Hospitalized this morning. That also a record. More than 217,000 new cases. Thats a record. A key model now forecast that the death toll in the u. S. Will double by april to more than half a Million People. And to give you a sense of that number, the city of atlanta has a population of about 500,000. So imagine atlanta wiped off the face of the earth. Thats what the pandemic is projected to do by april. So now, this is what the president elect and Vice President elect have had to say about all of this as part of jake tappers exclusive interview. The u. S. Is entering the dark winter that you warned about with the highest death rate. We just reached a new horrible milestone of new cases. On a policy level, whats going to be different, starting on january 20th, when you take office, when it comes to dealing with the pandemic . Its going to be a couple of things. Number one, its important that we set out national standards. We met with governors, democrat and republican as well as 50 democrat and republican mayors. They say they need guidance. And theyre going to need a fair amount of the money. Its one thing for us to talk about being able to get help out there, but its not getting there. Were having these hospital stays are overwhelming hospitals right now. Theres a need for more Financial Assistance. Theres more Financial Assistance needed as well when the vaccine comes forward. Theres need for planning. And so, now, the administration has been cooperating with us of late, letting us know what their plans are for the covid virus, for how theyre going to deliver on the vaccine. But theres not any help getting out there. Madame Vice President elect, pfizer and moderna have applied for emergency use authorization for the vaccines. Are you confident that, if and when the fda does give that approval, it will be safe and effective and will you take it . Of course, i will. And but we also want to make sure that the American People know that we are committed, the president elect and i talk about this all the time, that the people who need it most are going to be a priority. Weve talked a lot about the need to take care of our front line workers, as the president elect was talking about. Obviously, those front line workers are important, but you are, too, sir. And you will be the oldest president ever inaugurated. Do you plan to get vaccinated before Inauguration Day . And will you do it in public the way that president s obama, bush, and clinton have suggested theyre willing to do . I would be happy to do that. When dr. Fauci says, we have a vaccine that is safe, thats the moment in whichly sta li will s before the public and say, part of what has to happen, jake, and you know as well as i do, people have lost faith in the ability for the vaccine to work. Already the numbers are really staggeringly low. And it matters what a president and Vice President do. And so i think that my three predecessors have set the model as to what should be done. And once fauci says its safe, thats my measure, then obviously we take it. Its important to communicate to the American People, its safe, sa safe to do this. Speaking of fauci, have you spoken with him yet and have you asked him to stay on . Yes and yes. Ive asked him to stay on if the exact same role hes had for the past several president s and asked him to be chief medical adviser for me as well and part of the covid team. So what has to be done so we have to make it clear to the American People that the vaccine is safe when that is determined. And number two, you have to make sure, as he points out, you dont have to close down the economy like a lot of folks are talking about now, if, in fact, you have clear guidance. We talked about masking. It is important that we, in f t fact, the president and the Vice President , we set the pattern by wearing masks, but beyond that, where the federal government has authority, im going to issue a Standing Order that in federal buildings, you have to be masked, and interstate transportation, you have to be masked and airplanes and buses, et cetera. So its a matter of and i think my inclination, jake, is on the first day im inaugurated is to say, im going to ask the public for 100 days to mask. Just 100 days to mask. Not forever. 100 days. And i think well see a significant reduction if that occurs with vaccinations and masking, to drive down the numbers considerably, considerably. Okay, joining us now, we have cnn political analysts, David Gregory and maggie haberman. Maggie is a White House Correspondent for the New York Times. David gregory, did you hear that . Did you hear the sound of that . Its hard to recognize anymore to our ears. That was a plan that was the sound of a plan. It has a beginning and an end. It has shared sacrifice, an ask of americans. I mean, honestly, its been so long since weve heard of a plan on coronavirus, im just wondering what your take on that is . Yeah, ive been sitting with this as well, the idea of a kind of return to normal business, which weve been away from for so long, to look at an administration, an Incoming Administration that has a plan thats facing a crisis just as joe biden did when he was Vice President to president obama in a financial crisis. That they had a plan, they had coordination, that they were asking for the publics health. And we know from back then during the financial