Green light needed this morning in order for you to get it. Now the race is on to make it, pack it, ship it, and administer it. But tonight theres a problem we need to get out in front of in this country, and it starts with each and every one of us. Having confidence in the vaccine our government says is safe. Anyone can be forgiven for discrepant schism. The president s rhetoric has contributed to it, especially in communities that need this vaccine the most. The africanamerican community, were not trusting because we feel were the first group they want to test it on. We dont know if its going to be acceptable to everybodys body, would everybody immune system be able to take it. We dont want to be guinea pigs, test dummies, anything like that. Our Community Still carry tuskegee experiment syndrome. Not only do they not trust the Science Behind it, they dont trust the government. Heres more proof. This chart shows how black and Latin Americans say they are much less likely to get the vaccine. Nonetheless, the rollout is under way. Trucks are being packed with the lifesaving viles as we speak. They will head out tomorrow. And then from there theyre going to the east, some to the west. Our key focus tonight, how to bridge that divide in skepticism because the vaccine is worthless if not everyone takes it. Lets hash it out. Dr. Susan lopez, assistants professor at Rush University medical center. Dr. Vin gupta is a Critical Care pulmonologist and affiliate assistant professor at the institute for health and metrics and evaluation in seattle. Hes also an msnbc medical creditor. And Francis Dietz setters, Health Reporter at the washington post. Dr. Gupta, lets talk about the basics of this vaccine for a moment. The cdc just published a list of recommendations and guidelines for who can take this. Among them, it encourages pregnant women to check with their doctors, warns those with allergies to other vaccines, to avoid it altogether. I wonder when you make of the guidelines and how concerned you are about possible side effects. Good evening, alicia. This is what ill say. The data that we do have is what we should focus on. We have a pfizer phase 3 latestage trial that shows that amongst 40,000 individuals this was a broadly welltolerated vaccine. Those are two really good metrics there. There is some concern that if you had a prior very serious allergic event, technically called an anaphylactic reaction, that means you developed a skin rash, maybe shortness of breath, something along those lines, your throat swells up in really severe cases, those are exceedingly rare to foods and medicines. In that case, that needs to be a conversation with ones provider to see when and where the individual should get a vaccine. Theres going to be alternative platforms coming online, so it may not be the Pfizer Vaccine they get necessarily. Theres a lot of enthusiasm given the efficacy of this vaccine, proven efficacy. Thats why youll see a lot of clinicians messaging on this getting it. Im hoping thats going to build confidence once other people start to qualify and once we have more different types of Vaccines Available in the marketplace. Dr. Lopez, you wrote this week about how this pandemic is uniquely difficult for black doctors, for latinx doctors like yourself. What are the additional challenges that you are up against as a Health Care Provider . Well, i think in general m t most i think because we understand these communities, we also understand the history of the mistrust. Earlier there was a discussion about the tuskegee experiments, but it impose beyond that. A lot of these things things are historical. We understand in order to regain that trust, thats going to have to be to be hardearned, were going to have to partner with trusted partners within the community, communitybased organizations, houses of faith, in order to gain that trust in order to try and win people over and try and get people to take this vaccine. Dr. Lopez, i have to ask, when you were with the communities you serve, are you hearing resistance to this vaccine . I do. I do hear resistance. I have colleagues who work in the community. I specifically only work within the hospital setting. But i do hear resistance, especially concerns about whether or not someone can get the virus from this vaccine. And the reality is you cant get the virus from this vaccine. Francis, we know frontline workers, people in Nursing Homes are going to be the first in line for the vaccine. Who, then, comes next . Essential workers. This group of essential workers, which could include many people from communities of color, thats the hope. Were talking about people who drive the buses, who do serve in restaurants, do a lot of the work that needs to be done to keep the country functioning. But one of the things were hearing now is theres been this remarkable achievement to get the vaccine 10 billion investigated in this extraordinary achievement of getting it within a year. But theres a mantra in public health, and it is vaccines dont save lives, vaccination does. Thats process were entering now where theres enormous logistical challenges lying ahead and enormous challenge of persuading people, not just people in communities of color, but many people across the whole spectrum in the United States that this vaccine is what they want to take. In that context, the initial Health Care Workers who take the vaccine in the next few days are going to be acting as role model for the rest of the country to watch and potentially