That was nbcs miguel almaguer. Joining us former fda commissioner dr. Scott gottlieb. Doctor, it was a widely held prediction, once we reopened we would be a spike across states. What is the data ngo now telling us . What we should be looking at is hospitalizations. Caseless go up as we test more. Were going to find more cases, but metropolitanizations are a more objective measure. So we saw steady declines nationally, both when you look at the tristate region, even when you exclude the tristate region, we saw sustained declines, and then over the last week we saw hospitalizations start to go up. You look at states like florida, georgia, alabama, maryland, virginia, wisconsin, weve seen hospitalizations go up. Arc sauce and mississippi as well. We expected that. So so we just need to watch it closely. Thats why a lot of states had a faced reopening t open a bit, look and take a pause, then open other parts of the economy. Is there a certain range that for you would warrant states to reconsider reopening, change course . Yeah, its a really great question. It depends on a state. Its going to depend on the baseline, what the capacity of the state is, how much infection they had to start with, you know, so a big increase in a state that wasnt heavily impacted might be less worrisome because it might be symbolic of a single outbreak. You never really got rid of the transmission, and now youre seeing a big up tick like maryland or virginia. So it depends on the state youre talking about. Minnesota, for example, north dakota, they saw increases, but they were increases off a lower baseline and the result of isolated outbreaks that should be containable if they get the proper resources into those regions. I understand why investors in big pharma care about every possible headline around a vaccine, but is the general public getting too wrapped up in the hopes for a vaccine in the immediate future . Is it tracting from the need for all of us to practice basic safety and understand what its like to live with this . Yeah. I dont know. Thats a good he question whether or not consumers are looking forwards a vaccine and think it will be right around the corner and letting their guard down. I dont think to. I think most people recognize a vaccine isnt right around the corner. Early data, and i think it gives us renewed confidence well get a vaccine for covid. But its a 2021 event that well have a vaccine available for widespread use. We might have something available in the fall we would use in a limited circumstance by the food drug administration, but in terms of mass knock la inoculation, and i think we have to get through one more cycle in the fall and winter before we get a vaccine, so we need the screening, casebased interventions to prevent outbreaks in states and cities from becoming another national epidemic. Do you think we are doing that . We are seeing it happen in specific states, but we seem to be the only developed country that doesnt have a National Game plan for this. I think what we lack is a National Plan with respect to screening and the reporting of data. I think what we need to do is have a much more comprehensive aproperty to try to get testing into the community and into atrisk environment, so were testing people who are not just symptomat symptomatic, but people who are low symptoms or asymptomatic. We need to make testing more pervasive and start turning over those cases. Its not just symptomatic individuals. You can be just as contagious if youre mildly symptomatic or even asymptomatic. We need to start identifying those individuals as well. Thats take a coordinated testing. People who are in contact with a lot of people, these warehouses, also certain housing situations, where people cant social distance at home. Those are higherrisk settings. People who come into contact with a lot of consumers, people who have to take mass transit to work. We need to make sure were bringing testing into the communities. Whats the level of confidence that any of that is going to happen . Its happening on a statebystate basis. I think its not happening uniformly across the country. Im close to the governors of maryland, massachusetts, been working as part of their advisory boards. My home state of connecticut, ive talked to the governor, hes very fog kissed. They have a plan to bring in testing to certain job sites and certain housing environments. Its not happening nationally. There are states that arent as focused on these challenges. This is where we need to go to prevent outbreaks in the fall from becoming large outbreaks. We have the capacity to do that now. We shouldnt be caught offguard by this second wave. Its very likely to call in the phenomenon. We can split gait it, but we need to start planning now. Even if we get a vaccine, do you believe enough people will take it to make a big difference . I do. If you look if you think about how many people need to take it, you probably need to get to 40 . Vaccination rates, about 45 of people seek the flu vaccine. The reason why went dont need to get to 60, 70 , by the time we have the vaccine, probably 15 of the population will have already had the virus. If we continue on the slow burn right now, were turning over about 20,000 cases a day, its possibly well continue from here through seven at this slow burn of infection. By september, 15 will have had the vaccine. So when you think about a vaccine available in early 2021, by that point, at least 20 of the population will have had it. If you can vac nate another 30 , 40 , that gets you to herd immunity. I want to ask you about guidance. We did get the cdc guidance last week, but it was cumbersome and confusing. In many instances its exhausting people. Do you think American People have gotten clear messaging around the bakes to protect themselves . Well, theres two different messages. One is clear messaging of practice advice on how to lower the risk. I think we have done a good job of providing that. Practice good hygiene, narrow down your social circles, limit your activity. All of the things can you do commonsense things to tro i to reduce your risk of exposure, and then theres technical guidance to businesses. How do schools reopen . How do restaurant reopen . How do you set up a restaurant . How do you practice good hygiene . How do you each up hospitality venues . Is the more specific it is, the more i think its beneficial to businesses. You know, the documents that the cdc put out were really intended for businesses and people can digest the more technical guidance. Mcdonalds put out their own guidance, and the airlines have extensive guidelines. I think