crisis and know in this acute crisis how difficult its going to be for the Biden Harris Team to come in under these circumstances during this winter and i think the mask piece alone is obviously so important, because it uses the bully pulpit to say, this can help. Lets all do it. And it puts more pressure on, you know, flight attendants. Ive been on flights where not everybody is wearing a mask, not wearing it properly. And theyre fed up. On the flights im on, theyre not going to go around and enforce it to every person. Maybe this is a president saying, weve got to do this. And having the backup of health officials, who are going to see far more frequently, i think, with a new administration, can make a difference. What struck me, when you first heard that sentence, this is something new, something that broke as part of jakes interview. It seems so simple. But i sat back for a second and thought, wait a second, here. This is so vastly differently. Imagine starting on january 20th, when you have a president and the scientists, and he said dr. Anthony fauci will be there in an even bigger role, who come out every single day and same the same thing and ask americans to do something all together. I dont know if it will work, but i do know,s it is so different. Thats right, john. We dont know if it will work, but we know that it hasnt been tried before. And we know he is not treating this as some kind of a restriction on peoples personal freedom. And hes making clear the limits of a national mandate. But he is framing what he can do in terms of federal buildings as a requirement and is asking the public to take part. And as you say, this has been absent from the conversation over the last year. I can think of literally one time where the president really made clear to the public, and it was at the end of march when he was extending the guidelines, the restrictions related to controlling the virus for a couple of weeks. And he talked about that the country was going to go through a lot of pain. We never heard that again. It could have all been very, very different. So i think that biden is easing the public into its own transition into how the trump team has treated it as to how he, the next president , will. Were not exactly sure what january 20th, the inauguration will look like. It seems hard to believe that President Trump would go to the inauguration. He hasnt signalled that he will. So jake tapper asked joe biden about this. Listen. President trump has not saud if hes going to attend your inauguration yet. Do you think its important that hes there . Youre laughing. I think it would important only in one sense, not in a personal sense. Important in a sense that we are able to demonstrate at the end of the chaos that hes created that there is peaceful transfer of power with the competing parties standing there, shaking hands and moving on. I think thats an important. What i worry about, jake, more than the impact on the domestic politics, i really worry about the image were presenting to the rest of the world. David, john and i have been talking all morning, about the mood, about the temperature coming down. With every question with joe biden, its this sort of steady answer. Its not personal, its not he doesnt take the bait, not that jake was baiting him, but there may have been a way to Say Something snarky there. He doesnt engage in that. And were all going to have to deprogram from the past five years of, you know, not covering a nasty tweet or some nasty rejoinder from the president. But i totally agree with the president elect. And it has nothing to do with whether trump is going to somehow get over his grievances or act like a mature president in that role. Its about how the rest of the world sees us. Its about the strength of our institutions and its about the passage of power. And you know, president obama in his memoir writes pretty extensively about how trump and sarah palin before him understood that they could violate certain norms of public behavior and Political Behavior and it simply didnt matter anymore. I think we want to rye to stop that decescent. And we want to show some of that pageantry. Thats a way to arrest that slide where there are things that matter. I dont have high hopes for trump being able to do that. But i hope theyll be some people who prevail upon him and say, hey, for the sake of, if you want a political future and be listened to, symbols like this matter. Joe biden clearly and consistently has behaved in a different way than twitter wants him to, than some in the progressive wing of the Democratic Party want him to. This is who he is and how he operates. But maggie, this is something Toluse Olorunnipa talked about, hes behaving as a man who knows that he won the election. And theres nothing that would bug trump more than seeing that. Whats our current reporting on what donald trump plans to do on inauguration and how he plans to react to the president elect over the next 47 days . Everyone ive spoken to, john, says they do not expect President Trump to go to the inauguration and, again, we will see what happens. He likes to keep people guessing about his own behavior. So if you see a commodity in that, i think youll see him tease out the possibility that hell go. Most people think its very unlikely, whether or not hes even going to still be in washington is