That follows pfizers announcement just last week of 90 effectiveness for its vaccine. This morning, dr. Anthony fauci says he anticipates modernas vaccine will be granted emergency use authorization by the fda. Its going to be all hands on deck. Everybody is going to do it correctly. Theyre not going to cut any corners. Theyre going to do it quickly. We project that by the end of december, that there will be doses of Vaccines Available for individuals in the higher risk category. Do you anticipate we could end up with three, four, five efficacious vaccines . You know, savannah, i think thats quite conceivable that that will occur. Yeah, i think its fair to say this good news couldnt be more needed as the pandemic worsens to alarming rates in virtually every state in the country. Lets get you caught up on the latest facts. The u. S. Just crossed a terrifying milestone. More than 11 million confirmed coronavirus cases. One million more than in the past week alone. According to the covid tracking project, close to 70,000 americans are hospitalized with this virus, a new high. Its presenting new challenges for our front line workers. The daily beast details one sobering account. A single nurse in rural namaha county, kansas, where there are 10,000 people, one single nurse triaging more than 600 covid positive cases over the phone because the hospital she works at has no space left to accept new patients. Ahead, well be talking to a nebraska doctor about what hes seeing in his hospital as the pandemic intensifying. Cities are trying to curb infections from illinois to wisconsin and vermont. In a couple hours, were expecting philadelphia to announce new restrictions on businesses there. First, we start with the major vaccine news from Pharmaceutical Company moderna. Nbcs Miguel Almaguer has the details for us this morning. Miguel, i think a lot of people smiling, breathing a little bit of sigh of relief, but tell us what we actually know right now about this vaccine. Reporter yeah, chris, certainly good news. Moderna says theyve been working on this day and night. Researchers and scientists have been in the lab the last several months. The efficacy rate is 94. 5 effective, higher than pfizers 90 effectively that was rolled out a few weeks ago. This study was done on about 30,000 people, and it was shown to be quite effective. Moderna says they believe they can get this vaccine rolled out here in the u. S. As early as december. Now, who it would go to remains unclear. Likely First Responders and the highrisk folks dr. Fauci spoke about. I spoke to modernas president , and he told me this. My hope is that it provides hope that were going to beat this thing, and beat it sooner rather than later. The truth is, it is still going to be a lot of work to get there. This is an important milestone. We now know we have a vaccine that works and can protect against covid19, including severe covid19, which is terrific. But we now need to manufacture those doses, millions and millions of doses, as well as make sure that those are distributed to people in need. Thats a process that will take many months from now. Reporter moderna hopes to get fda approval for emergency authorization of that vaccine in a couple of days here. That means the vaccine could be rolled out as early as december. I asked the president exactly how long it would take for the general public to get the vaccine. He said most likely by the spring, which is certainly good news, as coronavirus cases continue to spike all across the country. We also face many new restrictions in mandates and more than a dozen states across the country. Scientists say they are very optimistic about this news. Its great news, not just for americans, but for folks all around the world, chris. Yeah, these efficacy rates are really incredible. Miguel, thank you so much for that. Let me bring in dr. Jha, dean of Brown Universitys school of health and ashley parker, White House Reporter for the Washington Post and msnbc analyst. Dr. Jha, i want you to help us pick up, first of all, on where miguel left off, this moderna news, coming on the heels of pfizers good news and Anthony Fauci saying this morning that doses could be given to highrisk individuals by the end of december. Then, in addition, we heard from the developers of the Pfizer Vaccine that the bigger impact probably wont happen until next summer. Maybe a real sense of normalcy may not return until next winter. So help us put this positive news into perspective. What does it mean short term and long term, doctor, for all of us . Good morning and thanks for having me on. First of all, it is good news. I mean, i think none of us were expecting a 90 or 95 efficacy, so this is terrific. I love it when im wrong in this way. I was expecting something lower, but well take this. We need some good news. The truth is, if everything goes well, were going to have maybe 20 million doses by the end of december. Thats 10 Million People getting vaccinated. Thats a good start. Thats not going to bring the pandemic to an end. My hope is that we can add another 20 million a month for the next several months, before we get fullscale production. I think things will start turning around for a broader part of the population by february march. Were going to see wide scale vaccination maybe late march, april, may. So people really just need to hold on tight. This is not about to end tomorrow or next week. Were still several months away from widescale vaccination, if everything goes well. There is something to be said for replacing incredible uncertainty with light at the end of the tunnel, for sure. That leads us, a sh lshley, to question about how to maximize the impact that how one or two vaccines can or should have. You and your colleagues have been reporting the president has tuned out from the surge. Hes not going to meetings or reading reports. Operationally, is the Administration Ready to wield the power of the federal government to maximize this opportunity, to get this pandemic under control . Is there anybody in the president s true inner circle who can help