businesses need to be granular with what theyre going to do. That needs to come from cdc. Frankly it came late. The more that cdc put out, bottom like, practical guidance, the more its going to facilitate reopening. Some of them will wait until the cdc, and in the absence of that, theyll develop them on their own, and it will take far longer to do it on their own. You mentioned masks a moment ago, but i want to underscore it before we go. Theres a lot of talk and confusion. Do you believe that wearing a mask makes a difference . Absolutely. First of all, we know wearing a mask will protect people from you. If you have covid and youre asymptomatic or mildly symptomatic, you dont know you have the infection so youre not inclined to state home, if you war a mask, you are far less likely to spread is the infecti infection. The guidance was always if you had flu and you have to go out, wear a mask. A cloth mask less so than an n95 or level 3 procedure mask, something that comes out of a mel set, but a clolt mask will provide some protection. The way this spreads is through contaminated surfaces, touching somebody and touching your space. Respiratory droplets either need to get into your nose or your eyes or and mucosa. But jack dorsey is up for this fight. He doesnt work for the president. This is a state that republicans didnt usually have to worry about, bud mark kelley, and martha mcsally. A big lead in the polls for kelley over mcsally. Democrats have been very optimisti optimistic. I a state that biden can compete in and win, and a state that mark kelly, and the polls heart certainly been encouraging. We can put this up. Maybe montana, democrats got governor bullock who is running slightly ahead of the republican incumbent. Kansas, democrats unusually have a pretty decent shot there, depending on who republicans nominate. And in georgia there are two seats up in georgia, and democrats have opportunities in both of those. So right now i think the senate is about 50 50. In your mind, what is the most interesting race to watch . Theres lots of them. I find the Maine Senate Race to be interesting. Susan collins is the classic, you know, independent oriented, had always been able to pull support from both sides. Last time she was up she had 39 of democrats support. But these days its just hard to stay neutral. Between the tax cuts, the kavanaugh vote, and impeachment, her ability to pull over democrats and independents has been really hurt. I think thats probably the most interesting race to me, but there are a whole lot of good stories here. Theres going to be a lot to cover in the next five months. Tough to be independent sitting on a fen for such a long time starts to hurt. You wrote that unthis is Senate Republicans on the defense. Explain. Republicans only had one seat up in a had been had beillary c. So the democrats were playing defense. This time democrats just 12, but a bunch of republicans, and its not really Enemy Territory quite like last time, but in very purple swing states. They tend to be in states where democrats fortunes are rising. The state that went into the fastgrowing suburbs, where the complexion of the states are changing, like arizona, like north carolina, all these people moving in from other states into charlotte, Research Triangle or atlanta suburbs, and savannah suburbs. This is the area where democrats are doing better, and workingclass whites up in the frost belt, where the democrats had not been doing as well as they used to. So were seeing some real alignments taking place in american politics right now, and these Senate Incumbents that are obvious in tough states, theyre, you know, its something they have to cope with, and the senate is make a erratic behavior cant make them feel good about their chances of surviving. I want to ask you about that before we leave. Theres new analysis in New York Times specifically talking about the trump effect in these races and how republican incumbents in swing states are struggling to detangle themselves from President Trump whose voters are super loyal, but they represent only a minority of the electoral. If youre a republican in any one of those states, how do you balance the two . Can he . Can you . This a nowin situation, if they aggressively or visibly try to put distance between themselves and the president , you al onnate the president s base, but on the other hand if they embrace the president , they hurt themselves with the middle appeared what theyre doing for our constituents and almost act as if President Trump you dont want to diss him, but you dont want to embrace him. Do you think its a headline thats supposed to get washington worried, or do you think its real . Could this actually happen . Well, i think its a bit of both. I think theyre using us as political pawns to try to put pressure on the federal government to try to get the money theyre asking for, but i do understand the economic crisis that were in, and i think that it could be a possibility. If they dont get that money, or if the money doesnt exist, there could be cuts, but before there are cuts, there could be things where money could be saved in some of these progressive perhaps, the Progressive Agenda thats been going on. Again, i think safety of the citizens of new york, safety of firefighters, safety of police, safety of ems should be a primary concern. What have the last few months been like for the fdny . Well, theyve been troubling. Weve been very busy. Obviously the pandemic, the concern about ppe, the concern about bringing this disease home to your family since were on the front lines and being exposed on a regular basis. The fire side of the fire department, we havent lost anybody, but ems has lost four. Civilians we have lost seven, so the concern of your life, your safety, the concern if you get sick or if you become disabled result of this sickness and not being covered. God forbid you die, are there any benefits there . Are our firefighters getting hazard pay . No. We are not getting hazard pay. Weve been working a lot. You know, we get overtime for working the extra tours, but theres no hazard pay here. At this point, now that the numbers are down and the hospitals are not overrun, do you have all the ppe that the think will keep all your men and women safe . Well, right now im told we do have enough. For a a time we are dangerously close to running out so the department started rationing. That seems to work. We got through the high points of this pandemic. As long as our scheduled ppe deliveries continue to come, i think well be fine, but they right now ppe seems to be good. Well, that is one positive.