follow. But we need to get to 70 or more of people taking this vaccine. Have we seen anything that it 70 people have rallied around, thats the question. Fair. Dr. Gupta, to that point, you need 70 of americans to take the vaccine for it to work. You have americans who are nervous about it because of the history of racism in medical and scientific testing in this country. You have americans who are nervous about this because of the politicalization weve seen around this vaccine. How do officials and influencers combat those some very legitimate concerns and ensure americans that they can trust this vaccine . Well, alicia, a key piece here, i dont like that we have to ration vaccine supply, but one of the potential Silver Linings here, as francis said, im getting the vaccine on wednesday, as are my colleagues in icus across the country. Theres going to be a time for people to look at that, influencers at the national level, at the local level, and see them leading by example, seeing what theyre experience is, what happens after that first and second doses four years ago later. That, i hope, is going to be a confidencebuilding measure. Youre right, were not going to get to 70 right away. Its going to require that trust. But right now were looking at the same set of phase 3 trials from moderna and from pfizer. Theres only so much we can look at that data. We need to draw from experience now and from anecdotes, thats going to get us to that goal of 70 before long because people want normalcy, and this is the quickest way to normalcy. Dr. Lopez, you were the one on the front lines. What are you seeing in the hospital setting that the rest of us are not . Its a lot of adjustments with the first wave. I think we all adjusted as quickly as we could. We learned as much as we could in the first wave in terms of stopping elective procedures in order to be able to prepare to take care of those patients. We learned some lessons. Weve learned how to continue to expand the hospital as much as we can. We have what we call surge planning. But i think right now what were learning, what were experiencing is with that first wave a lot of people afraid to come to the hospital. And now the reality is that we still have our usual patients coming into the hospital. Were, of course, treating everyone that comes into the hospital, but on top of that, in addition to that, were also taking care of Covid Patients on our covid floors. So it definitely were doing the best that we can, but i think with the help of everyone in the public and the continued messaging between using masks well until vaccines can be available, are widely available, i think thats going to help us out in the long run so we dont burn out our health care system. Right, which is something weve been talking so much about. Dr. Gupta, how concerned are you that this vaccine news is going to make americans overconfident . Why snow not the time to let up on everything you and i have been talking about for months, the masking, the social distancing, the proper hygiene . Well, alicia, in the short term, vaccine is not going to get us out of our current predicament. We dont have enough vaccine quick enough to vaccinate 70 of america. So we still have the next three months ahead. Respiratory viruses likes covid like cold, dry air. It has dire predictions. Frankly, the reality is even slightly worse than what we predicted, and its not going to get better. What im seeing in Critical Care units across the country from colleagues firsthand is shortages of adequately trained staff to care for patients that need that type of service. So thats number one. Number two is, once you get the vaccine, its not like the next day you dont have to mask and socially distance. We still dont know yet if this vaccine only prevents severe illness or does it actually also prevent the transmission of the virus. We dont know that yet. Were going to know it soon, but not yet. Francis, what are the greatest concerns officials face when it comes to supply chain and Raw Materials issue with this vaccine . Yeah. Well, this is an enormously complex undertaking thats beginning right away now. Theres the process of cold storage. The Pfizer Vaccine is very finicky in how it can be handled. Theres a whole new storage facility. Many other hospitals have had to bring in, you know, individual freezers to keep this vaccine. But because its a twodose vaccine, it gets a lot more complicated. Its very important that we have these priorities. But if somebody doesnt come back for their second dose, do you wait or give the second dose as a first dose to somebody else. There has to be flexibility, and thats going play out all along. I wrote recently about the 2009 rollout of the h1n1 vaccine, which was a bit over ten years ago. And there the manufacturing slowed down. There were hitches and people lost confidence because clinics were set up and then they had to be closed down because there wasnt vaccine available. This is a far greater challenge. We would be affordable to think everything is going to align straight away. We have to have confidence that it will eventually, but there are going to be hiccups almost certainly. All right. Dr. Lopez, dr. Gupta, and francis, thank you all. Doctor, thank you so much for being with us. You reportedlily were fired before for talking about the lack of icu beds in arizona. I wonder, first, how are you holding up . Im good. Theres been a lot of uncertainty here, but ive had a huge outpouring of support here in arizona, which has really