And also in different parts of the world in Uganda and Pakistan and when I'm working as a clinician as a family doctor basically I'm seeing patients in the hospital in the clinic in their homes sometimes in nursing homes I delivered babies for 25 years close to a 1000 babies and I loved every moment of that and I also really relished the opportunity to make house calls to get involved with families and communities and to understand how people live because how they live affects their health the more I understood my patients' lives their contacts their family the better job I could do as their physician and I often told my patients my job as your family doctor is to try to help you stay out of the hospital and to stay as healthy as possible. And I think that was probably more education than anything that you had to do with your patients down that's right and so many people are facing health challenges that they weren't expecting we know that patients can have accidents or develop chronic diseases or unexpected things can happen to people at any moment and so family doctors are really on the front line delivering primary care at the community level where things are happening and we are trained to take care of as much as possible that we can at the community level but also to refer patients to subspecialists when needed. And so we know our limitations what we can do what we can't do and how to help guide our patients to the resources that will serve them best either at the community level or in the health system. You know you mentioned that you have worked out of the country here. How did you have a language barrier to speak a lot of languages are Was that how do they work that that's something I've always wondered Well unfortunately I don't speak many other languages but I always chose to work in places where medical education was delivered in English and where there wouldn't be such a great language barrier I'm also quite comfortable working with interpreters when I was working in karate Pakistan for example I sometimes had to work with 3 different interpreters because the patients were speaking tribal languages that were very unusual and so we would have to go from one language to another of course you lose a lot of the nuance of communication when you're working with an interpreter but you can also still convey to the patient through non-verbal means that you care for them that you respect them and that you're giving them your most attention that's awesome the school has got you here I have to tell you I. Think that doctors are I mean they keep us alive they keep us going so well we like to work as part of a teen and so that's the other thing that I'll emphasize is that family doctors are really team players we work with our primary care colleagues nurses medical assistance their post social workers pharmacists they are all so important as part of the team and when we're working together in a coordinated fashion the patients will benefit you know I think fast fast awesome I would have loved to have gotten into the medical field like my sister but I don't think that I could do what you guys do so. I enjoy I've come across some really nice doctors that I've worked with over the. Can you share with our listeners you know the changes that you've seen in the medical field from the time you started I guess there's so much new technology that the education you've had to work hard to stay on top of or is it little by little that it comes out to you with the new technology so in family medicine like most other specialties we're required to continue our education really throughout our lifetime so we have requirements for continuing medical education for board recertify cation we have to take a board exam every 10 years to maintain our certification and so it really is constant study one of the reasons I chose to go into academic medicine and focus on teaching is I felt it would help me to become a better doctor and I continue to learn new things from my trainees from the resident physicians and from the students every day and so this is part of the joy of being in medicine is that it's you never get bored you never can stagnate you have to continue to keep up with new developments. Of course when I started my training. In the 1980 s. We didn't have the Internet we didn't have tools at our fingertips that we have now so I feel so fortunate that today we have literally in our smart phones tools that help us to keep up to date and there are literally of there's literally a program called up to date that if I'm confronted with a challenge that I haven't read up on for a while I can quickly check that reference and get the latest recommendations to determine whether or not they apply to my patient and so we now have much more effective tools at our fingertips to help us keep up to date but of course you don't know what you don't know you have to ask the right questions and keep an open mind and a curious mind to really understand the challenges that your patient is facing must be something with fast girls from Indiana because I've worked with a lot of young people and I think I learned as much from them as they do from made . Up that Midwest humility I mean. And we know that that's part of our life journey there are continuing to learn before it was. Absolutely my parents and their main eighty's and still active and probably get out work most of us so that work ethic is important very important to us can you share a little bit about what that direction that you that you see in the health care I know there's been so much talk both ways about the Obamacare and you know how do you feel about everybody having coverage insurance being able to go see a doctor well in order for people to be healthy it's important that they have the fundamentals of what contributes to health. If a safe place to live good healthy food a place to exercise loving relationships and good social support we know these are the fundamentals of health but also access to health care is important primary health care where people 1st go when they're sick and receive preventive care and also management of chronic conditions or acute problems primary care is very important but in the United States we have a shortage of primary care physicians and clinicians such as nurse practitioners there are many rural counties in the United States that simply don't have enough primary care clinicians there are even urban under-served areas such as our practice located in Santa Ana where many patients do not have access to primary health care. Why do we have these challenges we are actually not training enough