another open question. There are some people who are hoping hell go down to people in his own group who are hoping hell go to maralago in florida for the holidays, not come back. Other advisers ive talked to do not see that as likely. But the current president is doing everything he can to try to suggest that the incoming president is not legitimate and by going to the inauguration, as david correctly said, that would send the signal about the transfer of power. I do not expect donald trump to suddenly decide that this is the moment to care about norms. I think if anything, what we have seen from election day, john, is hes going in the other direction and he is lashing out, after the first couple of days, everyone around him said that he seemed to understand that he had lost. He just wanted to play out the string. That attitude is gone. He has dug in. So well see where it goes in january. Okay. David, maggie, thank you both very much. And stick around. We have many more questions, because coming up, more from cnns exclusive interview with joe biden and Kamala Harris. Their response to the criticism from the progressive wing of the Democratic Party that says that the cabinet is not diverse enough and more. keeping your oysters growing while keeping your business growing has you swamped. you need to hire i need indeed indeed you do. New this morning, president elect joe biden responding really for the first time publicly to pressure to appoint more minorities and progressives to top positions in his cabinet. Biden and Vice President elect Kamala Harris in this exclusive interview with jake tapper also talk about how they will work together. Whats been the Biggest Surprise about working with her . I know you knew her before, through beau. Well, look, there hadnt been any surprise. Because we had a discussion about this beforehand. We are sim pat kco on our philosophy of government and how we want to approach these issues that were facing. So when we disagree, its been just like when barrack and i did. Its in private and weve discussed at length our views on foreign policy, on domestic policy, on intelligence and the great thing is, she has a background in the senate on intelligence, on a whole range of things that will be pertinent to what we have to do. I dont have to tell you that the naacp is suggesting that they are disappointed so far that there havent been more black americans named to highprofile positions, present company excepted, and that jim clyburn said, hes watching your cabinet picks and quote, i can think of at least ten black folks that qualify for every single one. One position that you have yet to announce is attorney general. Do you think given george floyd, black lives matter, the social justice issues. The justice criminal Justice Reform issues that a black attorney general would be the right direction . Look, im going to be announcing the remaining 50 members of the cabinet. The first eight members are the most diverse cabinet anyone in American History has ever announced. There are three white men, there are excuse me, there are three men, five women, there are five people of color, three white people. I mean, this is going to be an incredibly diverse i am going to keep my commitment that the administration both in the white house and outside in the cabinet is going to look like the country. Last month, senator Bernie Sanders told the Associated Press it would be, quote, enormously insulting if your cabinet, the biden harris cabinet, ignored progressive voices. Who would you point to now as a leading progressive voice in the cabinet . Well, were not done yet, jake. So were not even halfway there. So i think that we should have this conversation when were done. What i think people are saying is, a lot of people are saying, am i going to pick some very, very prominent and wellknown progressive who sits in the house or the senate right now . As close as everything is in terms of the house and the senate, they are tough decisions to make. To pull somebody im going to badly need out of the senate, and we now dont reelect or have an appointment of somebody who is a democrat. And so, it is, i think people are going to see not only at the cabinet level, but the subcabinet level, theres already people we have appointed and well appoint many more. But it is not again, i understand the push. Back with us, David Gregory and maggie haberman. Maggie, that was a very interesting discussion. There are two sort of separate issues there. Theres political diversity in terms of progressive versus moderate democrats. But also the issue of racial diversity. And the push is real. The naacp now is concerned that they dont believe the cabinet is as racially diverse as they were hoping. Weve heard from hispanic members of congress, the hispanic caucus who also feels the same way. So there is that feeling out there and the Vice President elect and the president elect in a different part of the interview acknowledges that he knows that there are people but hes saying wait. Is that answer enough . Its the answer for right now and theyre going to continue pressing on and appointing new people and we will see what those additional faces look like as they come into the Biden Administration. But it is true that this is a concern he is going to have to deal with. There had always been predictions that there