been amazing. But we are in a war right now, and were not acting as such, and people are dying as a result. When you say that we are in a war, can you pull back the curtain for me, paint a picture of what it is that you were seeing at work every day . Yeah. So what were seeing is a virus that is infecting people of our state, our country. These patients are presenting to the e. R. Hypoxic, unable to breathe, theyre being put on bipap, ventilators as well. I think the other aspect thats not really being appreciated of this is that the war is being waged in the hospitals. Its not being waged on the streets. Also, you can go into the hospital to treat a patient and have that trojan horse and bring it home to your family and infect them as well. Its devastating. The hospital responded and said what happened with our firing was a misunderstanding and you were scheduled to work this weekend. Your response . Yeah, it was not a misunderstanding at all. What happened is i was on a shift. I was unable to transfer any of my icu patients out. The next day i was told that i was unable to return back to work. I was very quiet for this week, i didnt say anything. I let the negotiations go on and they refused to let me back in the hospital to treat the patients here who needed me. It was only when i took to social media and the pressure had came did they say i was allowed back to work on the front lines. And i want to be very clear about something that you know. Ive had such great support here in the community. Ive had people in my neighborhood drop off bottles of champagne and thank you cards. I had my doctors and nurses reach out as well thanking me for speaking up. And i think one thing is clear, is that we need to hear the voices of frontline providers, and we cannot silence their voices during a deadly pandemic. Right. And part of that, i mean, for you as a frontline provider, give us a sense what did you see on an average day at the hospital in yuma that led you to speak out . Yes. What was happening is that we were seeing more and more patients who were extremely short of breath, extremely hypoxic, and just unable to breathe. And im talking about people of all ages, 20yearolds, 30yearolds, 40yearolds. And so what ended up happening is that patients began overwhelming the hospital system. And so they began backing up in the e. R. And so these are people here in the community who want to know the truth of whats actually happening. We have patients who were waiting on a liver transplant who got covid and were unable to get their transplant. This virus is real. We had 150 cases in our hospital who are hypoxic. On top of that, we have a prison outbreak here of people in the prison. Someone on the front lines, what is your biggest concern as we head into this second round of holidays . Yeah. Its very clear that we need a harsh we need a harsh approach to the pandemic. We need to shelter in place, we need to wear masks, we need to avoid large indoor gatherings. And please do not travel. We do not have the icu beds to take care of you. And i think were hearing the same thing all across the u. S. Were running out of capacity. What are those of us who are not every day walking into a hospital who are not providing the care you are providing, what is it that we are missing about this virus . What is it that we are missing about this pandemic that we cannot possibly understand because we are not confronting it every day the way that you are . Yeah. Just how unpredictable the virus is. Ive already lost three colleagues. I lost a 27yearold cousin who was healthy and training for the nfl. This virus is very unpredictable and it spreads through households very, very quickly. You know, im really happy because im going to be returning back to work here to actually provide care to people of yuma, arizona, and im looking forward to that. All right. The center on budget and policy priorities reports renters who lost employment income because of the pandemic are more than twice as likely to say their theyre not caught up on rent, nor do they have confidence they can pay next months rent. Our nbc station in charlottesville, north carolina, spoke with a couple who secured housing last year. They say theyre limited to what they can pay, but willing to compromise any way possible to avoid eviction. I can make payments. You understand what im saying . Because im not going back on the streets no more. Reporter but the payments are only possible if the landlord actually agrees to it. What hes really saying is that it doesnt matter how much i pay him. He said up to january when the eviction thing is over. And he still can put us out. With me now, Julian Castro, former secretary of housing and urban development. Secretary, good to see you. What do you Want Congress to do when it comes to the Eviction Moratorium . What can immediately help people like the couple we just heard from who had experienced homelessness secure housing and are now on the brink of losing it again . Alicia, you point out and the woman in the interview tells a story that is very common right now in the United States. We have millions of people who are on the brink of eviction. In some states evictions are already being filed. As soon as that moratorium is done, thats it. People are going to get thrown out. Theyre going to get kicked to the curb. Folks are going to be homeless or troubling up. Theyll be scrambling. What we need to do is the Senate Republicans need to pass the h. E. R. O. E. S. Act or a version of that that