What we are seeing in Emergency Rooms are patients are still coming in. This is still the acceleration phase. Its not always what you see on test results versus whats happening in real time, because sometimes the test results take time to come back. Not everyone gets tested. So im glad the curve is flattening, that gives me hope, but there are many parts of the u. S. And many of our listeners that are in areas that are still in an acceleration phase. Harris you know, i do want to say, coronavirus confirmed cases have topped 400,000, not 401. That was an error made earlier. I want to go to you, dr. Nesheiwat. Just on this idea of what Governor Cuomo was saying. That is that he wants to Start Testing particularly people of color, because what we are seeing across the nation in some cities is they seem to be hit by coronavirus more often and worse. He says its important to start to gather that data now. Can you speak to that . Dr. Nesheiwat absolutely. And its heartbreaking to see this Racial Health disparity. Sometimes when im at work and i have patients come in that i know have coronavirus, they are hispanic, they dont speak english, they dont understand the severity of disease come of undiagnosed diabetes. It is definitely something that we need to tackle. We need to look at the root cause of why this is happening. Is it lack of access to health care . Is it cost, is it education . Lack of understanding . Maybe a lack of transportation . All these factors we have to take a closer look at to better treat this population. These ethnic groups. Because we are only as strong and healthy as our weakest link. So, yes, thats a good idea, because i worry about those who are underprivileged and those who are of different ethnicities that maybe dont speak english. Sometimes language is a barrier. So thats a good idea to help reach out to those that may need that health care, that easier access. Its definitely something we should be looking at. Hopefully, though, we will look at having everyone with access to the testing in the care they need. Harris all right, i want to get to viewer questions right away. This is an interesting one. An Infectious Disease expert had this morning last night. But lets get to lisa h first. Lisa h is asking, how long from the time of exposure do people become contagious . Dr. Devi . Dr. Devi so, its unclear. Basically, from the information we have now, lets say you get exposed. Not everybody who is exposed actually develops the infection. But we know it can take up to 14 days, sometimes longer, usually less than five days for the person who got infected to show symptoms. There is some evidence that even a couple days before they showed symptoms they are more infectious. So its hard to know. If you have had an exposure, i would say its best to quarantine yourself or separate yourself for at least 14 days after that exposure. Because you may or may not develop obvious symptoms, but you still maybe able to transmit that infection to somebody else. Harris we can understand when people find out that they were exposed to somebody somewhere, and they are notified of that. Being nervous about that. Thats a frightening thing, when you look at the numbers every day. Yet they are told to wait for the symptoms to pop. Sometimes that weight can be excruciating. I am still curious about what happens during the time you are waiting for that fever to pop. If you caught it earlier, could you maybe save some people . I dont know. Ill move to this. I started with this a moment ago, its ready to go now. There was an Infectious Disease expert on with tucker last night with this morning. Lets watch. We are all obviously occupied and terribly concerned about whats happening right now, but we can be in the first inning of a 9inning game. We got 18 or more months to go unless you got a vaccine. Thats the kind of planning we have to deal with, getting ready for a longterm transmission situation. Harris dr. Bose, you are seeing this patient, youre in the emergency room right now. Your thoughts on what was just said . Dr. Bose this is tricky, because we are in this phase where we dont know a lot. We dont know how many people are just out there that have it. We dont have full availability of testing. So i think the biggest point for the listener is to just assume you have it. Assume you are shedding it. What we are seeing, the viral shedding, its probably not going on in the first few days. May be before you even have symptoms. I think thats the safest move. It seems hard to comprehend, but this isnt new. Even with influenza, you may be shedding the virus before you actually im hopeful for vaccines, im hopeful for what might be coming down the pipeline. For now, we have to fight with the army we have. Harris you know, its interesting, you mention the flu. Our next viewer as this question. It also mentions the flu. Lets watch. What happens next fall when flu season starts back up again . Are they going to shut the world down again, or are they just going to let the virus run its course . Harris dr. Nesheiwat . Dr. Nesheiwat well, specifically for flu, no. See, the difference between the flu and coronavirus, with flu we have experience with it. We have a vaccine, we have antiviral medicine. We know and can predict how its going to behave and how it affects patients. For the influenza virus, i doubt that. I highly doubt that, unless theres a completely new different strain thats going to come about. Hopefully we dont see a seasonal recurrence of coronavirus, but its too early to tell. Thats why we need to continue to take the guideline precautions, safety measures, and get this vaccine out as soon as we can. Harris you know what, i want to follow up to what you are saying, dr. Nesheiwat, with this idea that, come fall, we could possibly see some of this all over again. Of course, we are learning along the way. Now that we know how to flatten the curve, you can do that if we need to. Some other parts of the country wont even pop with their