primary care clinicians there's a shortage of family doctors as well as primary general internists and pediatricians So we are having problems attracting primary care physicians into training and actually in working in communities where they're needed most so we have many medically underserved health professional short each areas in the United States where people don't have enough access to care. You are so right that affordable health care insurance is really important we know that people who don't have insurance are less likely to seek preventive care they are more likely to end up in the emergency department with more advanced conditions. We feel it's important for everyone to have health insurance coverage so the Affordable Care Act Obamacare I was delighted because 20000000 more Americans were able to gain health insurance coverage it's not a perfect program it still has problems but we hope that we can build on the success of the Affordable Care Act How do you feel that Medicare for all I mean I feel like for me I am on Medicare and I feel like the younger generation needs to be healthy they are contributing to our economy absolutely I'm a strong advocate for Medicare for all in my opinion I'm delighted that seniors have access to Medicare and also that children under the age of 18 are by and large throughout the country insured through the Children's Health Insurance Program the chip program but what about the people between 18 to 65 this is our workforce they should be covered with health insurance Rijkaard lists of their employment status in a country that says a fan says the United States one of the wealthiest countries in the world we're one of the few wealthy countries that does not insure health insurance coverage for all people. In my opinion shame on us we need to fix that problem sounds like the commonsense strategy here is not very common. So it's you're a little bit out you mentioned earlier before you started talking something about what matters and why and what step you hadn't mentioned something about speaking can you share that with listeners so earlier this year I had the chance to be interviewed. Regarding what matters to me and why a program here is u.c.i. And I shared with that group that health equity the chance for everyone to be healthy to have opportunities to be healthy has been a major driver of my career it's why I decided to become a family doctor because I felt like everyone deserves access to health care. As well as why I've chosen to work in areas of great need medically underserved areas where many people don't have access to health care I went to Uganda early in my career when one out of 3 children was dying before the age of 5 that was it broke my heart I had my own children at the time and I thought why should children be dying for lack of very simple basic things and so I learned so much about the importance of community in supporting health and that for health equity for everyone to have a chance to be healthy it's not just about medical care it's about clean water sanitation housing food education all of those things are important to promote the best chances of the population to become healthy I think like my sister. I says you know better you do better and we do know better so we should be doing better that's right we do. Know that's pretty well about I know you guys have got an anniversary coming up here can you share a little bit exciting news about fat yes I am thrilled that the University of California Irvine Department of Family Medicine is celebrating its 50th anniversary this year we were one of the 1st 15 programs to train family doctors in the United States we were established in 1969 we had a partnership with Hoag at the time and we worked closely together to establish the training for family doctors at that time many physicians were G.P.'s generalist physicians who would complete medical school and then go out and start practicing but as medicine became more complex. The profession realized that we needed a more in-depth 3 year training program for physicians to become family doctors to to be able to take care of patients from womb to tomb from children to the elderly and so we were one of those 1st programs we've been going strong ever since we've trained over 400 family doctors that are working in California and many other locations and we're going to be celebrating that weekend of October 26th that's exciting very exciting so now I understand that I know it's a long term and some time out but I understand they're either planning a new hospital in Orange County or. Yes So u.c.i. Is as the only academic Health Center in Orange County we feel a responsibility to provide access to high quality health care for everyone in the County of course we have our main medical center now in orange but we have exciting plans to build a new hospital in Irvine a new children's health center new primary care sites really from all the way in the south to the north of Warren County we would like everyone in our county to be able to easily access a u.c.i. Primary care physician who could provide the highest quality of care for them and their families so the medical center has come a long way in 50 years right over the last the medical center has expanded it's changed of course we used to be a county hospital you see I bought that hospital and now we provide everything State of the are from very basic primary care to very super sub specialized Cancer Care transplants heart surgery you name it you. I can provide it that's amazing is there anything that we've left out here that you think might be kind of good to go over that you think that the community might benefit from. Well I'd like to remind listeners that when you stop to think about it health is our most precious resource our health and our lives is so fundamental to everything else we do and if we're not healthy we can't work we can't study we can't be a loving partner and so health is fundamental we think everyone should deserve a chance to be healthy and have their basic health needs provided and so we're glad to be part of that solution we also realize that we need partnerships throughout the county throughout the community because we can't do it alone health is not something that's delivered by doctors it's something that's developed in the community with many different partners working together and I'm so glad that you have this program and listeners who might be interested in what they could do to promote the health of Orange County and you brought to thought to my mind something that I always like to say is you have to be good to your bodies because