puts significant investment in direct rental assistance, and also extend the moratorium on people who are renting and provides protection to people who are on a mortgage so people can have a safe, decent, affordable place to live right now. On top of that, we need to ensure that we do all of the other things wee that wethat we there, unemployment insurance, providing aid to local and state governments that are at the breaking point right now. And the problem is that Mitch Mcconnell and his buddies in the senate are so disconnected from the everyday lives of people, whether those americans are republicans, democrats, independents, in red states, blue states. Mitch mcconnell is so disconnected from everyday people that he feels no impetus, just go through holiday season, go into the new year, let people fall off this cliff, and he the country want to budge. We need to change that. To say nothing of the delay where even if theyre able to come to an agreement, you and i know theres going to be a delay in actually getting relief to these people. Weve almost come to the point where theres going to be some gap regardless of how this gets rolled out. I want to play you sound from congressman Pramila Jayapal on covid relief. Take a listen. I dont call them stimulus checks, i call them survival checks. Families are getting kicked out of their homes, they cant put food on the table. And they need money to be able to do that. This is a bipartisan issue because families are struggling. And Mitch Mcconnell and the Trump White House is refusing to acknowledge the scale of the crisis we face. I wonder how you think about those checks, secretary, right . Because in addition to what we saw from the congresswoman, austin said theres going to be a need for stimulus. But we cant even have a conversation about stimulus until you both get this virus under control and begin to stabilize this economic situation. How does Congress Need to be thinking about these checks . They need to very much think of these in terms of survival of people so that folks are able to just get by. Ill give you a good, precise example of this, alicia. We have 12 million folks out there who on average are behind in rent by 5,800. So even if you said, okay, you cant get evicted for the next month and youre going to be able to stay in place, as soon as the Eviction Moratorium runs out, it could run out at the end of the year if Senate Republicans dont do anything, right away youre going to have landlords across america saying, okay, where is my 5,800 . And folks obviously dont have that money. Theyre going to get sent to court and theyre going to be out on the street. So we need to be thinking about this holistically. We need to be thinking about it in terms of providing a means of survival for families right now. And then easing families and our overall economy into a time in 2021 when the vaccine has already taken hold, where we dont have to, you know, worry about people getting infected, when businesses are more up and running and providing jobs again. But until then, this is not a luxury. It is a necessity. It is a dire emergency, and they need to summon some of that will they were able to summon for the c. A. R. E. S. Act about eight months ago. I do want to ask you about your recent trip to georgia to campaign for the democratic candidates. You did a tiktok with jon ossoff. Tell me about what you heard from voters down there. I talked to Small Business owners down there and they told a consistent story, which was that this pandemic has rocked their ability to do business. One of them said that their business was only at about 50 of what it used to be. Another one said 70 . One of them said they used to have four employees. Now it was just them and one Family Member that were working the business. I worked to folks who were worried about getting evict. There is a real concern out there. The reason i went to georgia is because Mitch Mcconnell has shown us that hes so disconnected from Everyday Americans that if we continue to give him the keys to the senate by keeping perdue and loeffler there, nothing is going to get done, even in the next administration, to help people who need it on rent, on unemployment insurance, on just the things to get by. People need to get out to vote in georgia. We have two fantastic candidates in reverend warnock and jon ossoff. It looks good. Theres a lot of enthusiasm because people know that the stakes are so high. All right. Secretary Julian Castro, thank you, as always. On tuesday, 14 leaders at the Fort Hood Military base were fired or suspended, including general officers. An army report found they created a, quote, permissive environment for Sexual Assault and sexual harassment. Its an acknowledgment that came at the cost of heartbreaking loss, the brutal murder of Army Specialist Vanessa Guillen. Guillen went missing from the base in april. Her remains were found two months later. A fellow soldier who later killed himself is believed to be involved in her death. Guillens family alleged the soldier sexually harassed her. The army conducted a sweeping independent review and the results tonight are devastating. More than 600 individual interviews were conducted. 