was going to be a debate between politically the left of bidens party and the Biden Administration, after the glue that had kind of held the wings together was no longer necessary. Trump has been voted out. The election is over. You are still seeing republicans destroy each other, because the president refuses to accept those results. And i think that has given biden and harris a little bit of cover in public, at least to buy some time. But i do think that theyre going to face increasing pressure to make appointments that will satisfy folks. But, david, you heard a little bit of him saying, no good deed goes punished. He didnt say that. Obviously, hes not expressing any sort of resentment or anything, but hes trying to point out that it already, as it stands, is the most diverse cabinet in history. Right. Theres a hint of defensiveness there already. Look, we know that progressive politics, the future of the Democratic Party, and that internal schism, which is not even on hold. You know, i think that there were a lot of progressive who is went for biden because they wanted to tradition away from trump. But that fight is still there. And the role of identity politics in the Democratic Party are Major Pressure points. Because then beyond that, theres, whos the right fit . I think about the secretary of defense job and michele flournoy, whos extensively experienced, who was thought to be a top pick in the Obama Administration was seen as being passed over and she even had the support of senator Lindsey Graham at the time. And thats not happening yet here. So that would be a truly groundbreaking joy ining choice defense. So, you know, theres people are watching and looking at that very carefully, as well. So, yeah, these are big areas of pressure for him, because i think hes got a more resti base than were used to seeing, certainly over the past four years with donald trump and the unique relationship he had with his own base of support. I think one thing he declared about as clearly as youre going to get is hes not going to appoint a senator clearly at this point, because he thinks its too close and raises too many problems. Maggie, its interesting to watch the personal interplay between the two. And what you see in president elect biden, who is a guy who was sitting in the other chair for eight years. And i think really comes at it from that place. I think they clearly seem to have a good rapport, seem to have that good relationship. I think they like and respect each other. As you and i both know, there are always adversarial pressure points between the office of the president and the office of the Vice President. We are a ways away right now from seeing that. So far, they have seemed pretty at ease in each others companies. They have said theyre on the same page on policy. They seem content with each others staffing choices. And this seems to be where its going. The thing i am struck by, john, over and over again with joe biden, is he is a different kind of politician in the sense that the things that make other politicians insecure and i dont mean donald trump who has his own insecurities, but just typical politicians have insecurities in certain ways other politicians might be very upset about how donald trump is acting. Joe biden is clearly not that person. There are benefits to having not just run as many times as joe biden did, but having served as the Vice President. And i think you see that in how they deal with ooeeach other. One more thing i want to say, although ill figure out a way to say this delicately, is that it is also the triumph of old people. Joe biden is 78. He just said that he that Anthony Fauci is going to be his top medical adviser for the next four years. Fauci is 79. I think hes about to turn 80 this month. Janet yellen is 74. In the recent i mean, it was it feels like yesterday that older people were put out to pasture, you know . We thought they had an Expiration Date of some kind. And they are sort of showing how vital and important their vast resource of you know, expertise, is, still. David, youre the grayhaired person on this panel. I was just wondering if that was the delicate way of putting it, how the indelicate way was. I think its a good point. They are vital voices in their areas of expertise, and i think that goes to the previous point of the role of Kamala Harris. You know, heres somebody in joe biden, as maggie said, who has you know, hes got different strengths and weaknesses and hes wanted to be president for a long time and hes been in washington so long and now finally hes got the keys to the car. Hes got some views and ways of doing things that he wants to see done. But he also understands that theres a generational break here. That he can be so well served by a Kamala Harris ain that role hs somebody who can provide advice and can be a liaison on the hill to, you know, different parts of the Democratic Party, because she herself may be a big part of the future of the Democratic Party. So the thing to watch is, what is that interplay like . Biden wanted to be the last person in the room to speak to the president on a major decision. Do they have relationship . Thats something that will be curious to see as we move forward. I think the way maggie frames it, that joe biden has different insecurities or not the same insecurities that were used to seeing