where else can you live that's right and if you don't take care of it who else will absolutely absolutely there's so much out there I have to say this is been a really exciting information and I know listeners will benefit up a lot but is there any kind of either contact website anything that listeners can go to to maybe learn about your programs will so u.c.i. Perfect perfect could use your schooling where where did you go to school with a bachelor's in social working out there back in Buffalo New York where I'm from Buffalo State College and then I had my master's in social work at Long Beach State . And then my doctorate I got from the Southern California versity for professional studies and how long have you been in Irvine I've been in Irvine for 10 years do you I can hear. Me too I think it's probably about the best of the best yet I understand that you're very into the cognitive behavior therapy could you explain to that for people like me exactly what that is about sure sure tell me of us My specialty is kind of behavioral therapy sometimes you'll hear be called the terminal c c b t just for short just so much you know so that we're cognitive just means your thought process so the theory behind the kind of people therapy is that the way that we think creates their moods which affects our behaviors and our physical reactions so most people live their life living off of their moods meaning depending on how you feel determines what you do and that's what most people really don't get well because they just do the behavior which makes them feel better but it's like a Band-Aid because they always go back to whatever negative mood whether is depression anxiety if they have obsessive compulsive disorder. Panic attacks you know numerous different things so would I teach people is instead of just living off of your moods is to figure out why you feel the way you do and that's based on your thoughts and then once we figure out what you're thinking we want to challenge your thoughts to change them which in turn will change how you feel and your behavior in your physical reactions physical reactions means like if your sleep disrupted appetite changes difficulty concentrating memory crying a lot those are physical reactions and the one last element which is really important is your environment that you're in so I can be work home school and you really need to assess if that's part of the problem it's not always just you it can be the environment that you're in that you need to change as well in all the years of of these studies too they see that people are getting. Better Are they getting you know how how is it now for cognitive behavior is it is it worse than it used to be the. Matter of the same or do you would you have a clue. What all the research shows and when you read just about any article on mental health cognitive therapy is recommended It's the most recommended therapy it's the most research therapy that's out there just recently they had some new research stating that for people with mild or moderate symptoms that cognitive therapy works just as well as psychiatric medication for people with no side effects so. You don't know medicine has it's place is a good thing but a lot of times you know changing the way that you think in really you know giving some time to therapy in yourself you can make those changes as well so if they are p. Is only going to make things better it works all the time I mean I specialize in it and I see people get better you know which is which is what you want young people come in and learning tools and move on you know when I look at with somebody like you is I feel like you're a 3rd party like if somebody is having conflicts your 3rd party This is not emotionally attached to them hopefully you know that you could give like in objective point of view is like you know what a well check you know exactly we go to the doctor you know once a year for a well check I think that our you know that our thoughts are probably the most important thing that we have yes yes oh they are a bit of a they affect everything and you just people don't think about what they're thinking you know let me just say you know it's not positive thinking that I teach people because that doesn't last it doesn't mean everybody everybody can positively think and we'd all be well if they worked but it doesn't last it's kind of like to get over the finish line in a race positive thinking works so you really need to figure out what you're thinking and most people when I say to them what were you thinking in that moment most are like I have no idea I just know I felt horrible but your brain never shuts down you have to remember and your thoughts affect your belief system the choices you make the values is because that's the way we think about things and most of my clients once they just start being aware of what they think even though they don't know what to do with their thoughts yet they start to feel better because they say they feel like they're more controlled they're not just reacting to everything in their life based on their moods but they're like oh what am I thinking about is that even true you know most people think is they have a thought that it's true and they don't question it and then they act on it. Wow that's pretty pretty amazing. I believe everybody needs help and people in denial need it worse than the rest of us. I don't know if it's a blessing or not you know but you know you know it's sad isn't it it is because you know most people are going if there are peace because they come in to come into therapy but it's something I've always said you know everybody would challenge their thoughts you're the world would be a different place you know we make decisions on all levels in our world specially even with God for up to government make you know why do you go to war based on how you feel normally not because what are my thinking about you know there are the feelings of they're going to hurt us or you know they did this or that and then people make these huge decisions and I always tell my clients you know you're making life decisions based on thoughts or even true and it's really profound when you stop and think about it and when people say I wish I thought about that more I say to them you didn't think about it at all you felt that he needed something Wow Yeah yeah it makes sense you know. Now I think everybody has a fault in everything I would be a bad person to have the wrong person to have on a death row because I feel like I feel like there would be a reason for that person doing I mean people that lie they actually have a mental