Today is the day he promised to have the website running smoothly for americans seeking Affordable Health care insurance. Two months after its launch, how is it going . Jeff pegues is at the white house. Good morning, jeff. Reporter good morning, vinita and anthony. They say its all part of a planned maintenance session. The administration reached their selfimposed deadline overnight after announcing late friday that there would be what they call performance maintenance throughout the weekend. For extended periods of time users would not be able to complete enrollment. But the team charged with fixing the website believes its working better now and will be able to handle at least 50,000 people at one time and 800,000 during one day. But there is still uncertainty about whether it will hold up. An official fill yated with the project tells cbs news im sure everyone is biting on their fingernails. Nearly two months into the troubled rollout the president s Approval Rating has hit a record low. Gallop reportup gallup reports were tied. In an interview with barbara walters, president obama said he isnt too worried. The good thing about doubt is usually you have nowhere else to go but up. Reporter hes optimistic they will succeed. Every president in their second term is mindful, you know, youve about got a limited amount of time left and you want to make sure youre squeezing every last ounce of energy you have to try to deliver on the commitments you made to the american people. Reporter later this morning its expected to end and although the administration believes that the website will be performing better they do ask people not slug the website all at once. Officials say its not a relaunch. Its not a magical date. What exactly can we expect today . Well, i think thats right. Thing they want to keep expectations very very very low. What we can expect is more people being able to sign up getting from the very beginning of the process all the way through to the end in picking a plan but we really arent going to see any real indication if things are mump mump better probably well into next week when there are, if its working lots of stories of people being able to move through this smoothly or if its not working more of these Horror Stories that weve become accustomed to. John, if the website is fixed, does that mean that the system is essentially fixed . No. Thats the crucial distinction. The administration has said that the vast majority of the users will be able to use the website after this fix is made but the websites only part of the issue here. What needs to happen once youve picked a plan is that information has to get translateder of the the Insurance Companies and thats where a big problem has cropped up. And basically what happens is either the Insurance Company get bad information, duplicate information or sometimes there are these things called orphan records where somebodys picked the plan but no information gets to the Insurance Company. Thats a crucial step. The website can be fixed but the system may not be fixed, and so thats something were going to be looking at very closely in the coming weeks. Weve been witness to so many bumps along the way and the day before thanksgiving there was a quiet bump. We quietly heard that the lawn f. Date for Small Businesses could be delayed by a full year. What else could go wrong at this point . There are many things that could go wrong, seems to grow and is unexpected. This is one the administration knew was an issue so they decided to kick the can down the road on that the same way they did near the fourth of july when they basically extended the employer mandate for a year. That was for companies with over 50 employees who had to buy those employees insurance. They said lets put off that mandate for a year. The next one of the next Big Questions will be a political one. The president promised in selling the plan if you liked your doctor, youd be able to keep it. The way some Insurance Companies are working, theyre limiting some of the ak stoes the doctors. Thats something the president is going have to deal with. It doesnt have do with the website but the implementation of the underlying program and there will be some debate on whether those limitations are part of what the Insurance Companies are doing or a result of this law. It will be politically charged, though, because people like their doctors and that will be an area of some political tension. John how much longer do you think they have to get this working or at least the problems behind him before it becomes a significant political problem next year . I sthink its going to be a political problem for the elections next year. I think what the president needs and democrats need are two things. One, they need lots and lots of stories of people signing up. There are still lots of people signing up and people getting on medicaid. There are some good stories is. What they need to do now is match the signups with the states in the federal system. Then the president can say all of the people who were shut out of the system before who were worried and anxious about their Health Care Coverage they no longer have to worry, and thats a good thing in their lives. If he starts talking more about that that could push down some of the bad story. This was a law that was meant to disrupt the insurance system and those disruptions are just going to cause some issues going forward. Thats sort of the way the law was intended. Handling those issues being able to meet peoples fears as they have them, that will be a political challenge for the democrat and precedents in the next