make that happen, ashley . Well, two things are going on. As we reported, the president , even before the election, and especially since the election, has abdicated his governing responsibilities, including on monitoring the coronavirus pandemic. It was confirmed by members of the task force that donald trump himself hasnt attended a Task Force Meeting in more than five months. That said, if there is one area of the pandemic that animates the president , it is the vaccine. It is operation warp speed. The president , on the one hand, was frustrated when pfizer first announced its news six days after the election. He wanted it to come in time to help him in the polls. But this is the one area where he is going to be engaged and it is on the vaccine front. Having said that, even if he is engaged, is he going to tell the people who are working now in the administration, look, right now, this minute, you need to get in touch with the people from the Transition Team over on the biden side because we need to make sure that not only are we doing everything we can do, but the very minute that we have a new president , they can pick up and, you know, get running with it . Well, that was the next thing i was going to say. Thats not happening. The president has not officially conceded. He has not begun a formal transition of power, which poses Grave National security risks. The whole reason you have this formalized transition is in the wake of 9 11, there was a sense george w. Bushs administration was not up and running. While we cant predict a future terrorist attack, we do know that we are facing a major domestic security challenge in the form of coronavirus. So we talk to people. Theres people in the administration, in the Trump Administration, who are basically saying, we want to reach out to our counterparts in the biden transition. We want to be helpful, and theyre being active lly told n to start the process yet, which is a real problem. Theyre trying to do as much as they can behind the scenes, without the buyin of the president , of donald trump, taking notes and hoping that as soon as biden takes over, even if this transition starts late, in a way that, frankly, is dangerous, that if theyre doing everything they can, they can still sort of pass off a baton and catch up and make up for this lost time. So, dr. Jha, we have this critical gap, right . Great news we got last week and this morning, but then the reality of proscience administration taking over not until january 20th. Youve been vocal on twitter about your concern for the state of the pandemic right now, wa warning, in part, the gap between how bad things are and how little were doing has never been greater. This morning on the today show, dr. Fauci put it this way, the virus is not going to call a timeout. Meaning, while we wait for the Biden Administration to take over. So whats your greatest concern, and what, in your mind, needs to be done immediately . Yeah. Two and a half months, two months, whenever it is until mr. Biden and his team take over is too long to just sit around and do nothing. The president has walked away. His senior adviser, dr. Atlas, is sort of taking the approach of ill let people get infected and see how it goes. Just the number of people who are going to die, well have at least 100,000 americans die between now and Inauguration Day unless we do something different. What needs to happen is the 2008 model that president bush and president obama did in the middle of a financial crisis. They essentially created a joint task force. The joint task force worked together during the transition to keep the country safe from a financial crisis. We need exactly the same thing right now with a health crisis, but it cant happen unless President Trump is willing to work to protect the American People. Im baffled that he chooses not to. Yeah, that brings us to this reality check, dr. Jha. The reality of this alternate world view, of denying facts, ignoring science. In fact, many of the 73 Million People who voted for donald trump embrace this hoax mentality. I talked to a lot of them on the campaign trail. So if you have that as a fact, can this pandemic be brought under control . Is it too late to change minds . Is it too late to change habits . What can be done, dr. Jha . So i dont think its too late. I think we can we need to demand that our political leaders, whether its the president , senators, members of the house, governors, speak truth to the American People. The bottom line is, you may have thought it was a hoax, but when your family and friends start getting sick and dying, thats not a hoax. And we need to level with the American People about where we are. I think the American People can handle it. Then we have to act in a way thats in accordance with science to protect people. I think thats the responsibility not just of the president but all our political and religious leaders. I want to see others step up,fa so. Ashley parker, thank you for your great and really important reporting on all of this. Dr. Jha, youre going to stay with us. Because we mentioned how some cities are taking action. Starting just this morning, the nations Third Largest city, chicago, is under a stay at home advisory, as cases surge in illinois. City officials are trying to instill a greater sense of urgency about this pandemic by turning to a new strategy. On the ground outreach in neighborhoods that have been hit the hardest. Nbcs cal perry is in chicago for us. Cal, what does this new advisory mean, and how does it fit into this outreach strategy . Reporter so the hope is, look, obviously, the mayor wants folks to stay at home. There is a stay at home advise i. While that doesnt carry the legal weight of some kind of mandatory stay at home, it does encourage people and businesses to allow folks, unless there is an emergency or you have essential travel, the idea is to, obviously, not be going outside. We are in a crisis state here in the city of chicago. The number of hospitalizations has doubled in just the last week. Were talking about a 40 rise in cases. We know that communities of color are impacted more negatively, and that is especially true here in the city of chicago. Latino community here has an equal number of cases as both the white and africanamerican populations of the city and a 30 Positivity Rate. Chris, we are in the community yesterday talking to Community Organizers about some of the challenges they face. Take a listen. A lot of our community is challenged by living in overcrowded conditions and not having insurance. Undocumented immigrants fearful of going to get tested because of what might happen to them. So a lot of those stories are coming out, and were learning as best we can to make sure they get the right information. It doesnt matter what immigration status you have. You can have access to testing. Reporter very Important Message to the community, nothing should hold folks back from getting tested. Situations in the hospital is critical. 