problem you don't do that if you I wouldn't think that you would do that if you you know if you look at you know just a criminal system you know they say 80 percent of people who are separated are under the influence when they committed that crime and just using alcohol and drugs there's a reason people use alcohol and drugs and usually because they don't want to deal with the feelings or they can't deal with their feelings or their life I mean there's a there's a much bigger story to everything out there it's not black and white you know what most people don't have the opportunity to get help where they're not willing to go get help actually yeah you know they don't that's half the battle or the out just getting here. One thing one thing there's one saved you know 2 listeners is if you're so one of those people that think coming to therapy is weak you know it's actually a strength. I agree I've been reading this is not getting help because so many people think I'm weak because I need to go get help from someone I should know how and why should you know how nobody has taught you exactly exactly you know better you know better you do better I knew you would. I think I shared with you earlier my sister was in a psychiatric hospital nurse for 25 years you know it's been good to have somebody that you know study the books read the books that has you know but I still she and I both have reached out to professionals because I just I just think oh well check on your thought pattern like you said was it's a smart thing to do yes yeah could you share with is exactly where your practice is at where you work in everything you have here in Irvine in Woodbridge I'm on the corner of rock and call for my behind me me scant faith in Alice restaurant if you're familiar with that and I've had my private practice here for 10 years now I was as you see I medical center for 10 years as well over an orange in the outpatient setting there I did therapy and I did training with the residence teaching and new behavioral therapy. At the u.c. I think you mean that the Medical Center doctors there yeah did you know that for the sake yet you're going to residence Ok you're going to the residency there say the hospital and everything so downstairs is outpatient and so they are with their training I was the one that had a group of cognitive behavioral therapy group and they would sit in. That's amazing Yeah and I have to tell you something else that I heard one time and I think you're going to agree that if you always agree with yourself you definitely need. That. I love the Google alert. I ask you what kind of volunteer work do you do in the community because I know all good business all good businesses are always involved in our community so yes I'm sure that you must be doing something I'm just getting started with families forward I haven't seen anyone yet but that's a really excellent program and so I will be offering you know free services to a family or individual in need and see them as long as they need to be seen and another organization that I'm hooked up with is called Give an hour and that's for the military that if you're in the military or anybody is connected you know family cousins grandparents they have somebody in the military they need you know some counseling that they would call that organization I guess is called Give an hour and then they would find a therapist in the community and I would see them for free as well I understand make a stand that a smart person doesn't know everything but they know where to find to find it out yet that's very true and I think family forwards I'm very familiar with fam I'm very familiar with them and they're located at the police station right yeah they do a lot for their community they are the residents here I understand it's free for them and I think even if you work in Irvine you have access to the family for 4 families families for families forward yeah yeah yeah now it really is a great program. Who could you share with us maybe a little bit about some of the I know that you've been involved Dr fail and I think that this is kind of an exciting thing because I've read his book strategy in doing what matters during my cancer it was a book that really made me realize that hey I did do contribution to things that were done wrong because we all have so how was that experience for you it was it was good it was fun that's what I was working you said at the time and. They had a show called Moms on the verge and these moms were of verbal abuse of to their children and they were like on the verge of becoming physically abusive so he wanted to have therapists there in the office to hook up with each of the I guess he had they were because they didn't want Child Protective Services to step in they knew if they had you know help for the women that. That they want to happen so and it was so the and so he chose me I was told from the producer because of the cognitive behavioral therapy and so I was went to the show you know was you know it was finding I picked up you know in a limo a guy that went up there and you know got all fixed up for the show but it was a good experience and it was a great experience with the woman that I worked with and then I saw her for free and she was definitely a success story I wish that they had a follow up with that show they were supposed to but because she once she went on that show and surrendered in front of really the world that I'm abusive towards my son the things that came into her life were just unbelievable and she did great with the cognitive therapy and she committed herself to come every week and we met for about 10 weeks now that's what I mean you just just realizing there's there's something wrong I mean get help for crying out yeah but you know there's so much there's so much shame and guilt involved that people don't want to say anything or think they're going to be able to figure it out themselves and then once you get worse the one thing you know about cognitive therapy which is really good as well is considered short term therapy so if someone comes in and really works hard you know sometimes 8 or 10 sessions and redone it's not a long term commitment if you don't want it to be and I think one you know people think of you know Sigmund Freud laying on a couch for you know years with he has to be about you know you can come in learn tools and move on with your life now that totally totally makes sense well I really want to thank you so much for sharing the information today how could our listeners reach you do you have a email a phone number of website or anything all of those so.