year. John dickerson. The white house on friday released a letter mr. Obama sent to congress advising of the change. The plan updates the president s strategy he outlined in 2010 from strengthening as world leader to fighting al qaeda. Joe biden will be invited to south korea next week. Hell try to ease claims. They scrambled two yesterday when they fly fly what the chinese claim as an air defense zone and now there is a warning for u. S. Commercial planes. Lucy craft has more from tokyo. This morning Scottish Police said theyre still treating this as a rescue operation. Two Police Officers one civilian on board, 32 people were injured. Our top story this half hour is one we should all take to heart. Its about a young woman with a disability who decided to take control of her life and did so despite opposition from her own family. In the process she blazed a trail for others facing similar challenges. Her name is jenny hatch, and don dahler has her story so far. Reporter jenny hatch has worked at this virginia thrift store for more than six years. Its my favorite store. Reporter the 29yearold is meticulous making sure the shelves stay organized and the merchandise fashionable. She even runs the register. The store owners Kelley Morris and jim talbert have long been jennys friends. Now they are also her guardians the result of a Landmark Legal battle for her independence. Im so much happy that i live here now because now i can come home and spend time with my friends. Hatch, who has down syndrome wanted to live with morris and talbert. Her mother and stepfather wanted her to live in a group home. The case went to court. Jenny won. Attorney jonathan martinez. We had to change long held ingrained beliefs about what people with disabilities can and cant do. The judge granted morris and talbert temporary limited guardianship or jenny until the 14th. After that its up to her. I dont think jenny need as guardian for every egg in this world. I think, to quote jenny, she just need as little help. Reporter since her court victory, hatch has become an icon for the developmental i disabled. She became part of the justice project. Ryan king is their first client. Everybody need as little help now and then but once youre independent, i think youre good go. Reporter when kimg became 18, his family became his guardian. But his mother says he doesnt need that level of supervision so they took the case to court. We followed jennys story and try not try it ourselves. We tried it before even though we were turned down. We decided to turn it down. Reporter when her guardian guardianship expires next year hatch is Still Deciding what to do power of attorney guardianship, or neither. She has the right to choose. Holly phillips and dr. David agus. This morning were taking a look at Critical Issues related to health care. A serious report with costs revealed that the price of health care in the u. S. Has climbed significantly. But many medical outcomes for patients are falling behind other developed countries. John, give us the headlines here. Well, we spent 2. 7 trillion every year on health care, okay . Thats almost 18 of the Gross Domestic Product and thats a percentage thats doubled in the last 30 years. So we spend a lot of money. We consistently hear the dollar amount were spending but it seems were no healthier for spending that money. No. Actually when you compare us to other developed countries, we do not do very well. We spend twice per person on health care and yet when you look at anything we can measure, were toward the bottom of the measure. We have the heist mortality rate and highest obesity and second for births. If the price keeps going up why arent we getting healthier. Were not focusing on prevention. Chronic diseases are overwhelming all of us. It seem like its going to a lot of young people. You would think it would be going to older people. Why is that . Its one of the things thats really a paradox. As we see now, more and more people are having thiess chronic diseases and chronic diseases cost an enormous amount of money. We need focus on prevention. We need to actually focus on ideas that can delay the onset of the diseases and then were going to have a big impact on health. Holy,elyholly, i want to talk with you about this report. An increasing number of americans dont go to the doctor when they need it and they dont fill prescriptions because they dont feel they need it. It shows 76 of americans dont go with followup care whether its drugs or tests. 37 dont follow through with these things because of costs. When you put it all together this is a pretty bleak picture. Is obama care going to make a difference . It could be a good start. If you have 37 going to the doctor rather than the emergency room, theres going to be that kind of improvement. How do you gel get people insured . Really what you need to do is change the whole system and change the preverse incentives that they have with rewarding doctors. How do you change the system . You know i think that thats one of the things that the Affordable Care act is doing is making change. I think to effectuate a big change, you have to start somewhere. And actually that Commonwealth Fund study showed 75 of americans really wanted to see health care totally overhauled. So even though the Affordable Care act is by no means a perfect plan it is a plan and it represents a change. And then we can tweak it as we go along. You get the sense because this report was written by your colleagues other fellow doctors versus politicians which we normally hear this will get more weight. Is this true . Do we look at doctors as