85 capacity in the city of chicago. Doctors want to send the word that thanksgiving celebrations should be virtual. Our doctors and nurses, front line workers, for them, itll be another thursday in covid wards across the country, thinks. Cal perry, thank you for that. Dr. Jha, listening to him is another example, and an important one, of the complexities of dealing with this, trying to convince people about what they should be doing to stay safe, especially as we are going into a holiday season. Would you like to see more cities doing what chicago is doing . I dont know, is that only really feasible in a blue area . Whats your thought about what youre seeing in chicago . Yeah, so i think the model that id like the whole country to follow is what governor whitner of michigan did yesterday, last night. Chicago is, i think, a version of that. Look, what we know is the virus is really spread when people gather indoors. Im fine with people heading out to a park. Im fine with people being outsid outside, if they need to be. Whats critical is the indoor gatherings. Thats killing us right now. I want to see more local action. Its very clear were not going to get real federal leadership right now. So i like what the mayor of chicago is doing. I really like what governor whitner last night did in michigan. Just a few hours, president elect joe biden is going to be laying out economic agenda points. Looking for a financial turnaround in the middle of pandemic. His team is pressing ahead with critical planning, despite getting zero cooperation from the current administration. Joining me now, nbcs ali vitali in delaware. Garrett haake is on capitol hill. And Senior Business correspondent and anchor Stephanie Ruhle is with us, as well. Ali, we all heard it on the trail. President elect biden spent a lot of time talking about what he called his build back better plan. Now that hes president elect, what do you think were going to hear today . Will it be anything new or different . Well, look, the biggest, new thing about this, chris, is the fact hes now saying these words as president elect. Certainly, when youre a candidate, policy matters, but now, americans both who voted for biden and who didnt, are going to be hearing this new build back better plan in a new light, because he is doing it from his biggest bull horn ever. You and i know from watching and listening on the campaign trail a little of what he might talk about, which is that we know where the plan is going to go. It involves another massive undertaking of a stimulus. Specifically giving money directly to americans, as well as focusing more on unemployment assistance. We know that those efforts on the hill right now are shaky at best. We havent seen Much Movement in that regard. Biden has spoken to the two Top Democrats in both chambers, so they know thats where hes eventually going to want to put this as he gets into office in january. Really, this is a chance for biden to explain his plan for the first time on an issue, the economy, that he regularly trailed in the polls from President Trump in the campaign. That was an issue where President Trump regularly earned better marks, more trust from the American People on bringing back the economy. This is biden as chance. Not on a debate stage or where waters can be muddied, to explain his plan directly to the American People and build the trust as he explaining Building Back better. I know this is a speech on the economy, but, really, these are remarks that are about the coronavirus. Biden and, frankly, Vice President elect Kamala Harris have been clear the past few months that these two crisis are in in in in inextricably linked. What are members of the biden team telling you . They know they have to speak to the over 70 million americans who did vote for President Trump. When you look at previous elections, people who are doing better off financially traditionally vote for the incumbent president. That wasnt the case. Overwhelmingly, when you look at urban centers, places where people are doing better, they voted for biden. He knows he needs to reach out to rural america, where they need infrastructure, where they need faster wifi, where theyre not thriving economically. Places that dont have enough hospitals right now. To alis point, hes going to take it back to the coronavirus, where you cannot get better economically until you get better from a health perspective. Thatll be the key to all this. Remember, President Trump has been absolutely silent on coronavirus. The last we heard, were turning the corner, moving past this. Absolutely not. People who can work from home, like all of us, may be saving more, but were in the eye of the storm. With more restrictions coming in the near future, which is going to have to be the case because of corona, theyll have to dig deep with an economic stimulus plan. People are going to suffer, especially small business. Lets talk about a stimulus package. Critical component of a recov y recovery. Both houses are returning to congress tomorrow. What are you hearing on progress of a relief bill . The two sides have been stuck at 1 trillion apart for a month and a half or so now. If theres reason for optimism, we believe the Majority Leader Mitch Mcconnell is going to get more involved in the negotiations, replacing mnuchin as the republican side of the negotiating table. It is basically speaking for a president who hasnt been involved or using the bully pulpit in a serious way toward getting to a deal. That said, every time we see the stock market number improve, republicans say, look, the economy is doing enough on its own, using that as an argument for a smaller relief deal. You still have Mitch Mcconnell at a 5 million deal. Pelosi excuse me, 5 billion. Pelosi, 1. 