in politicians s is so true. Its not that he doesnt have them, but theyre a different set. One other interesting thing is we still dont know what Vice President elect hars will be in charge of. We think they have a great working relationship, but they havent said yet that theyll run this, that, or the other thing. Well talk to jake how she responded about that. Maggie, david, thanks so much for being with us. Jake tapper will join us in our next hour to talk about his exclusive sitdown interview. We have many questions for him. So the United States smashing every coronavirus record overnight. What can americans do today to turn this around . Dr. Sanjay gupta answers our questions, next. [announcer] your typical vacuum has bristles that can leave cleaning gaps and wrap hair. So shark replaced them with flexible power fins to directly engage floors and dig deep into carpets. Pick up more on every pass with no hair wrap. Shark vertex with duoclean power fins. When dr. Fauci says we have a vaccine that is safe, thats the moment in which i will stand before the public and see that, look, part of what has to happen, jake, and you know as well as i do, people have lost faith in the ability of the vaccine to work. Already, the numbers are really staggeringly low. And it matters what a president and a Vice President do. That was president elect joe biden, telling cnns jake tapper that he will roll up his sleeve for a Coronavirus Vaccine when dr. Fauci says its time to do so. He will join three past president s who are pledging to do the same thing. Thursday was the worst day yet of the pandemic. The u. S. , again, smashing every record for reported deaths, hospitalizations, and new cases. A key model that the white house relies on, that we report on a lot is now forecasting the death toll in the u. S. Will nearly double to more than half a Million People by april. Joining us now is cnn chief medical correspondent, dr. Sanjay gupta. Sanjay, good morning. Great to see you. And so, the vaccines are coming and yet, the death toll will be doubling by april. Explain that. Well, i mean, part of the issue is that its going to take some time for these vaccines to get out there. You know, weve been talking about celebrating the idea that theres probably going to be an authorization of two vaccines. But you know, its one of these things that typically you would build up a supply of the vaccines and then start distributing them over a period of time. Theyre going to distribute as quickly as possible. But for the vast majority of people, theyre really not going to be able to get vaccinated until next summer. Thats part of the issue wit, b youve got to conceptually think about how vaccines work in the general population. You try to vaccinate more and more people, and that makes it harder for the virus to find a willing host. Vaccinated person, vaccinated person, unvaccinated person. As you get more and more people vaccinated to that what we call herd immunity, 60 to 70 , thats when you start to see the dramatic impact of vaccine on a society. And thats really, again, perhaps june, perhaps july until you get that. It takes a while to get vaccinated and another week or so to actually become immune. When i looked at the models, there is some impact of the vaccines at about 30 of the country being inoculated, but it doesnt happen right away. Its not like a Surgical Strike on this, as would be masks or stayathome orders, things like that. In fact, the model gives a hard number as a projection. If the vaccine is rolled out really well, they say, 11,000 lives will be saved by april 1st. And i dont want to say just 11,000 lives. Thats a lot of people to be saved, but may not be as many as people thought by april 1st. They do say that increased mask wearing could save more than 60,000 lives by april 1st, which just goes to show how important mask wearing is, and comes at the same time that we saw the president elect, sannier, say that hell ask all americans for a hundred days to wear masks. What kind of impact do you think that has . We have plenty of evidence on this. And ive always been the first to say, were learning together over this last year about all of these things, including the benefit of masks on a Large Society in the middle of a Novel Coronavirus pandemic. We didnt know for certain when people when it became clear that people could spread this, even if they didnt have symptoms, i think the idea of masking became very clear. But ive looked at this data glob globally. Ive looked at this data nationally, hyperlocally. Weve seen that in places that got into trouble in arizona and delaware, once they put mask mandates into place, you had a pretty dramatic change. I can show you columbus, georgia. This is another place that got into trouble. They had a mask mandate that went into place at one point. And you saw how the numbers of daily cases came down for a period of time. They took the mask mandates off and the cases went right back up. It is very clear now in communities and across nations that that blue line in the middle is where they took the mask mandate off. Thats the one to really Pay Attention to. And within a few weeks, you see it going back up. In the far left, you see the numbers coming down. Its a much more directly and quickly impactful sort of thing. So the 100 days is a little bit of a political thing. The first hundred days of office. You would see impact as we see in all of these models within a month, within a few weeks. Sanjay, the New York Times has this interactive exercise you can do where you plug in your details of your age and your health and where you are in line. Im 296th million in line for the vaccine. 