apex of fatalities and confirmed cases until later this year, on into august, even. Youre absolutely right. Thats why its critical to have expanded testing. To look for prevalence of this virus in the community. To look for maybe early signs of outbreaks so we can know where to focus our resources and our efforts to help prevent any huge peaks or massive spikes of thousands of people dying in the state like we have here in new york. Thats why one of the most important things that we can do is focus on having these tests available to everyone. Whether its an antibody test, an oral swab, and nasal swab. In addition to learning from what we are doing now and taking that into consideration for preparedness in the future. I want to get something straight off my twitter before we get to other questions, because its popping to me. I think its a good question. If we have deaths with normal flu, why do so many people die every year from it, dr. Nesheiwat, i know the shot has something to do with it. The flu shot. Dr. Nesheiwat well, the flu shot and vaccines for the most part help save lives. The flu vaccine has helped save thousands of lives every year. Of course its not 100 effective, but the flu can be a very serious disease for some people. Especially if you have underlying medical disease. Asthma, copd, a weakened immune system, if youre a smoker. And it can affect some people who have no health problems, as well. We cant control everything and prevent every disease and virus in this country, but we can at least take precautions. Keep your hands washed and clean, dont rub your eyes, dont rub your nose, get your vaccine. If you are sick, dr. Doctor, because we have medications. Thats part of nature and part of life, that its going to happen. Until we can get a vaccine that can fully, versus 40 or 50 be effective, be closer to 90 or 100 . And we have to focus on whats called herd immunity. The more we can get the population immunized, the less number of deaths we will have. This past flu season it was estimated 22,000 to 40,000 deaths. Two years ago we had about 80,000 deaths. Of the steps, the majority of them were not vaccinated, harris. The majority of those people were not vaccinated. That definitely plays a huge role, in my opinion. Harris if people are making the choice not to get the vaccination, it cant really change herd immunity very effectively. Maybe thats the big question about coronavirus, too. If one pops onto the market and this comes back every season, how do we know people will help build that herd immunity . We dont know. We just know theres a crisis right now. I thought it was an interesting question. I want to get to this, it came up during the White House Task force press briefing yesterday. It was from dr. Fauci on people of color. Watch. When you look at the predisposing conditions that lead to a bad outcome with coronavirus, the things that get people into icus that require intubation and often lead to death, those are just those very corcomorbidities that aredispron the africanamerican population. There is nothing we can do about it right now except try and give them the best possible care to avoid those complications. Harris and of course, dr. Bose, the president spoke on that issue, too. Looking for those commonalities of why people in minority communities fall more often and more tragically to covid19. What do you think . Dr. Bose well, from an Emergency Department point of view, as a fellow of the American College of emergency physicians, we treat anyone, anytime, for anything. When you do look at certain demographics, they do have more challenges. For example, in my personal experience and expense across the country in Emergency Rooms, africanamericans and latinos have higher rates of certain conditions. Debbidiabetes, hypertension, ob. It could be access to transportation, it could be if you look at the quality of care, the cost of care, the access to care. These are things that i am hopeful even in the precovid world, this was a problem. Im hoping that now in the postcovid world we can really tackle those issues and fix them. Harris you know, dr. Devi, we saw this as well with other things we were just talking about influenza. We have seen this in communities of color, as well. Im curious how we, though, change it during a pandemic. What do we need to do . The governor of new york says he wants more testing in those communities. Dr. Devi i think we have to look at our policy so far, and the history of why they might have some of these changes. If we look generally, most people watching this pandemic evolve, they are skeptical about different aspects of it. Like who knew what, when, even starting from china. If you look at the Africanamerican Community in particular, they might have even more skepticism about our health care system. The tuskegee study, for example, that was another Infectious Disease. Syphilis. It was killing people, by the hundred thousands. What happened at that time was the cdc and other researchers and doctors decided to do a study where they looked at how syphilis evolves. Even when there was a cure, like penicillin, these africanamericans who were part of the military, they didnt get it. Instead, the doctors let the people continue to get sick to see what would happen. The cdc justified it by saying this was Critical Research that we needed. I think a lot of africanamericans may be very skeptical seeing all these images of the cdc and these experiments being done. On top of that, if you look at another figure, henrietta lacks, there is an oprah movie more recently, she had cancer and went to Johns Hopkins. We hear a lot about Johns Hopkins, even from the mass we are currently seeing. She went to Johns Hopkins for treatment, and a cancer doctor was also a virologist, ironically. Instead of focusing solely on her care, he actually used her cells to facilitate his research and to make money off of that while she and her family suffered. Harris oh, wow. Dr. Devi i think these