8 trillion. Miles and miles apart here. The republicans also do know theyre more likely to get a deal signed by a president who is of their party than they would be if this waits until january. There is, of course, the real actual human cost of waiting that much longer. Were in the same place as we have been for some time, but we have a mixup in the players involved at the table here that might produce something. Stephanie, the markets like what theyre seeing with the moderna announcement. When we came on the air this segment, the dow was up about 400. It jumped to 450 in the last five minutes. Now its 440. Talk a little bit about what this means for the overall economy and how its viewed by wall street. Its really important right now, we talk all the time, about how the market isnt the economy. This is the perfect example. The stock market is forward looking. The stock market is looking at this mo duderna advancement and saying, in a few months, we could be past this thing. There is huge demand for people to go out and spend again. Thats a positive. At the same time, we have to stay in the present. Look at all of our lives. Were not spending money right now because we cant go out. Many people are without jobs. They could be losing more jobs in the coming months because were not going to see those immediate holiday jobs come back that we were expecting. You have to separate the two. Those that are saying, we dont need stimulus, look at the market with these vaccines coming, well be great in a few months. A few month it is a lifetime away if you run a small business. If youre employed by a small business. Weve got to solve for those issues right now. So that brings us back, ali vitali, to the biden Transition Team and the lack of cooperation from the trump team and what this might mean for moving forward to get this coronavirus under control. To be able to move forward the way they need to in various cabinet positions. Lets start with the coronavirus aspect of this. Is there any sense within team biden that things may be moving in their direction in terms of cooperation from the white house . Well, it cant until they get the letter of ascertainment from the General Services commission. Its stunning, this one tiny piece of work flow in government bureaucracy has become a national deal. Though there is an administration in waiting, theyll still be waiting on the cooperation front until the letter is signed, sealed, and delivered. It is making a lot of roadblocks here. Theyre still pushing ahead. At the same time, theyre really pushing uphill because they cant have conversations with people that are already within these agencies, who may be planning a potential vaccine dissemination strategy that will start in the early part of next year. You know, theres the saying that if youre not at the table, youre on the menu. So for these folks, they are not having any input in what the menu looks like. Theyre going to be the ones running the logistical operation of a potential vaccination dissemination, as well as the other pieces of that. I would say, this comes against the backdrop where were getting a better sense of who the personnel within this transition are. Of course, were waiting on cabinet announcements. Were likely to start seeing those around thanksgiving. It could be possible we will be getting other senior staffing announcements for what the internal makeup of the west wing and other key positions are going to look like. That could come in the next few days or weeks. Were really looking at that. But were also seeing this group in waiting leaning in on the idea that they promised early on that this administration would look like america, that it would be reflective in its makeup. What were seeing from the transition sort of bears that out. Yesterday, i saw some new diversity data of the transition. It shows that this is a majority female staff, and that people of color make up more than 40 of this current operation. Thats something theyre promising to push forward into the next administration. Something to keep an eye on for us as we start looking at senior positions starting to get filled, as well as who makes up this cabinet. This is a group of people that are really keeping reflectiveness and diversity top of mind, chris. Yeah, there are a number of positions that have never been filled, for example, by a woman. At this very moment, hospitals are nearing capacity, with just 20 of the states icu beds still available. I want to bring in the doctor sounding that warning, dr. Dan johnson, division chief of Critical Care at the university of nebraska medical center. Thank you for being with us. Obviously, thank you for all you do, dr. Johnson. Look, your state is in a world of hurt. The fifth worst outbreak in the country. You wrote a really passionate post on facebook, and i want to read part of it. It says, the doubling time for covid19 hospitalizations at omaha is currently 21 days. We currently have 372 patients in the hospital. This doubling time means that in three weeks, there will be 744 Covid Patients needing the hospital. In six weeks, there will be 1,488 covid19 patients needing the hospital. Bottom line, when you look at these numbers, doctor, is there any way you could possibly handle them . Well, if we get to that number of hospitalized patients, theres no possible way the hospitals at omaha could handle it. And thats partly why my colleagues and i tried to sound the alarm a little over a month ago. We took to traditional media and to social