296 Million People are in front of me, okay . But thats okay, because if i have to wait until june, the case levels are still going to come down. Its still going to benefit me that people are starting to be vaccinated this month. Right, right. And ill give you my place in line, for sure, in case im ahead of you. I dont even know that i am. But once you get to that 60, to 70 , or even earlier, you are going to see impacts. I hope people can understand this visual. We talk about herd immunity. Its the idea that people around me protecting me. Ive got enough people around me who are vaccinated, thats going to protect me. We think about that a lot with children, and actually the term comes from livestock. There is benefit to you getting the vaccine, but also benefit to you from those around you getting the vaccine. Obviously, decreases daily infections, decreases likelihood of hospitalizations and deaths, but also protects the herd. You better not let sanjay give you spot in line if hes ahead of you. Hes a brain surgeon do you think sanjay has given im not saying we dont need you, but we need sanjay more. I love alisyn. Thank you, sanjay. Thank you. You see john elbowing us both out of the way. Hell take your spot. Sanjay, so dr. Fauci did immediate with president elect joe biden yesterday. What do we know about that meeting and what the future looks like for dr. Fauci in the Biden Administration . I spoke to dr. Fauci right after he had this meeting with the action review team, its called. Its a Transition Team where they start to go through what is known institutionally and how to apply that Going Forward. Dr. Fauci met with this new coronavirus administer. David kessler, to name a few. So there were several people on the call. Hes been asked, as you know, by president elect biden to stay on as chief medical adviser and continue to sort of be the head of Infectious Diseases for the country. It was interesting, you know, when i was talking to dr. Fauci, he reminded me that hes been through you know, hes worked for six president s, hes been through five president ial transitions. And he said, quote, this transition process at this point now is exactly the way that transitions should happen. It sounds like theres a real sharing of knowledge thats going on. Obviously, people who will be transitioning from one administration to the other, who carry a lot of constitutional knowledge. So he seemed quite pleased with how things were going. You know, i know there are a lot of people who want to be a fly on the wall for a conversation between dr. Anthony fauci and sanjay, but hell get that chance tonight. Because dr. Anthony fauci and sanjay will join Anderson Cooper to answer your questions. A new coronavirus town hall at 9 00 p. M. . Sanjay, do you like that photo of yourself in that promo. You know, ive actually asked for it to be changed several times. But no ones really listening to me on these things. Because im also lobbying for it to be changed. You look a little nefarious in it. Right. Apparently its the only one where im actually staring at the camera. I have a tendency to move my head. Thank you for the backstory. I think it looks like youre hiding something, which makes it more interesting. I like it. Sanjay, well see you next hour if you agree to come back. Okay. Are President Trumps challenges to overturn the Election Results all about raising millions of dollars . Quef we have a reality check, next. I didnt know you were listening. Why is President Trump fighting so hard to stay in the job of president when he doesnt seem to be enjoying it anymore . Maybe he has a different motivation. John avlon has our reality check. As our nation suffers through the surreal spectacle of a president refusing to accept Election Results, while some of his supporters threaten Election Officials because they believe baseless conspiracy nears more than established facts, only one thing could make the situation even more twisted. If it was all a grift. Now, trumps true believers would never believe theyre being played this way. But theres plenty of evidence to suggest it. Get this, the Trump Campaign sent out nearly as many fundraising emails in the three weeks after election day as the three weeks before. Some 400 emails, 125 texts, essentially demanding supporters donate to an election defense fund, bringing in more than 170 million in four weeks. Now, if the term grift seems unfair or unkind, you might want to read the fine print. Because it says that most of your donation doesnt go to the recount effort at all. It goes to a new Political Action committee that can use your money to pay down any Trump Campaign debt, finance trumps future political adventures, including staff and travel, and a possible 2024 run. You ha you would have to give 5,001 before 1 goes to the recount ever. No wonder the president hasnt conceded. Turns out the business of losing can be very lucrative. But wait, theres more. A new lawsuit alleges that the Trump Campaign funneled more than 170 million through firms headed by