historical factors will play in here. Maybe they are contributing to people seeking treatment at a later stage. Not just for coronavirus, but for all these different conditions. I will just add harris i had not heard this part of the feedback as detailed as you were laying it out. Im almost 80 years old, harris, and supposedly the elderly especially elderly males are much more at risk. So maybe you want to keep them quarantined for a while and let other groups that are much less at risk get back to work. So there are a lot of different ways. My, of course, preferred way is to increase economic incentives and let people make their own decisions. Id like to do Something Like waive the payroll tax through december 31st, would you be a huge incentive for people to come back to work as soon as they possibly can, comfortably. And it would be a huge incentive for employers to retain workers and hire new ones once they are available for doing it. Harris thats really interesting. I had read that you wanted to see Something Like that in lieu of the stimulus weve seen, but youre saying even now you could still do it. Certainly with talk of a phase s coming. Perhaps this in lieu of that, until the end of the year. Thats right, harris. It wasnt so much the profanity used by the acting Navy Secretary during that visit to the ship, but his lack of judgment in handling this crisis for the navy. The state of captain Brett Crozier come as a mention, the former Teddy Roosevelt skipper, now rests with the navy brass. Their investigation should be delivered within the week. The defense secretary accepted thomas modlys resignation almost medially after he returned home from guam. He not only had to resign, but is now quarantined having been exposed to the virus on board the Aircraft Carrier. The acting Navy Secretary addressed the crew who cheered the skipper he fired last week. Crew members listened in disbelief as modly attacked the press, cast aspersions on president ial candidate joe biden, and captain crozier, who wrote that letter pleading for the navy to remove the sailors from the carrier amidst driving covid cases. This audio from his profanitylaced rant. It was my opinion that if he didnt take that information, it was going to get out into the public. This Information Age that we live in, he was either too naive or too stupid to be the Commanding Officer of a ship like this. He initially stood by those words but later did an aboutface. I want to apologize to the navy for my recent comments to the crew of the tr. Let me be clear, i do not think captain Brett Crozier is naive nor stupid. I think and always believed him to be the opposite. The chairman of the House Armed Services committee, democrat adam smith, said it was out of character for the acting Navy Secretary he knew. I would a competent, capable veteran who has extensive experience, extensive leadership skills, make what seemed like such an obvious mistake . I think its because everyone over there is trying to figure out, how do i stay in the good graces of the tyrant across the river . Who could potentially far them tomorrow if they dont. I want to bring in a retired fourstar general jack keane, fox news senior strategic analyst and chairman of the institute for the study of war. General, this went all sideways pretty quickly. Im curious, just to begin with you, on how you describe what just happened. Gen. Keane well, first of all, its a very unfortunate situation. Everybody involved here is so wellintentioned and so experienced and had led great public and private lives before this. I think there is no denying the fact that the secretary of the navy made this a very personal issue, harris, right from the outset. As a result of that, it denied him the sufficient emotional discipline that is required to see him through the crisis and use the capable leaders, some four admirals, that were between him and the captain. I think if he had done that it would have gave us a much more reasonable conclusion and result, where the focus would have been remaining on the health of the crews. And not this public fiasco that we have entered into, where the secretary of the navy becomes the story, where he extends the life of the story by granting interviews, writing an oped, traveling all the way to guam. All of this is done to justify what . Why he relieved the captain. I dont think he goes to the Aircraft Carrier if the crew did not respond in such a positive way, in supporting the navy captain. So, here we have a Navy Secretary and adam smith is trying to find out why he lost his way . I just gave you a reasonable explanation of why that happened. Harris yeah, thats really interesting. Always interesting to get the take of someone who has led so many, general, as you have. What should happen next with the captain of the ship, the commander who wrote the letter that leaked to the media trying to say theres a lot of covid19 among the people on the ship . Legitimately looking out for them, but what should happen to him now . The president is saying they are going to take a look. Gen. Keane thats kind of interesting. The captain spoke, as i understand it, to the secretary of the navy days before he wrote the letter paid actually, the secretary of the navy, by the way, wanted to come out there and give him a hand on the ship. The captain waved him off. Thats the last thing he needed, the secretary of the navy giving him a hand on the ship. He also gave him his cell phone number, by the way. That gives you a sense of how personally involved he was. The guy in charge of the navy, uniform 4star, also spoke to him. Also the chain of command. So why did he write a letter on the nonsecure net . I think its because he wanted to go on record. I think he knew full well he was likely taking his career in his hands there. Because something happened in that chain of command that he believed they were not being responsive to him despite conversations he was having with them, and he wanted to put it in writing.
