients to private hospitals. And so we would be on divert. And one of the things that we found out part of the reasons there were patients upstairs who did not need beds but there was nowhere else to go. Part of the hospital were half than pleased. They were not getting reimbursed and too pleased to do charity care. This did not necessarily evolve for the good of laguna or laguna patients. Some of these folks who are at laguna honda do require some additional staffing. To take care of them, i want to make sure that were maximizing the staff and staffing up to prevent any, other mishaps that may happen. I feel most of the staff that has come up has been due to short staffing. And i just want to encourage the board to stay a breast of this and make sure that the staffing is appropriate for the patients and make sure that the staff have what they need to take care of the patients that they do have. And closure, discharge of patients and closures should be complete he uncoupled from certification and recertification, there is no advantage to anyone. The deaths will resume. Im asking the Health Department to stand up for the lives of the people of laguna honda and do whatever necessary to stop the discharges. Cms says they will pull funding. If the city needs to give bridge funding while, writing the neglect that has occured over the years at laguna honda, then maybe bridge funding should occur. Palmer said, the, the what is at risk here is the funding. Laguna honda must be kept open. This is really a situation of city having broken and it needs to, it needs to buy it and fix it huge amount of money, so be be it. Why does it take an emergency of nine people dying in order to get the service thats are needed for years. We would always hear of positions that were in place but not being filled. The other thing is has the ability to declare a local emergency. It can be blamed by the governoring body if the conditions of disaster are extreme. Thank you for your comments. I want to stress that the closure of laguna honda means that over 1200 members would have lost their job and this we cannot let this happen. Were working hard to make the improvement that mr. Peck ins talked about today. The survey demonstrated the status of the hospital with failures it highlighted the mountains that we have to climb to recertify. We do appreciate the trainings that were provided to everyone to make sure that everyone understand the procedures and protocols that needs to be in place to go through the recertification process. Though not perfect, they have been willingness to address issues that we have brought up. Much more improvement since weve learned about mon da status. We do not believe that the six of month period is enough. Given everything that has been put in motion. The speed by this is taking place, is bringing a lot of stress. Our members are fighting every day to keep the hospital open. We have to admit management have tried to address it but they cannot keep up with the number of staff that are going on. Retiring and transferring and replacing them. So if the board of supervisors can do anything in that way to help, that would be helpful. We have to state and that the plan is not always followed. It was not the death of the people that got them to stop. Im donna and i live in complimentary medicine and i have a disabled brother at laguna honda. San francisco is his home and we both want him to reside there. We have been involved in the benefit of the brother. Its very difficult to not feel terrified as a Family Member and completely outraged especially dwards the cms decision. Im grateful for their commitment. And to demand a safety net for those Skilled Nursing facility, as i listened to mr. Peck ins, they have doing a good job of getting into compliance and i dont think this is the problem. What is really important is the cms they will continue to transfer patients despite the suffering that this may cause. At least that is what they say. Im i am mroeg a board of supervisors. The inappropriate admissions of people to laguna honda Hospital Free of those beds by supporting Substance Abuse programs and beds to give people the care they need and the sufficient services for elders and people with disabilities stay in their communities. The use of inappropriate home admissions has crippled the ability to deliver care to the catchman area and the appropriate population for which it was to serve. Its mission must be fulfilled. Im also very concerned about staffing issues. I feel like mr. Peck ins did not address the staffing crisis that on going at laguna honda and has been for a long time. So board of supervisors do what you have to do to keep laguna honda open. To keep the beds free. Thank you for your comments. Do we have another in the queue please. Speaker can you hear me . There are issues at laguna honda, issues of nepotism, we to prevent and block from being able to be at this hospital. This is a very important healthcare facility. To ouier community laguna honda was built to take care of the old and it must be there for the community. It must be its a Vital Healthcare facility that must be there to serve the country and the city. So we must keep it open irrespective of the challenges we face, we must overcome those. Cms cannot address the issues of poor people, the issues of patience who need this facility. So lets make sure to prioritize and make sure that the staff is there to take care of these issues. I look forward to continue to go work with you and i praoern the progress that we have made. Laguna honda has remain open and accessible to the community whom reserve. And disrespecting or undermining poor people and people who really need this facility. The city and county of San Francisco, the country needs more healthcare not less. I would like to read a statement that sda board passed. Says that action, to stay open and continue serving residents of San Francisco. But we also call on the city and county of San Francisco to put city resources into creating more options for resident of laguna honda and other nursing facilities to live in the community. Many have not been given that option to live in the community with support. More housing and support must be created to make this possible. We realize, i will add that many people need to be in a Skilled Nursing facility particularly in log na honda which is our premier housing facility. But we also need to put city resources in creating more options to some people so they can get housing and care who do not need to be in skilled facility. To support people who need to be laguna honda, we oppose any transfers. But also to work with the community so