being more . Certainly this is an important step forward. We actually have to focus not just on Health Care Finance but health care. Doctors are speaking up and so are patients. We need to change the quality. One of the great things about the Affordable Care act is it starts to cover all preventive care. Theres no excuse now. Responsibility is with each of us, with each individual. Theres no excuse not to get that prevention. We have to change the culture from the culture of dealing with disease to a culture of preventing disease. Theres an attempt to actually reward doctors, pay them a little bit more if they improve the health of their patients. And so youre going to see this more and more debated. How do you actually measure quality of care . It can be tough. I think its a shift in focus from the Health Care Providers and the parents. They say you spend so much time mopping up the floor, no one thinks to turn off the faucet itself. When we focus on the fiance sult, well focus on prevention and improve Overall Health care. How do you provide incentive . One of the things the Affordable Care act does is you can charge Health Insurance based on behavior. If you smoke or youre obese, you can charge differently. One example that jumps out at me, medicare in the old days wouldnt pay for a podiatrist, say, 150 bucks, to look at the foot of a person with diabetes. Instead they will pay for 20,000 to amputate that foot months and years down the line when you havent done the prevention. I also think ideally its going to provide more funds for Public Health measures for getting messages out how to eat well, 000 exercise what we really need to do to prevent illness. I think getting people out and focusing on it will help. This is obama care 1. 0. And there will certainly be 2. 0 and 3. 0. All the negative controversy has started a controversy. People are saying how do we look ahead instead of not responding . Theres glimmers of home. New york city had a mayor who said if i start to change things what will happen to the health of the cities . What do you know. Rich or poor healthy or not, they got better. We need to change the way were thinking about this top to bottom. Top to bottom. We dont trip over it and say how do we do this . The whole community is helping. I tried to calculate how much writing goes on every day. Its about 3. 5 trillion words every day. Thats a huge amount of writing. Its never happened before. But is it worthwhile writing . Most of it is probably bad but the good stuff floats to the top. We get a new surplus of stuff and we vo to figure out how to get the goingod stuff to the top. We look at what we can rely on. You get a flood of books. What do you do . Invent libraries. Were in the middle of achieving that right now. What about this antisocial question. You mentioned the telephone. Obviously the same argument is being made about facebook that people are onto connecting over the website there but theres not the actual real contact with friends. People say that a lot but its not necessarily true. Any time you do a study, they do a lot of online contact but also offline. People get this idea that because people are communicating more that something must be lost, you know that you have to be losing the facetoface contact, but it doesnt seem to be true in any of it. People who text more hang out with their friends more. When you look at some fields like medicine you want a lot of doctors who have a lot of things in the memory bankle. You dont want a doctor who relies on googling. Definitely what youre seeing now is people have access to a lot more information outside their heads. They reach for it more often. I dont think it actually degrades the stuff you know if its stuff youre actually passion at about. The trick with memory is how to get people to be passionate about it and learn it and get it inside their heads but the outside access, we have more of it and it makes us better thinks. Do you think it makes us smarlter . Absolutely. If you have a quick question to something, ordinarily you wouldnt look it up. People looked at their encyclopedias once a year. You look at google more often than that, so youre actually learning more. Do you think kids are better writer writers . Absolutely. When you just write for your classroom, it feels a little artificial and kids when theyre online, they have an audience. Theyre right 40g more than they ever did before and its often complicate. Thats something that clicked with me. Sure enough it stuff and i had the good fortune of working with a lot of great restaurants throughout the city achlt thenldnd then ultimately danny. We did the same program. I heard you literally begged for your first cooking job. I did. Thats true. I said, ill work for free for you a well and it worked out. You say basically not mump as changed. Youve updated these really traditional dishes. Yeah. In some ways nothing has changed but in some ways a lot of things have. I think a lot of creativity is born out of researches and archiving and going after old recipes well gashlgter around and say lets take these recipes and breathe new life into them. We look at it and see what people have been doing for hundreds and hundreds of years. Not changing it too much is probably a good thing. I think so. We cant do everything the way we did because were in new york city. So we find creative ways. We pourt spin on what ask, you know classic roman fool food. Youre constantly hearing the word foodie everywhere we look. You have i think its word. If you try to describe to an italian what a foodie is. They would be like, theses especially everything.