media to try to make sure people in nebraska understand the gravity of the situation. So talk about your message. Because, obviously, the numbers keep going up, and it is not just nebraska. Its all over the country. What do you think people arent getting . I think that people have grown tired of covid19. Theyve let their guard down. Honestly, the number one message right now is people need to avoid being indoors with people who arent in their own household. There is this false concept of being in a bubble, where multiple families can be getting together indoors, having parties, having dinners. That time has passed. We need everyone to stay at home with their own household and not be gathering in any capacity. Yeah, this, i only see my best friends kind of thing that maybe we saw in the summer when people were outdoors cant be the case when people have to be indoors. In places like new york and nebraska where its cold. Yo back in the spring, doctor, when new york, where i live, was the epicenter of the crisis, we saw doctors and nurses from all over the country literally flocking to new york to lend a hand. How worried are you, that your state could run out of space and medical staff, and because the virus is spreading so much everywhere in the country, that extra help just wouldnt be there for you . Extremely worried about that. There are no Health Care Workers in other parts of the country to come help us because every part of the country is now surging and spiking. Unlike in the spring, when new york was really hit so hard. What people need to realize is, if we run out of beds, we run out of nurses, respiratory therapists, its not just for covid19. Its for heart attacks. Its for strokes. Its for car accidents. So imagine one of your family members needs medical care and cant get into a hospital because we filled all the beds with covid19 patients. Thats a really bad situation. Its a devastating situation. Obviously, its one that you and your colleagues are increasingly dealing with day after day after day. Difficult decisions, Difficult Conversations that have to be had by telephone with family members. How are you doing . How are your colleagues doing . Right now, were holding up okay. Right now, we have a scenario where we wouldnt be able to deliver medical care because we dont have the facilities or the staff. Mentally, physically, i feel that omaha Health Care Workers are holding up. Its most difficult for nurses and respiratory therapists. I want to emphasize the physicians have it easier than the nurses and the respiratory therapists. But, you know, we can get to a scenario where we have people outside the Emergency Departments and we are not able to get them in. That is what we are most worried about. I was talking to dr. Jha at the top of the show. We were talking about this period between. There is this great news about moderna. There was previously last week great news about fidpfizer. The efficacy rates, early trials of their vaccines are much higher than, i think, a lot of people expected. As somebody on the front lines of this crisis, how encouraged are you by this news, but how worried are you about this period between now and when we actually see widespread vaccinations . Any time we have a vaccine well studied, appropriately tested, and it comes out it works well, its excellent news. Vaccines only work if people take them. I want to comment on, i hear people saying that they wont take a vaccine, even if it is approved. If a vaccine makes it through the rigorous process, and the fda says its safe and it works, people need to take it. If they want to put covid19 in the Rearview Mirror anytime soon, a vaccine a critical component of that. Again, thats months away. We need people to take action right now and stop gathering. We need masks to be worn, and we need people to stop gathering. We cant say that enough. Dr. Dan johnson, thanks to you and your colleagues. We really appreciate you taking the time to speak with us. Potentially attending superspreader events. Some schools simply arent taking any chances. Theyre just going remote after thanksgiving break. Local media reports thats the case for Cleveland State university, seton hall university, schools in the State University of new york system, as well. Nbcs blayne alexander is in atlanta. Blayne, tell us more, what youre learning about these hard decisions that schools are making ahead of the holiday weekend. This is a big concern, both for Health Officials across the country and School Officials we spoke to. Remember, a few months ago, we were seeing College Students back by the hundreds in some cases, attending bars and parties, leading to covid outbreaks. Now the holidays are coming up, there are concerns among Health Officials and School Officials about hundreds of thousands of students essentially fanning out across the country, going back to their communities and families. So a number of schools are actually urging their students to stay on campus during the break. Some schools like cornell and others are making provisions for students to do so, saying you can stay here during thanksgiving and the holidays. For students choosing to go home, a number of schools are offering exit system, allowing them to get a covid test before getting on the road or plane to get back home. Some schools are imposing strict guidelines. For example, a negative test is required before leaving campus. Other schools are saying if you choose not to take this test before leaving campus, you cannot be enrolled in the spring. Harsh guidelines there. One official we spoke so says, yes, even though you may take a test, thats not enough. Once they get home, as you suggested, create this bubble. Create a small area that you operate in over the holidays. Of course, you never know where people have been. The smaller number of people you interact with, the better. Several other of the most populous counties, they wrapped up early over the weekend. So far, officials here tells us theyre not hearing of any problems, not seeing any evidence of fraud or other issues with the original count.