former Campaign Manager brad parscale. The accusation is that it was effectively laundering the money to mask payments to contractors and advisers, including president ial daughterinlaw, lara trump, and don jr. s girlfriend, kimberly guilfoyle. The best is yet to come yes, it is. Because we havent even gotten to the 800 million the New York Times reports the campaign torched in 18 months, including 11 million on super bowl ads and six figures on a copy of don jr. s book trigger, which helped propel it artificially to the top of the best seller list. As republican consultant mark murphy says, you could literally have ten monkey with flame throwers go after the money and they wouldnt have burned through it as stupidly. This is just par for the course in trumpland. A california emergency room doctor admits he faced an ethical dilemma while treating a possible coronavirus patient who had swastikas and other symbols of hate tattooed on his body. Joining me now is that emergency room physician, dr. Taylor nichols. Dr. Nichols, thanks so much for being with us. So this patient comes into your er with difficulty breathing, he has multiple nazi tattoos on his body and he says to you, dont let me die, doc. So youre jewish and not only that, your whole team is racially diverse, so what was this moment like for you . Yeah, thanks so much for having me. So it was a difficult situation obviously, i mean, the patient was particularly ill, and in those circumstances we really dont have time to talk to the patient all that much. I face these situations a number of times before so far in my career and some of my favorite conversations with patients have been just about that, about talking to them about their tattoos, about how theyre feeling and really being able to sort of lean into my compassion for them and make sure i take the best care of them that i could. With this patient i really didnt have an opportunity to talk to him, i was only left with the impact that that symbol had on me, which is, you know, its a symbol of hate and its it challenged me a bit. It challenged me in a way that i didnt really expect and i think part of that is because of the stress that were all under right now and i know that other members of my team felt similarly. Yeah, as you say, youve had situations like this before and you say you have a mantra that you tell yourself whenever your empathetic core waynes. What is that mantra and why did it feel different this time . Whenever ive found myself in an ethically difficult situation ive leaned into the fact that im the doctors there, im there to take care of them, im not there to judge them and im not there to make assumptions, im just there to do my best by everyone who comes through our doors and thats always been the goal, thats always been easier to do when youre not facing the same sort of stresses that were under now. You know, you tweeted this, which i have to say really struck me, you wrote, the pandemic has worn on me. And my mantra isnt having the same impact in the moment, all this time sold ring on against the headwinds, gladiators in the pit and i realize that maybe im not okay. What do you mean when you say that, im not okay . Yeah, i was struck by the impact that this had on me, that ive never really faced that kind of hesitation before in my career. You know, i felt fortunate, though, that i was able to recognize that, that i was able to recognize that this has really worn on me and that may be im not doing as well working through this as i normally do, that i pride myself on, that i want to be able to provide towards all my patients, reach deep into that well of compassion. And i felt fortunate having that moment, but i know that not all of the other frontline workers are having that experience, and so my fear is about, you know about our Mental Health and how were all doing right now. Talk to me about that. How is this wearing on you . How is it wearing on the people you see around you . I know all of us are under a lot of stress. Its been frustrating at times to know that were approaching the finish line as vaccines are starting to be rolled out, we see the finish line in sight, and we know that if we just all lean into this together we can get through it and we can save lives and so every every case, every death is just that much more heartbreaking at this point. Do you ever let yourself think about what if the situation had been reversed, what if this guy with the nazi tattoos was the doctor and you were the patient, what he would have done . Yeah, i had that thought sort of fleetingly. I would say that when we go into this job we all know that were going to do our best by everyone and were going to provide the best care to anyone who comes through our doors. Its sort of a mantra of emergency medicine is anyone, anything, anytime. That well take care of them. Thats what were here for. And we will continue to do that no matter what. And so i know that whether the roles were reversed or not, i would expect the best care to be provided and i know that we will do that Going Forward regardless, but i just really hope that everybody does lean into their compassion and really know the impact that this is having on all of us. Dr. Nichols, thanks so much for being with us and sharing this story. I really appreciate it. Absolutely. Thanks so much and stay safe and be well.