that there are opportunities. Mr. Lam, lets hear from our next caller please. Senior and disability action board. Youve heard the positions of both of those organizations, so im not going to go into that, you can do that over and over. I just want to say that in my working life, i was a geriatric social worker and i had clients who did have to go to laguna honda eventually. Log na honda at that point in time took very good care of them. And allowed them to stay within reach of their communities their families, their medical providers. So please put more energy into this issue, log na honda must continue and we need more resources in the community, thank you very much. Thank you for your comments. And i endorse the comments made, the great panthers have made senior and disability action have made. And i want to point out that laguna honda provides services. I happen to be fortunate as a senior, i live in a continuing Care Community that has a Skilled Nursing facility. Has a care unit and i can afford to pay for that. But we have an obligation to serve. We need to staff it fully. Were having staffing issues here and we can afford to pay pretty good salaries. But there is a shortage of skilled nurses and cnas and other people right now. So the city has to step up. Im one of the people that is watching. Im watching on comcast available, i can tell you and my community of 300 seniors that were very concerned about this. Have many conversations about this around here. Im really happy to find the progress. And there is a lot to be done. We cannot allow any patients to be transferred out of this point. Next up would be supervisor safai to hear introduction of new business. Love going after a long hearing, got to get my head together. So i have a few things i want to talk about them. Some of them im going to talk about abbreviated version. One of which something that supervisor peskin and i were talking about. Large institutions and hofpts and hotels others, when we looked at the legislation, we were looking at our goals to achieve zero waste and diversion by 2030. We realize that number of participants were not really participating in the program of sorting and recycling. And diverting landfill. And a lot of these facilities and other frontline staff. We then put in on to the legislation that we would require update. We wanted to see how the progress was taking place in terms of diversion and who was participating. Question wanted give people the opportunity to really survive generally during what happened in that economic down turn. Were calling for a hearing im introducing that today to give us progress on how those large generators of trash are doing. Secondly, its colleagues will remember, we saw a number of institutions businesses, Small Businesses that were broken into. We have worked with the Planning Department looking at our planning road to allow for and again, this is some of the cannabis industry to allow for their business to use roll down gates. So we have used for Planning Department to have a roll down gate and railing and grow work so that they can protect their businesses. Something the industry ask for. Something that they were denied when they requested it. So we also wanted to deal with there is a lot of existing businesses that are not non conforming that would not be allowed to continue to maintain the roll down gates. And going forward, we would not allow for these anyone other than these particular areas. We have to prioritize safety with organized groups and they had the billing to overwhelming the existing. So these crimes impact the businesses and they have an affective on the entire corridor. And these types of activities. This allows for the role of gates. Next, saoupz we have contract at the interest introducing an extension of towing contract. A lot of abandoned vehicles that were creating blithe that were stolen and dropped off and left there for a significant amount of time. This allows for removing vehicles. So that this can have towing. This is supported by organized labor and teamsters, we did not allow for a longer amendment the last time that we wanted see proof. It should be coming to the budget inland use committee. Supervisor stephanie put a letter of inquiry as it pertains to our economic tax base return to work practices. But essentially what were envisioning, the sometime tore discussion and putting the data together and looking at the facts were at about 30 office occupantly. But because of that, were seeing a slower return to office and recovery of our economic base. All of these areas have business tax base in our entire city. Ill say it again, has to be on the table but also reenvisioning and understand of our land use policies. We cannot believe that we will continue to have the same amount of office space utilized as the way it has in the past. Comprised about 12 . Our business tax revenue is over a billion dollars a year. As our policies are right now, and slower return, we will see a Major Economic hit to our budget and will impact many many industries. So this should be the uses in the downtown score and what we think about the future. Im going to be asking forming, and anyone else that wants to participate in that. To look at other measures that encourage the overall recovery of our economic Downtown Core. Others that will say their membership has been laid off consistency over the last two years and the end insight. The hotel and tax revenue that is generated and also that are frequenting and visiting these businesses that are not coming back at a faster pace. And recovery continues to lag behind the rest of the nation. A report indicated that our job recovery is 50 lower and 10 lower thats a whole. Right now we have the most challenging office mafrkt in the country. Thed San Francisco metro area has the lowest area of workers back at the office with about 10 employees. The City Office Market had a 22. 6 vacancy rate at the end of 2021. And 30 of defined as the citys economic core. Prior to the pandemic, the area was responsible for generating more than 345 of the city sales tax since then. The neighborhood has seen some of the largest decline with some zip codes experiencing 60 drop between 2019 and 2021. Since recess, i have met with department and theyre developing a model to protect the impact of of the lack of demand. Iefp also met with the chamber of commerce who are telling incredibly dis heartening stories about their attempts to even giveaway property and rent it for a dollar and they cannot do that. I know that will be ready in october. The impact op our local tax revenue. I will also be closing todays meeting on monsignor sander field.