503 of them with female soldiers. The panel found 93 credible counts of Sexual Assault, but only 59 were reported. The Committee Identified 135 credible instances of sexual harassment. Only 72 of them were reported. Quote, some of the accounts of unreported Sexual Assault were extremely serious and had Significant Impact on the Victims Health and wellbeing. Why . The panel found most of it boiled down to fear of retaliation and zero confidence in confidentiality. As one panel member put it, one of the things they needed was to be believed. So how do you change it . The panel that uncovered all this issued 70 recommendations to change the culture at fort hood. Secretary of the army, ryan mccarthy, accepted them, creating whats now called a peoplefirst task force. It seeks to promote change to staffing, structure, and implementation of the Sharp Program at fort hood, possibly beyond, to address deeply dysfunctional norms to weed out harmonious. Talk is talk, words on paper is still words on paper. Fort hoods commander did this week. Everyone in the army is responsible for the culture, but commanders are responsible for the climate. And so i can tell you, i told the chief of staff, general mcconville, i own this. We initiated a plan to rebuild and maintain the trust between the leader and the lead, because thats the bedrock of our profession. And its what the American People expect. Of their army. That brings us to the real task, not letting up on the accountability, not just in the army, but in congress. Theres a push on capitol hill, the eye of Vanessa Guillen act that legally mandates all allegations of military Sexual Assault and harassment be reported to and investigated by a third party. How has your organization responded to the unprecedented need and challenges that have been brought on by this pandemic . Thanks for having me, alicia. And as you said, i really cant stress this enough. We have a hunger crisis in america today. 26 million americans reported they didnt have enough to eat last week. And world central kitchen, our nonprofit, is seeing this firsthand in cities all across the country. Long lines of families waiting to get food. And really this has been going on since march. And its only going to get worse as we enter the next phase of this crisis. And something that we have heard over and over again and weve spoken with people who have had these experiences, you have people who used to be in the position where they were contributing to their local food bank, where they were able to help others in their community. Theyre now turning in need to those same food banks. Can you talk to us a little bit about your sense of how deep and widespread this crisis really is . Absolutely. I mean, as you said, since march food banks and nonprofits like world central kitchen have really been carrying the weight of this crisis. World central kippen ekitchen a served 35 million fresh meals and put 135 million back into the hands of small local restaurants to produce meals for families. And this is all being done with donations. You know, nonprofits are stretched incredibly thin right now. And simply are running out of money to keep food going out the door to families. And really now is the time when more than ever we need our government to step up. You know, were entering the holiday season. The crisis is going to get worse before it gets better. And really steps need to be taken immediately to begin to address this. And i have to tell you, i have heard that echoed by people who lead food kitchens across this country, right . That philanthropic giving will not be enough to get us through this crisis. So when you talk about lawmakers seeing us through to the other side, what does that really look like . So we know that congress right now is working on a very slimmeddown stimulus package. Theyre trying to get it negotiated through. Its not going to be enough. Maybe it will help a little bit as a bridge until we get to next year, but we know its not going to be enough. But even if congress cant agree on a stimulus package next week there is something that can be done right now. The u. S. Has since march had a federal disaster declaration all across the country which unlocks existing funds, not new dollars, for things like food. This is the biggest disaster our country has faced. But fema has been absent. So weve helped introduce legislation. Its already passed the house now three times. Which tells fema using existing dollars, no new money, to work with states and local governments to feed the hungry through restaurants and nonprofits. So we see this as a really smart solution. Its one of many. This is going to be this is going to require a multiprong approach. Were going to have to fund schools. Were going to have to fund food banks. Were going to have to increase snap. But this is something that can be done right now even if we cant get a stimulus bill through next week. Right. Because part of what has been laid bare throughout this crisis is just how many families are this close to the brink. What do you want the lessons of this pandemic then to be . Food needs to be at the center of the conversation. I think this pandemic has been a wakeup call that our schools not only educate our students and our children but also feed them and that our food system is broken. When you have farmers that are plowing under crops and you have lines at food banks that go miles and miles long, you have a disconnect. So we need to make food a priority across all levels of government. And hopefully learn from the lessons from this pandemic and start to change things for the future so we can prioritize. Because again, its the most vulnerable communities that really are hit the hardest in times of disaster. The food deserts, lowerincome communities, and theyre most at risk right now.
